Quantitative Ultrashort Echo-Time MRI to Assess In Vivo Rotator Cuff Tendon Quality.

Rotator cuff tears are a common source of pain and disability. Surgical treatment outcomes can be influenced by tendon quality, but conventional MRI does not allow for objective assessment of tendon degeneration. This work aims to develop and evaluate quantitative ultra-short echo-time (UTE) MRI methods to assess rotator cuff tendon quality in vivo. Followed by a validation study using bovine tendons, five volunteers, twelve patients with rotator cuff surgeries, and twelve age- and sex-matched healthy controls were scanned on a 3 T GE Signa Premier scanner. For the patients and control subject

Usefulness of low dose (LD) CT arthrography (CTA) of the shoulder in diagnosis of rotator cuff pathology and SLAP lesions: comparison with standard dose (SD) CTA and correlation with MRI.

OBJECTIVE: The diagnostic efficacy of LD CTA for rotator cuff pathology and labral lesions of the shoulder was compared to SD CTA. MATERIALS AND METHODS: Two radiologists analyzed 60 shoulder CTAs with SD (n = 25) and LD (n = 35) protocol with suspected rotator cuff tears (RCT) or labral lesions. Qualitative evaluation of images regarding contrast and noise was done twice, using a 5-point Likert scale. Quantitative evaluation was done, by measuring HUs of intra-articular contrast, tissues, background noise, CNR (contrast-to-noise ratios), and FOM (figure of merits). The presence/extent of RCT

Effect of prior rotator cuff repair technique and timing on clinical outcomes of reverse shoulder arthroplasty.

AIMS: Reverse shoulder arthroplasty (RSA) after prior rotator cuff repair (RCR) is commonly performed for recurrence of pain and arthritic progression; however, the influence of prior RCR timing and technique on outcomes of RSA has not been adequately explored. Our primary aim was to evaluate the relationship between the postoperative American Shoulder and Elbow Surgeons (ASES) score, whether prior RCR was performed open or arthroscopic, and the time interval between RSA and prior RCR. Secondary outcomes were forward elevation (FE) and external rotation (ER). METHODS: A prospectively collecte

Knotless Independent Double-Row Repair and Biceps Augmentation for Anterosuperior Rotator Cuff Tears.

Rotator cuff tears are frequent, mainly involving the anterosuperior shoulder tissue, such as the supraspinatus, subscapularis, and the long head of the biceps tendon (LHBT). The double-row repair was designed to provide better footprint coverage and a lower retear rate than a single-row repair for supraspinatus tears. Traditionally, a double-row repair needs two medial-row suture anchors and two lateral-row suture anchors to provide a wide compression area. However, the medical expenses associated with this technique are quite high. We developed a modified knotless independent double-row repa

Deltoid Osteomuscular Transfer Suppresses Humeral Head Translation Caused by Massive Irreparable Rotator Cuff Tears: A Cadaveric Biomechanical Study.

BACKGROUND: The optimal treatment for massive irreparable rotator cuff tears (MRCTs) remains unclear. The deltoid flap transfer has been described in the literature as an option for MRCT; however, it has a high retear rate and therefore its effectiveness is limited. PURPOSE: To describe a modification of the deltoid flap transfer, termed the deltoid osteomuscular transfer (DOT), and assess its biomechanical effectiveness in the management of irreparable MRCT in a cadaveric model. STUDY DESIGN: Controlled laboratory study. METHODS: Eight fresh-frozen cadaveric shoulders were tested. Segmenta

Robotic-Assisted Latissimus Dorsi Transfers Around the Shoulder.

Robotic-assisted surgery has gained prominence in orthopedic practice due to its precision, enhanced visualization, and minimally invasive nature. While robotic applications for orthopedic surgery are well established in arthroplasties, their use in soft-tissue surgery remains limited. This study presents the indications, technique, and clinical outcomes of robotic-assisted latissimus dorsi tendon transfers for shoulder reconstruction. The robotic approach allows accurate identification of the axillary and radial nerves, minimal soft-tissue trauma, reduced bleeding, and excellent cosmetic outc

Factors associated with persistent shoulder pain following arthroscopic rotator cuff repair: a scoping review.

BACKGROUND: Rotator cuff (RC) tears are among the most common and disabling musculoskeletal conditions of the shoulder. When conservative management fails, arthroscopic rotator cuff repair (ARCR) is performed. Some patients, however, report persistent shoulder pain following the surgical procedure. To improve patient outcomes, it is essential to identify factors associated with persistent pain following ARCR, which is the aim of this scoping review. METHODS: The scoping review followed the Joanna Briggs Institute (JBI) methodology and Preferred Reporting Items for Systematic Reviews and Meta-

Ultrasonographic assessment of shoulder muscle thickness and isokinetic strength in patients with rotator cuff tendinopathy: A cross-sectional study.

BACKGROUND: Alterations in shoulder muscle thickness and strength are believed to be associated with rotator cuff (RC) tendinopathy. Previous studies have mainly focused on measuring the supraspinatus tendon thickness in relevant patients with shoulder pain. This study examined differences in shoulder muscle thickness and RC strength in the symptomatic and asymptomatic shoulders of patients with RC tendinopathy. METHODS: Thirty-five patients with unilateral RC tendinopathy were included in this study. Muscle thickness of the supraspinatus, infraspinatus, deltoid, and combined middle trapezius

Inhibiting muscle degeneration in rotator cuff tears: integrative biologic approaches targeting regeneration, fibrosis, and metabolism.

Rotator cuff (RC) tears are commonly associated with progressive muscle atrophy, fatty infiltration, and fibrosis, which often persist despite successful tendon repair and remain major barriers to functional recovery and structural integrity. These degenerative muscle changes are not sufficiently addressed by tendon-focused surgical approaches and contribute significantly to poor clinical outcomes and high retear rates. In response, biologically targeted therapies have emerged to directly modulate the cellular and molecular mechanisms underlying muscle degeneration. Current strategies focus on

The mid-term outcomes on reverse total shoulder arthroplasty with isolated latissimus dorsi transfer in patients with isolated external rotation lag without pseudoparalysis: an average follow-up of 5 years.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is a well-established replacement strategy to treat irreparable massive rotator cuff tears and cuff tear arthropathy. In patients with forward (FF) deficits combined with a loss of external rotation (ER), RSA with latissimus dorsi transfer (LDT), which was initially used for brachial plexopathies, has been addressed to restore both functional deficits. The aim of the study is to assess mid-term clinical and radiological outcomes of concomitant RSA with LDT in patients with significant preoperative external rotation lag greater than 30 degr

Magnetic resonance markers of supraspinatus muscle microvascular function and energetics following a rotator cuff tear.

Rotator cuff tears (RCT) cause pain and disability in adults. Histological studies indicate that the degenerative process following a rotator cuff tear may disrupt the vascularity and energetics of the shoulder muscles. Magnetic resonance imaging (MRI) postcontractile blood oxygen level-dependent (BOLD) response and 31phosphorus magnetic resonance spectroscopy (31P-MRS) offer methods to noninvasively assess muscle microvascular function and energetic status in vivo. This study aimed to evaluate the postcontractile BOLD response and 31P-MRS as potential markers of microvascular function and ene

Impact of preoperative shoulder osteoarthritis severity score on outcomes after rotator cuff repair: A correlation study.

Arthroscopic rotator cuff repair (ARCR) generally improves clinical outcomes, but the impact of pre-existing glenohumeral osteoarthritis (GHOA) severity on these outcomes remains unclear. This study aims to evaluate the correlation between preoperative GHOA severity, assessed via the Shoulder Osteoarthritis Severity (SOAS) score, and postoperative patient-reported outcomes (PROs) following ARCR with a minimum of two years follow-up. In this retrospective cohort study, 150 patients who underwent ARCR between 2018 and 2023 were included. Preoperative GHOA severity was assessed independently by t

Recombinant Human Amelogenin Protein Enhances Tendon-To-Bone Healing in a Rat Rotator Cuff Tear Model.

Rotator cuff tears (RCTs) often require surgical intervention; however, surgical outcome is frequently insufficient due to poor healing of the tendon-bone enthesis, leading to high retear rates. Previously, the recombinant human amelogenin protein (rHAM+) regenerated injured or diseased musculoskeletal tissues including ligaments and osteochondral injuries. We therefore hypothesized that rHAM+ can also induce regeneration of the biomechanical and structural properties of the enthesis. The right shoulder infraspinatus tendon of 75 rats was fully-transected, and treated either with 0.5 mg/mL rHA

Engineering a biomimetic multiphasic suture anchor system for enhanced rotator cuff enthesis regeneration.

The rotator cuff plays a vital role in shoulder movement and joint stability. Unfortunately, tears at the rotator cuff enthesis are common and frequently lead to retears after surgical intervention, particularly at the suture location and its anchor sites. These failures are typically due to the inability of current surgical treatments to mimic the native tissue complexity and provide the necessary metabolic, bioactive, and biophysical cues for effective enthesis regeneration. In this study, we engineered a biomimetic multiphasic scaffold system (BMS) to integrate with conventional suture anch
