Long head of the biceps tendon pathology is a frequent cause of anterior shoulder pain. Tenotomy is technically simple but may lead to cosmetic deformity and muscle cramping, whereas tenodesis better preserves the biceps contour but involves a more complex procedure. Multiple surgical techniques and fixation methods are available. No single technique has emerged as clinically superior, and no gold standard has been established. Among the various techniques and fixation sites, subpectoral placement within cancellous bone has been proposed as a favorable option for long head of the biceps tendon

INTRODUCTION: Pathology of the long head of the biceps tendon (LHBT) is commonly treated with either tenotomy or tenodesis, which provide comparable clinical outcomes but have distinct disadvantages. A novel self-locking biceps tenodesis aims to combine the benefits of both techniques. Comparative evidence in patients without full-thickness rotator cuff tears is limited. This study aims to compare the clinical outcomes and complications of self-locking biceps tenodesis and biceps tenodesis with 360° suture anchor in patients without full-thickness rotator cuff tears.

Robotic-assisted surgery has gained prominence in orthopedic practice due to its precision, enhanced visualization, and minimally invasive nature. While robotic applications for orthopedic surgery are well established in arthroplasties, their use in soft-tissue surgery remains limited. This study presents the indications, technique, and clinical outcomes of robotic-assisted latissimus dorsi tendon transfers for shoulder reconstruction. The robotic approach allows accurate identification of the axillary and radial nerves, minimal soft-tissue trauma, reduced bleeding, and excellent cosmetic outc

OBJECTIVE: The olecranon medial flip osteotomy offers adequate exposure to the posterior elbow, while preserving the continuity of the triceps extensor complex and sparing the innervation of the anconeus muscle. It avoids soft-tissue injury due to strain and aids in protecting the ulnar nerve by medially reflecting the long head of triceps.
INDICATIONS: This surgical approach allows the exposure required for total elbow arthroplasty and concomitant radial head excision.
CONTRAINDICATIONS: Patients who underwent previous surgical procedures to the triceps tendon insertion.

BACKGROUND: In relatively young and active patients without glenohumeral arthritis, massive irreparable rotator cuff tears (MIRCT) can be treated with tendon transfer. Arthroscopy-assisted lower trapezius transfer (LTT) for MIRCT needs an allograft such as the Achilles tendon, to bring its attachment to the greater tuberosity. The availability of allografts is limited, particularly in developing countries. The objective of our study is to describe the functional outcomes of arthroscopy-assisted lower trapezius transfer with the peroneus tendon autograft in MIRCT.

BACKGROUND: Rotator cuff (RC) tears are among the most common and disabling musculoskeletal conditions of the shoulder. When conservative management fails, arthroscopic rotator cuff repair (ARCR) is performed. Some patients, however, report persistent shoulder pain following the surgical procedure. To improve patient outcomes, it is essential to identify factors associated with persistent pain following ARCR, which is the aim of this scoping review.
METHODS: The scoping review followed the Joanna Briggs Institute (JBI) methodology and Preferred Reporting Items for Systematic Reviews and Meta-

Falls are a leading cause of injury-related hospitalization, morbidity, and mortality in older adults, with impaired postural control serving as a key predictor of fall risk. The triceps surae, and particularly the soleus, plays a central role in maintaining upright stance by generating continuous plantarflexion moments that stabilize the body's center of mass. This mini-review summarizes evidence for the neuromechanical contributions of the soleus to postural stability and how these functions decline with age. Mechanically, the soleus acts as a brace for balance, providing sustained torque th

BACKGROUND: Alterations in shoulder muscle thickness and strength are believed to be associated with rotator cuff (RC) tendinopathy. Previous studies have mainly focused on measuring the supraspinatus tendon thickness in relevant patients with shoulder pain. This study examined differences in shoulder muscle thickness and RC strength in the symptomatic and asymptomatic shoulders of patients with RC tendinopathy.
METHODS: Thirty-five patients with unilateral RC tendinopathy were included in this study. Muscle thickness of the supraspinatus, infraspinatus, deltoid, and combined middle trapezius

PURPOSE: To compare the biomechanical effectiveness of superior capsular reconstruction (SCR) using a thick porcine dermal xenograft versus a standard human dermal allograft (HDA) in restoring shoulder stability and motion in irreparable rotator cuff tears.
METHODS: Eight cadaveric shoulders were tested in four sequential conditions: intact, massive rotator cuff tear, SCR with xenograft, and SCR with single-layer HDA. Testing was performed at 0°, 20°, and 40° of glenohumeral abduction with 30° of external rotation under simulated balanced and unbalanced muscle loading conditions. Outcome meas

Rotator cuff (RC) tears are commonly associated with progressive muscle atrophy, fatty infiltration, and fibrosis, which often persist despite successful tendon repair and remain major barriers to functional recovery and structural integrity. These degenerative muscle changes are not sufficiently addressed by tendon-focused surgical approaches and contribute significantly to poor clinical outcomes and high retear rates. In response, biologically targeted therapies have emerged to directly modulate the cellular and molecular mechanisms underlying muscle degeneration. Current strategies focus on

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is a well-established replacement strategy to treat irreparable massive rotator cuff tears and cuff tear arthropathy. In patients with forward (FF) deficits combined with a loss of external rotation (ER), RSA with latissimus dorsi transfer (LDT), which was initially used for brachial plexopathies, has been addressed to restore both functional deficits. The aim of the study is to assess mid-term clinical and radiological outcomes of concomitant RSA with LDT in patients with significant preoperative external rotation lag greater than 30 degr

Rotator cuff tears (RCT) cause pain and disability in adults. Histological studies indicate that the degenerative process following a rotator cuff tear may disrupt the vascularity and energetics of the shoulder muscles. Magnetic resonance imaging (MRI) postcontractile blood oxygen level-dependent (BOLD) response and 31phosphorus magnetic resonance spectroscopy (31P-MRS) offer methods to noninvasively assess muscle microvascular function and energetic status in vivo. This study aimed to evaluate the postcontractile BOLD response and 31P-MRS as potential markers of microvascular function and ene

Arthroscopic rotator cuff repair (ARCR) generally improves clinical outcomes, but the impact of pre-existing glenohumeral osteoarthritis (GHOA) severity on these outcomes remains unclear. This study aims to evaluate the correlation between preoperative GHOA severity, assessed via the Shoulder Osteoarthritis Severity (SOAS) score, and postoperative patient-reported outcomes (PROs) following ARCR with a minimum of two years follow-up. In this retrospective cohort study, 150 patients who underwent ARCR between 2018 and 2023 were included. Preoperative GHOA severity was assessed independently by t

Rotator cuff tears (RCTs) often require surgical intervention; however, surgical outcome is frequently insufficient due to poor healing of the tendon-bone enthesis, leading to high retear rates. Previously, the recombinant human amelogenin protein (rHAM+) regenerated injured or diseased musculoskeletal tissues including ligaments and osteochondral injuries. We therefore hypothesized that rHAM+ can also induce regeneration of the biomechanical and structural properties of the enthesis. The right shoulder infraspinatus tendon of 75 rats was fully-transected, and treated either with 0.5 mg/mL rHA

BACKGROUND: The proximal common tendon (PCT), formed by the semitendinosus (ST) and long head of the biceps femoris (lhBF), represents the primary origin of the posterior thigh muscle complex and a frequent site of injury. Previous studies have reported inconsistent descriptions of its morphology and limited quantitative data. The present study aimed to characterize the anatomical variability and morphometry of the PCT and to propose a classification of its structural patterns.
MATERIALS AND METHODS: The PCT region was dissected in 61 formalin-fixed adult lower limbs (32 left, 29 right) obtai

Arthroscopic-assisted lower trapezius transfer (LTT) has gained in popularity as a technique to restore function and improve pain in large-to-massive irreparable posterosuperior rotator cuff tears. Surgeons who have adopted this technique have variable indications. While most agree that there is a limit of arthropathy, other factors appear to be contentious. Age, teres minor integrity, and degree of external rotation deficit remain debatable in terms of indications and contra-indications for LTT. The biomechanical narrative around LTT is sound and has been proven. Surgeons have recently raised
