The authors describe a case of avulsion of the long head of the triceps in a patient with kidney transplant. An analysis of the case and a review of the literature suggest weakening of the osteotendinous junction due to the associated action of renal osteodystrophy and cortisone therapy.

Authors present the parts of the shoulder that can be represented by ultrasound. Beside the rotator cuff of accentuated significance (within this, first of all the tendon of the supraspinatus muscle) the subacromial bursa, the tendon of the long head of the biceps, the muscle-tendon junction of the infraspinatus muscle are analysed. It is stated that the ultrasonography is suitable for a reliable representation of the soft tissues, surrounding the shoulder joint, further in possession of literary data they see possibility for a very exact judgement of the pathological alterations, too.

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association1992eng
Two patients were found to have superior labrum anterior and posterior (SLAP) lesions in association with complete tears of the long head of the biceps. Additional intraarticular pathology (retained biceps stump, loose body, and glenoid chondromalacia) was discovered in one patient. We recommend arthroscopy of the shoulder in two situations associated with rupture of the long head of the biceps: (a) in patients with acute ruptures in which the decision has been made to do a biceps tenodesis; and (b) in patients with persistent longstanding shoulder symptoms following nonoperative management.

In 148 patients with impingement lesion type I or type II, we performed an arthroscopic subacromial decompression (ASD). 122 patients ran a follow up one to three years post-operatively. All patients were pre- and postoperatively documented by a 100 point shoulder score. The mean score was 57.9 (+/- 11.5) preoperatively. Postoperatively there was a significant increase to 80.7 (+/- 17.9) (p < 0.05). 15% of the patients with a postoperative score less than 70 points were determined as failures. Patients with a preoperative pain history of more than one year had a significantly worse result (79.

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association1991eng
Thirty-one patients with arthroscopically documented partial thickness rotator cuff tears treated by arthroscopic debridement of the lesion were retrospectively reviewed. The patients had had symptoms for an average of 20.5 months prior to surgery. Twenty-two of these 31 shoulders also had bursoscopy, with 18 having arthroscopic subacromial decompression. The results were graded by both the UCLA Shoulder Rating Scale and Neer's criteria. Twenty-six (84%) of the patients had satisfactory results with the remaining 5 (16%) patients having unsatisfactory results. A classification system for the s

A cadaver study was performed to determine the effect of arm position and capsular release on rotator cuff repair. Artificial defects were made in the rotator cuff to include only the supraspinatus (small) or both supraspinatus and infraspinatus (large). The defects were repaired in a standard manner with the shoulder abducted 30 degrees at the glenohumeral joint. Strain gauges were placed on the lateral cortex of the greater tuberosity and measurements were recorded in 36 different combinations of abduction, flexion/extension, and medial/lateral rotation. Readings were obtained before and aft

The complex anatomy of the shoulder often impedes the orthopedic surgeon's evaluation and understanding of injuries to the shoulder joint, particularly the rotator cuff muscle-tendon complex. Arthroscopy, MRI, and digital data analysis are several techniques now at the surgeon's disposal. This article reviews the application of these techniques in the evaluation of rotator cuff injuries.

Abnormalities of the long tendon of the biceps brachii muscle commonly accompany other lesions about the shoulder, especially rotator cuff rupture, and are a frequent cause of a painful shoulder. The spectrum of abnormalities associated with medial displacement of the biceps tendon seen at magnetic resonance (MR) imaging was reviewed in six patients. MR imaging was performed on a 1.5-T system with use of a dedicated surface coil. The findings at MR imaging were correlated with those at surgery. Two types of medial displacement of the biceps tendon were identified. More frequently observed was

MRI is an accurate technique for diagnostic evaluation of a broad spectrum of shoulder pathology. In particular, rotator cuff injuries are a frequent cause of pain and disability. The mechanism of injury to the supraspinatus tendon usually begins with microscopic tears of fibers of the triple helix collagen molecule. This leads to increased motion of the bound water within the collagen molecule that lengthens the effective T2 of the tendon, allowing short TE imaging sequences to detect signal in abnormal tendons. Clinically significant disease probably does not occur without frank collagen rup

The demands placed on the shoulder girdle during athletics can often exceed its physiologic limits and result in significant injury. The goal of orthopedists, therapists, trainers, and ancillary personnel who care for athletes involved in sports which require extreme demands of the shoulder is to enhance athletic performance, extend their longevity, and prevent injury. With the new concepts available from basic science and clinical research, a clear understanding of normal architecture and biomechanical function of the shoulder girdle is better appreciated. A greater understanding of sporting

The application of dynamic integrated EMG and motion analysis to the shoulder has improved the understanding of dynamic shoulder biomechanics during athletic activities and rehabilitation protocols. These studies exemplify a symbiotic merger of basic science research and clinical application. EMG and motion analysis has produced specific objective quantifiable data concerning the muscles in and about the shoulder during normal planar motion, athletic motions, and rehabilitation exercises. This expanded understanding of intricate muscular interrelationships during athletic activities and rehabi

Treatment of the athlete's shoulder is one of the greatest challenges facing the orthopedic specialist. Surgical procedures, and in particular, the open surgical techniques presented in this article, are always a last resort of treatment. The primary treatment is always nonoperative with a particular emphasis toward physiotherapy. In selected instances in which patients have failed rehabilitation, the techniques that are presented can achieve a relatively high degree of success if particular attention is paid to the specific indications, approaches, and details of each technique. Open surgical

Fifteen asymptomatic volunteers and 300 patients with suspected lesions of the rotator cuff were examined with T2*- and proton density-weighted gradient echo (T2*-Rho-GEI) MRI sequences (500-700 ms/30 ms/30 degrees) with a 0.5 T (GE-CGR) system and an adapted surface coil. For the patients, the findings were compared with those of arthrography in 130 cases and of surgery in 42 cases. For the operated patients, T2*-Rho-GEI and arthrography allowed correctly diagnosing 31 complete ruptures (CR). However, surgical exploration of the ruptures showed that T2*-Rho-GEI showed the actual extent of the

The purpose of this study was to investigate the plain film finding of dysplasia of the lesser tubercle of the humerus and its relationship to medial dislocation of the tendon of the long head of the biceps brachii muscle as diagnosed by shoulder arthrography. Of 55 patients referred for arthrography of the shoulder because of undiagnosed shoulder pain, 12 demonstrated flattening of the medial wall of the bicipital tendon groove. Of these, 58% had medial dislocation of the biceps tendon, and 43% of patients with dislocation of the biceps tendon were also shown to have a tear of the rotator cuf

The anatomical basis of the application of ultrasonography to the examination of the shoulder joint and the diagnostic information gained by the technical possibilities of it are discussed. The author claims that by means of ultrasonic examination it is possible to judge the fact and localization of the rotatory cuff, the lesions of the tendon of the long head of the biceps muscle and inflammation of the joint, as well as certain lesions of the soft parts. In addition, it is possible to form by means of ultrasonography an objective judgement of the result of reconstructive joint operations. Th

Bilateral percutaneous muscle biopsies of the triceps surae were analyzed in 12 athletes who had sustained a one-sided subcutaneous rupture of the Achilles tendon while practicing their main sport. No necrosis, atrophy, or significant fiber grouping or regeneration was evident. The soleus muscle in these athletes was composed of approximately 70% of type I fibers in both the noninjured and injured sides, and no significant differences were detected in their histochemical composition. Fiber areas were within described values and not significantly different between the two sides. The noninjured

The specific repetitive activity of water polo, like baseball pitching, emphasizes adduction and internal rotation. This study used the Cybex II to evaluate the isokinetic strength of the rotator cuff in elite water polo players and in a group of control subjects. The water polo players were significantly stronger than the controls. Of greater importance was the confirmation of imbalances in the rotator cuff force couples of adduction/abduction and external/internal rotation. These changes are similar to those reported for pitchers. The adductors in the water polo group had gained in relative

The functional and anatomical consideration of the shoulder joint in relation to the clinical aspects can differentiate the general term of "scapulohumeral periarthritis". The most frequently pathological findings are seen in the rotary cuff and the long head of the biceps muscle. Significant causes are further arthrosis of the acromioclavicular joint, the instability of the shoulder joint and the frozen shoulder. The exact diagnosis of the disease makes on operative therapy possible.

The work presents the outcomes of clinical, radiological and mathematical analyses of 42 patients with the rupture of the whole width of the cuff of shoulder rotators treated operatively. The group comprised 34 male and 8 female patients, the average age 55.5 years. The mean period between the operation and final clinical evaluation was 23 months. According to the method chosen for the evaluation of the results by means of 100-point score acceptable result was achieved in 90 per cent of cases (excellent in 64 per cent, good in 26 per cent). Double t-test showed that significantly participating

Spontaneous or traumatic ruptures almost always occur in a cuff that has degenerated due to overuse. Despite the lack of absolute anatomico-clinical correlation, two clinical types can be described: a conflictual, often hyperalgesic shoulder with its painful arch corresponding to a small anterosuperior rupture, or a pseudo-paralytic shoulder betraying a major posterosuperior rupture. Standard radiography with Leclercq's manoeuvre is performed primarily to find out whether the cuff is continent or incontinent. Modern medical imaging (ultrasonography or MRI) adds precision to the lesions when su
