[Echography of the shoulder with the patient supine in the diagnosis of rotator cuff rupture].

We report a new method for shoulder sonography (US): the patient lies supine and his/her arm is positioned so as to optimize tendon depiction. The supraspinatus tendon, which is the most frequently involved structure in rotator cuff tears, is studied with the patient's arm dropped, elbow extended and forearm prone. Our series consisted of 49 surgical patients (arthroscopy in 16 and open surgery in 33 patients): US sensitivity in tear depiction was 84.8%, specificity 100% and accuracy 89.8%. Medium-small tears appeared as transonic areas within tendon substance and/or tendon focal thinning. Lar

Posterior glenohumeral subluxation: active and passive stabilization in a biomechanical model.

UNLABELLED: We examined the role of the glenohumeral and coracohumeral ligaments as well as the forces provided by the rotator cuff muscles, the long head of the biceps, the anterior and middle deltoids, and the pectoralis major in the stabilization of the glenohumeral joint in the posterior direction. Simulated muscle forces were mechanically applied to eight shoulder specimens. The humeroscapular position for testing simulated the 90-degree forward-flexion (humerothoracic) position used clinically for the so-called jerk test, which is the most clinically important position with regard to pos

Partial-thickness tears of the rotator cuff. A clinicopathological review based on 66 surgically verified cases.

This review is based on 66 patients with partial-thickness tears of the rotator cuff, verified at operation. Their average age was 54 years, and all had symptoms of subacromial impingement. The duration of shoulder pain was for between 2 and 108 months (mean 11.4 months). Ultrasonography, arthrography and bursography were helpful in establishing the diagnosis. On exploration, tears were found in the supraspinatus tendon, with 9 extending into the infraspinatus. After anterior acromioplasty, excision of the diseased portion and tenorrhaphy were undertaken. Satisfactory results were obtained in

[A study of the neurologic risk in tendino-muscular advancement of supra-spinatus and infra-spinatus in the repair of large rotator cuff rupture].

PURPOSE OF THE STUDY: An anatomical study by Warner et al. (1992) showed that an advancement of supra and infra-spinatus of more than 3 cm could be deleterious for the suprascapular nerve or its branches. In pathological conditions, the limit of possible advancement could even be less than 3 cm. The authors wanted to know whether this technique, which they use for the repair of large rotator cuff tears, could have neurological effects in surgical practice. MATERIAL: 24 shoulders among 24 patients having had a rotator cuff repair using a musclar advancement have been studied. In 13 cases only

A biomechanical analysis of rotator cuff deficiency in a cadaveric model.

We conducted this cadaveric study to define a biomechanical rationale for rotator cuff function in several deficiency states. A dynamic shoulder testing apparatus was used to examine change in middle deltoid muscle force and humeral translation associated with simulated rotator cuff tendon paralyses and various sizes of rotator cuff tears. Supraspinatus paralysis resulted in a significant increase (101%) in the middle deltoid force required to initiate abduction. This increase diminished to only 12% for full glenohumeral abduction. The glenohumeral joint maintained ball-and-socket kinematics d

Proteoglycans of human rotator cuff tendons.

Rotator cuff and biceps tendons that appeared grossly normal were procured from adult cadavers without a history of shoulder problems. These tendons were analyzed for the amount and type of glycosaminoglycan, type of proteoglycan, and histology. When compared with the distal/tensional region of biceps tendon, the glycosaminoglycan content of supraspinatus, infraspinatus, and subscapularis tendons was 2.5-fold higher and the glycosaminoglycan content of the proximal/compressed region of biceps tendon was 3-fold higher. The ratio of hyaluronic acid to chondroitin sulfate/dermatan sulfate in all

Histological analysis of the coracoacromial arch: correlation between age-related changes and rotator cuff tears.

The purpose of this study was to analyze age-related changes in the coracoacromial arch and correlate these degenerative changes with rotator cuff tears. We obtained 80 shoulders from 40 cadavers. The mean age at death was 58.4 years. We performed a gross examination of the rotator cuff and the acromion and histological examination of the coracoacromial ligament. The statistical significance of any difference for each group considered was determined by Student's t-test. The rotator cuff was normal in 66 specimens; there was an articular-side partial tear in 4 cases, a bursal-side partial tear

[Biomechanics of the shoulder and therapeutic applications].

The influence of biomechanics in surgery of the locomotor apparatus has been constantly increasing over the last few decades. The purpose of this study was to determine wether biomechanical studies can significantly influence therapy and treatment of shoulder injuries, especially shoulder instability. The investigation was performed on 23 fresh human specimens with intact capsular ligaments of the glenohumeral joint. A Bankart lesion from 3 o'clock to 6 o'clock was repaired, and a Bankart repair and anterior inferior capsular shift, as described by Neer, were performed. The measurement was don

Influence of anthropometric and psychological variables pain and disability on isometric endurance of shoulder abduction in patients with rotator tendinosis of the shoulder.

Predictors of isometric endurance of shoulder abduction were investigated in 18- to 75-year-old women (n = 59, mean age 48 years) and men (n = 53, mean age 46 years) with unilateral rotator tendinosis of the shoulder (median duration 1-2 years). They were asked to keep both shoulders abducted at 45 degrees, both wrists loaded with 2 kg, for as long as possible. The average force exerted to keep the required position was 17% higher in men compared to women. Mean times to exhaustion were: 103 seconds (SD 109) for the involved shoulder and 160 seconds (SD 81) for the uninvolved shoulder in women;

[Magnetic resonance in the study of the painful shoulder. The surgical comparison in 30 consecutive cases].

Eighty patients complaining of shoulder pain were examined with MRI from January, 1993, through December, 1994. Thirty of them were submitted to surgery, with an exhaustive inspection of shoulder structures and the treatment of abnormal findings. In this subgroup of surgical patients, MRI had depicted 16 complete tears of the rotator cuff, 4 partial tears, 8 cases of subacromial impingement, I humeral head osteochondritis and, finally, I humeral head osteochondritis with complete rotator cuff tear. Surgical findings confirmed MR diagnosis in 97% of cases. MR findings were then compared with li

Postoperative assessment of shoulder function: a prospective study of full-thickness rotator cuff tears.

Forty patients underwent surgery by a single surgeon for chronic, symptomatic, full-thickness rotator cuff defects. The study evaluated preoperative and intraoperative factors that influence postoperative outcome. The study also correlated objective measures of shoulder function with postoperative symptoms, patient satisfaction, and disability. Follow-up history and physical examination and strength measurements were performed at 2 years after surgery by three independent observers. There were 88% good or excellent results. Postscores correlated most closely with preoperative tear size. Postop

[Proton spin tomography imaging of the rotator cuff in calcific tendinitis of the shoulder].

An analysis of the MR-images of 75 patients with calcifying tendinitis of the shoulder was performed. The aim of the study was to recognize characteristic findings of the calcific deposits and their relation to the involved tendons as well as the coincidence with additional degenerative alterations of the rotator cuff. The calcifications can be demonstrated with high accuracy (> 95%) in T1-weighted images as areas of decreased signal intensity. It is possible to characterize the calcifications similar to Gärtner's radiologic classification differentiating form and density of the calcific depos

Latissimus dorsi and teres major transfer to rotator cuff for Erb's palsy.

During a 20-year period the authors operated on 56 patients with Erb's palsy to create active external rotation of the shoulder. Thirty-five patients had a follow-up period longer than 2 years (average 5 years), which allowed comparison with preoperative motion and strength. Preoperative passive external rotation averaged 5 degrees, whereas active abduction averaged 74 degrees. Preoperative external rotation strength was less than grade 2, whereas preoperative abduction strength was grade 2 or 3 in 33 of the 35 patients. Postoperative active external rotation averaged 31 degrees with only two

Kinetics of baseball pitching with implications about injury mechanisms.

Elbow and shoulder kinetics for 26 highly skilled, healthy adult pitchers were calculated using high-speed motion analysis. Two critical instants were 1) shortly before the arm reached maximum external rotation, when 67 N-m of shoulder internal rotation torque and 64 N-m of elbow varus torque were generated, and 2) shortly after ball release, when 1090 N of shoulder compressive force was produced. Inability to generate sufficient elbow varus torque may result in medial tension, lateral compression, or posteromedial impingement injury. At the glenohumeral joint, compressive force, joint laxity,

Morphology of the torn rotator cuff.

The morphological characteristics of shoulders with torn rotator cuffs were determined using 41 embalmed specimens. The following parameters were measured in the supraspinatus (SSP), infraspinatus (ISP) and subscapularis (SSC) muscles: the length, thickness and width of the extramuscular tendon; the length of the intramuscular tendon; the length and width of a tear, if present, muscle fibre length; and muscle volume. The cross-sectional area (CSA) of the tendon was measured on the photographic image of slices of the tendon using an image analysis system, and the CSA of the muscle was calculate
