Demographic, Epidemiological and Functional Profile Models of Greek CrossFit Athletes in Relation to Shoulder Injuries: A Prospective Study.

Objectives: Shoulder injury prevalence appears to be the highest among all injuries in CrossFit (CF) athletes. Nevertheless, there is no evidence deriving from prospective studies to explain this phenomenon. The purpose of this study was to document shoulder injury incidence in CF participants over a 12-month period and prospectively investigate the risk factors associated with their demographic, epidemiological, and functional characteristics. Methods: The sample comprised 109 CF athletes in various levels. Participants' data were collected during the baseline assessment, using a specially de

A standardized criteria-based progressive shoulder exercise program is effective in managing rotator cuff-related shoulder pain: A prospective cohort study.

BACKGROUND: Non-operative management of patients with rotator cuff related shoulder pain (RCRSP) has been shown to be effective with outcomes similar to operative interventions. Exercise therapy and education are recommended as first-line treatments; however, program content, progression and referral criteria are typically inadequately detailed to be easily reproduced by clinicians. This study evaluated the effectiveness of a well-defined, criteria-based progressive exercise physical therapy (PT) program in patients with rotator cuff related shoulder pain (RCRSP) over a 26-week evaluation peri

Decrease in cystathionine-β-synthase in tendon tissue is an independent risk factor for rotator cuff retear following arthroscopic repair.

BACKGROUND: Rotator cuff retear is a common complication after arthroscopic repair, and identifying its risk factors is crucial for improving treatment outcomes. The lack of hydrogen sulfide (H2S) in the degenerative rotator cuff tendon may inhibit normal tendon-to-bone healing, as H2S is a key regulator for stem cells to differentiate into chondrocytes rather than fibroblasts. This study was designed to investigate whether the expression level of H2S synthases correlates with rotator cuff retear. METHODS: This prospective study included 163 patients with rotator cuff tears who underwent arth

Comparison of clinical and biomechanical results of arthroscopic rotator cuff repair using conventional and triple-row suture-bridge techniques.

BackgroundThe suture-bridge technique (SBT), with its various modifications, is frequently utilized in rotator cuff tear repairs. This study aimed to assess and compare the clinical and radiological tendon integrity outcomes of conventional and triple-row SBTs in patients with full-thickness rotator cuff tears (RCTs). Additionally, it evaluated the construct strength of each technique through biomechanical experiments. We hypothesized that the triple-row SBT would yield better clinical and radiological outcomes, as well as superior biomechanical properties, compared to the conventional SBT.Met

Effect of controlled hypotension on postoperative awakening quality in shoulder arthroscopy with lateral decubitus rotator cuff repair.

BACKGROUND: Background: Shoulder arthroscopy is commonly performed to treat shoulder injuries, degenerative conditions, and chronic pain. The quality of postoperative emergence is critical to patient recovery and overall surgical outcomes. However, few studies have investigated whether controlled hypotension affects emergence quality during shoulder arthroscopy. Controlled hypotension can optimize the surgical field, reduce operative time, decrease the risk of intraoperative complications, and enhance the quality of postoperative recovery. These benefits highlight its clinical utility and supp

Evaluating diagnostic consistency: intra and interobserver agreement in rotator cuff tear classification using ultrasound videos.

OBJECTIVE: Ultrasound (US) is a more accessible alternative to MRI for rotator cuff tear (RCT) assessment. Rapid US can identify RCTs, but accurate US interpretation remains challenging, even for experts. We performed a retrospective cross-sectional study to evaluate intraobserver and interobserver agreement and accuracy in RCT detection with US between expert and non-expert readers. MATERIALS AND METHODS: Long-axis cine sweeps of supraspinatus tendon from 138 patients with suspected RCTs were analyzed and compared to corresponding MRI results (reference standard). Three non-experts (graduate

Editorial Commentary: Arthroscopically Assisted Posterior Latissimus Dorsi and Teres Major Transfers Show Improvement for Irreparable Posterosuperior Rotator Cuff Tears but Are Rarely the Best Choice.

Tendon transfers have shown good clinical results in a numerous reviews of treatment options for unrepairable posterosuperior rotator cuff tears. Combined latissimus dorsi and teres major transfer have been described as an option to improve external rotation strength. Although outcomes have been acceptable, other options for treatment of this patient population may be preferable in terms of outcomes and perioperative morbidity.

Rotator Cuff Tear-induced Changes in Tendon Structure and Mechanics Measured by Quantitative Magnetic Resonance Imaging.

INTRODUCTION: Rotator cuff (RC) tears are prevalent degenerative injuries associated with progressive loss of shoulder function. Although MRI is routinely used for diagnosing RC tears, the relationship between imaging biomarkers and tendon mechanical function remains poorly understood. This study investigates whether quantitative MRI (qMRI), particularly T2 relaxation time, reflects structural and mechanical changes in supraspinatus tendons with RC tears. METHODS: Twenty-four human cadaveric supraspinatus tendons (10 intact, 14 torn) were analyzed. Mechanical testing was performed to assess s

Postoperative imaging of the shoulder: Rotator cuff, instability, arthroplasty.

The use of imaging after shoulder surgery should be considered carefully and should provide a real benefit for the patient. It is not a routine request. Currently, there is no consensus on imaging indications for patients showing good postoperative progress, without complications and with satisfactory functional outcomes. In cases of unfavorable progress, the indications are clearer. Conventional radiography remains essential, and when combined with ultrasound, forms the first-line approach. Second-line imaging techniques, such as CT scans or MRI, are selected based on the type of surgery perfor

The Development of iA CuffLink for Rotator Cuff Repair Telerehabilitation.

Proper rehabilitation following rotator cuff repair (RCR) is necessary for successful postoperative outcomes, though the average course of physical therapy (PT) is lengthy and costly. The goals of this study were to (1) develop exercise programs for the CuffLink mHealth system and (2) evaluate early prototype efforts at meeting the needs of RCR clients. A panel of 24 clinicians participated in a Delphi study to identify consensus in rehabilitation, key informatic needs, and appropriate interface modalities for client usage. Utilizing the Delphi findings, the iA CuffLink mHealth system was deve

Exercise Initiation After Surgical Reconstruction for Rat Rotator Cuff Injury Affects Tendon Healing and Muscle Preservation.

Rotator cuff tear is a common disease, and surgical repair is often conducted, but the re-tear rate after surgery is still high. Mechanical stress is the key factor that promotes tendon healing, but the optimal exercise protocol to induce rotator cuff healing is unknown. This study aimed to elucidate the effects of postoperative exercise initiation timing on tendon healing and muscle degeneration after rotator cuff tear and reconstruction by controlling the shoulder joint motion with immobilization and exercise intervention. A total of 110 Wistar rats were used in this study and the supraspina

GLP-1 receptor agonist liraglutide facilitates rotator cuff healing by reducing tendon cell inflammation and endoplasmic reticulum stress through the GLP-1R-AMPK/SIRT1 pathway.

Rotator cuff injuries are a common cause of shoulder pain with limited treatment options. Recent studies highlight inflammation's role in tendinopathy. GLP-1R agonists have shown anti-inflammatory effects in various diseases, but their role in rotator cuff injuries is unclear. This study investigates Liraglutide's (LIRA) effects on tendon cell inflammation, apoptosis, and endoplasmic reticulum stress in vitro and its therapeutic effects on rat rotator cuff repair in vivo. LIRA significantly inhibits IL-1β-induced inflammation, enhances tendon cell anabolism, and upregulates the AMPK/SIRT1 path
