The aim of this study was to investigate the association between the critical shoulder angle (CSA) and the clinical outcomes following arthroscopic rotator cuff repair (RCR). An ambispective study was conducted in patients who underwent arthroscopic RCR and had adequate preoperative radiological shoulder studies (type A and 1 according to Suter and Henninger). Fifty-six patients with a minimum prospective follow-up of 3 years were included: 28 with a CSA >35º and 28 with a CSA <35º prior to surgery. At the follow-up, shoulder function was evaluated using the Constant-Murley score (CS), the Sim

In terms of rotator cuff repair, there is a goal for complete repair and healing, as rotator cuff integrity correlates with clinical and functional results. Retear has been shown to have a significant influence on progression toward osteoarthritis, and patients with an intact supraspinatus show superior abduction and flexion strength. However, in cases where complete repair may not be possible and/or cost limitations may prohibit augmentation, partial repair can provide a respectable outcome. Furthermore, regardless of healing status, partial repair may provide satisfactory relief for patients

BACKGROUND: A 2 mm-thick dermal allograft and autologous Long Head of the Biceps Tendon (LHBT) Superior Capsule Reconstruction (SCR) could provide improved clinical and radiographic outcomes for posterosuperior Massive Irreparable Rotator Cuff Tears (MIRCTs).
METHODS: A retrospective study was conducted between April 2019 and October 2021. The LHBT was rerouted 5 mm posteriorly to the bicipital groove, and a 2 mm-thcick dermal allograft was used to cover the rerouted LHBT. Clinical assessment included Constant-Murley Shoulder score (CMS), American Shoulder and Elbow Surgeons (ASES) score, Sub

Since the use of autologous fascia lata for superior capsule reconstruction of massive irreparable rotator cuff tears (MIRCTs), the technique has evolved into various modifications, including dermal allografts, long head of the biceps tendon (LHBT), and combinations of both, which will be discussed in this article. After making sure the remnant cuff cannot be restored to the anatomical footprint of the supraspinatus, a double-loaded or triple-loaded, suture-based anchor is inserted 5-8 mm posterior to the bicipital groove to secure the long head of the biceps (LHBT) initially. A bone trough is

The shoulder joint combines three main joints and two auxiliary joints, which enable a unique freedom of movement. Stability is primarily ensured by muscles and ligaments, making injuries, overuse or degenerative processes common causes of complaints. A systematic examination includes the anamnesis, inspection, palpation, range of motion assessment and functional tests. During the inspection the pattern of movement, muscle atrophy and malalignments are assessed. Bone and soft tissue structures are tested by palpation. Functional tests help to differentiate pathological alterations. A structure

This study explores the efficacy of superior capsule reconstruction (SCR) using fascia lata autograft for treating irreparable rotator cuff tears. Based on our experience and existing literature, SCR has demonstrated promising outcomes, offering improved shoulder stability, reduced pain, and prevention of humeral head migration. Patients undergoing SCR with fascia lata autograft achieved near-normal shoulder range of motion postoperatively, with maintained acromio-humeral distance observed in radiographic follow-ups. Various graft modifications, including the use of dermal grafts and the long

Overhead motion in humans often leads to shoulder injuries, a consequence of the evolutionary trade-off in glenohumeral joint anatomy that balances stability with mobility. Bats consistently engage in overhead motion during flight, subjecting their shoulders to substantial loading throughout their relatively long lifespan. Remarkably, despite the demands placed on a bat's shoulder, instability and rotator cuff tears, which could be fatal to bats in short order, are not observed in nature. We were thus inspired to study functional adaptations in the shoulders of bats that enable this overhead m

BACKGROUND: Lower trapezius tendon (LT) transfer has shown promise in managing irreparable posterosuperior massive rotator cuff tears (PSMRCTs) with weakness of active external rotation (ER). However, the influence of teres minor (TM) trophicity on restoring ER strength has not been thoroughly investigated. This cadaveric biomechanical study evaluated the effect of atrophic, normotrophic, and hypertrophic TM on humeral head stability and ER recovery following LT transfer in PSMRCTs.
METHODS: Eight fresh-frozen cadaveric shoulders were tested under 5 conditions: (1) Intact rotator cuff, (2) PS

With increasing age, rotator cuff tears are common in symptomatic and asymptomatic patients. Yet, there remains a limited understanding of the etiology of pain associated with rotator cuff pathology. Recent research has associated patient perception, resilience, repair integrity, tear size, biomechanics of the humeral head, and muscle kinematics with pain levels. However, the evidence remains largely inconclusive in answering the fundamental question regarding why some patients experience symptoms severe enough to warrant surgery and others remain completely asymptomatic. A deeper understandin

A painful shoulder is a frequent complaint in general practice. In this clinical vignette, we discuss the case of an elderly patient with pain and lack of strength in the right shoulder one month after a hospital stay. The diagnosis is an injury of the accessory nerve after the placement of a central venous catheter in an emergency setting. The different causes of non-orthopaedic causes of shoulder pain are reviewed with emphasis on neurological etiologies.

BACKGROUND: Reverse shoulder arthroplasty (RSA) and arthroscopic-assisted lower trapezius tendon (aLTT) transfer are both treatment options for massive irreparable rotator cuff tears (MIRCTs) without glenohumeral arthritis; however, the indications for each remain controversial. Although RSA is an excellent reconstructive option, preserving the joint, recentering the humeral head, and restoring in-line active external rotation with an aLTT transfer may be preferred in certain patients to better restore more normal shoulder biomechanics. In-vivo kinematics, however, have been historically diffi

BACKGROUND: Posterosuperior massive rotator cuff tears remain challenging to manage. While lower trapezius transfer restores posterior cuff function, it lacks the superior cuff's biomechanical role. Middle trapezius tendon transfer has shown efficacy in addressing superior cuff deficiencies with dynamic joint-centering and spacer effects. This study aimed to compare the biomechanical effects of lower trapezius transfer alone versus combined lower and middle trapezius transfer for posterosuperior massive rotator cuff tears.
METHODS: Eight cadaveric shoulders were tested under four conditions:

The management of irreparable rotator cuff tears presents significant challenges, particularly in active individuals experiencing functional limitations, such as reduced forward elevation and deficits in both external and internal rotation. Traditional latissimus dorsi (LD) tendon transfer has shown effectiveness in reducing pain associated with posterosuperior cuff tears but often yields inconsistent functional outcomes. This is largely due to the LD's primary role as an internal rotator, which limits its capacity to restore normal shoulder biomechanics. To address these limitations, the lowe

INTRODUCTION: Arthroscopic lower trapezius tendon transfer (aLTT) is an effective strategy to treat massive rotator cuff tears by dynamically restoring the posterior aspect of the rotator cuff force couple and improving range of motion, strength, pain, and stability. aLTT is technically challenging with higher complication rates, and its learning curve has yet to be defined. We aimed to establish a learning curve for aLTT through trends in surgical time, postoperative outcomes, and complications. We hypothesized that increased surgeon experience would be associated with decreased surgical time

BACKGROUND: Posterior-superior irreparable rotator cuff tears (PSIRCTs) are a significant challenge in managing active individuals without arthritis traditional latissimus dorsi (LD) transfer is prone to retear due to the tendon's thinness and yields suboptimal outcomes with subscapularis tears. This study evaluates the clinical outcomes of fully arthroscopic combined LD and teres major transfer in patients with PSIRCTs without arthritis.
MATERIALS AND METHODS: This retrospective study included patients who underwent fully arthroscopic LD teres major (LDTM) transfer for PSIRCTs from January 2
