Although magnetic resonance (MR) images of the glenohumeral joint frequently demonstrate intraarticular fluid, no specific criteria have, to the authors' knowledge, been published that allow accurate assessment of the amount of fluid present. Also, despite the increasing use of MR arthrography of the shoulder, the optimal amount of intraarticular fluid that should be used with this technique has not been determined. The authors progressively distended the glenohumeral joint in six cadaveric shoulder specimens with a dilute gadopentetate dimeglumine solution and obtained MR images after injecti

The clinical evaluation of the painful shoulder by an experienced and knowledgeable health care professional remains the most sensitive and clinically useful tool for the assessment of most patients with rotator cuff disease, glenohumeral instability, glenohumeral arthritis, and adhesive capsulitis. The judicious and appropriate use of diagnostic imaging studies requires a thoughtful and thorough evaluation of the data obtained from these studies and their correlation to the patient's symptoms, history, mechanism of injury, and physical findings. The appropriate use of all of this diagnostic i

OBJECTIVE: Power Doppler sonography is a new technique that offers extended dynamic range over that provided by conventional color Doppler imaging, thereby facilitating measurement of tissue perfusion. We evaluated the efficacy of power Doppler sonography in depicting soft-tissue hyperemia in musculoskeletal inflammatory conditions.
SUBJECTS AND METHODS: Twenty-three patients with predominantly unifocal musculoskeletal symptoms were evaluated with conventional gray-scale imaging and power Doppler sonography. The shoulder was evaluated in 14 patients, the elbow in three, and various other site

Twenty fresh frozen cadaver shoulders were dissected in order to study the rotator cable-crescent complex. The rotator crescent is a term that we have used to describe the thin, crescent-shaped sheet of rotator cuff comprising the distal portions of the supraspinatus and infraspinatus insertions. The crescent was found to be bounded on its proximal margin by a thick bundle of fibers that we have called the rotator cable. This cable-crescent configuration was found to consistently span the insertions of supraspinatus and infraspinatus tendons. The dimensions of the rotator cable and crescent we

Between January 1989 and December 1991 614 arthroscopic subacromial decompressions were performed for impingement syndromes. The first 100 cases represented our learning curve. The first 74 of the remaining 514 cases were available for evaluation and form the basis of the results presented here. Twenty (27.02%) patients were rated stage II in Neer's classification and 54 (72.98%) were rated stage III, 33 (44.59%) of them with a small full-thickness tear (less than 1 cm) and 21 (28.37%) with larger defects. Patients were evaluated pre- and postoperatively utilizing the ULCA shoulder score. The

The suprascapular nerve supplies sensory nerves to the posterosuperior aspect of the shoulder, including major portions of the rotator cuff. Suprascapular nerve block using steroid/bupivacaine is temporarily effective in reducing pain in rotator cuff tendinitis and tears, improving movement range in tendinitis and is possible in an outpatient setting with little or no complication risk.

Surgical reconstructions of anterior-inferior shoulder instabilities and rotator cuff injuries require secure fixation of soft tissue to bone. Sutures are inserted directly through transosseous tunnels in current techniques, which are surgically complex and not always adequate for fixation strength. Using fresh-frozen cadaveric human specimens, our objectives were 1) to compare immediate pull-out strength of two versions of polyacetal suture anchors (wedge and rod) with conventional suture-only attachment techniques in Bankart lesion and rotator cuff repairs, and 2) to compare pull-out strengt

In the last 15 years the methods of examination of the shoulder have intensively expounded. According to a well documented retrospective study we describe the value of the shoulder arthroscopy and its correlation to MRI and arthro-CT findings. MRI represents nowadays the best way of exploring rotator cuff lesions and of investigating an impingement syndrome. Associated with a contrast medium its ability to clear up an anterior or posterior shoulder instability is now superior to the arthro-CT's. The arthroscopy allows the dynamic exploration of the joint and can be used not only as a diagnosti

The glenoid labrum of the shoulder has extensive anatomic variation but appears to be important for contributing to shoulder stability and for increasing the depth of contact between the glenoid labrum and the humeral head. Tears of the labrum are commonly seen in association with other pathologic entities, such as instability and rotator cuff tears, and treatment of the labral pathology may be incidental to treatment of the other more significant pathology. However, conditions isolated to the labrum do occur and can be a significant source of shoulder problems. Effective treatment of these le

A retrospective, average 2-year review of 14 patients with a primary glenoid labral tear treated by arthroscopic debridement was conducted to evaluate the long-term results of labral debridement based on tear location in preoperatively stable shoulders. All patients with concomitant pathology requiring an additional open or arthroscopic procedure were eliminated from this study. Every patient complained of shoulder pain mainly with overhead activities and nine patients (64%) also described "clicking" with shoulder movement. All shoulders were stable to translation in all quadrants during the p

The lesions were characterised by their size (19 tears of less than 2 cm, 30 between 2 and 4 cm, 18 over 4 cm) and location (27 tears of the supraspinatus, 15 tears of the supraspinatus extending to the front (coraco-humeral, long biceps, subscapular), 20 supra and infra-spinatus, 5 "massive" tears). The tendon of the long biceps was pathological in half of the cases. Fourteen patients presented with a subacromial osteoarthritis. All patients benefited from a decom

Lesions of the rotating cuff is a most frequent cause of disorders in the shoulder joint. However, they are frequently misdiagnosed and their manifestations use to be considered as those of scapulohumeral periarthritis, cervical osteochondrosis, or even brachial plexus palsy. The paper deals with symptoms and signs of rotating cuff lesions that are fairly different from those of the conditions above cited. It is emphasized that the deltoid muscle palsy cannot result in a permanent loss or restriction of active elevation of the arm when the function of the rotating cuff remains undisturbed. 16

Arthroscopic subacromial decompression has become a popular technique supplanting the open Neer acromioplasty in many instances of chronic rotator cuff disease. A review of 61 consecutive decompressions with a minimum follow-up of 12 months was undertaken to evaluate preoperative criteria and surgical outcomes. Of the 61 patients, 53 patients with an average follow-up of 23 months were available for review. Thirty-four men and 19 women with an average age of 47 years comprised the study group. Eleven (21%) had full-thickness tears, 35 (66%) had partial-thickness injuries, and 7 (13%) had norma

In 148 patients with impingement lesion type I or type II, we performed an arthroscopic subacromial decompression (ASD). 122 patients ran a follow up one to three years post-operatively. All patients were pre- and postoperatively documented by a 100 point shoulder score. The mean score was 57.9 (+/- 11.5) preoperatively. Postoperatively there was a significant increase to 80.7 (+/- 17.9) (p < 0.05). 15% of the patients with a postoperative score less than 70 points were determined as failures. Patients with a preoperative pain history of more than one year had a significantly worse result (79.

Thirty-one patients with arthroscopically documented partial thickness rotator cuff tears treated by arthroscopic debridement of the lesion were retrospectively reviewed. The patients had had symptoms for an average of 20.5 months prior to surgery. Twenty-two of these 31 shoulders also had bursoscopy, with 18 having arthroscopic subacromial decompression. The results were graded by both the UCLA Shoulder Rating Scale and Neer's criteria. Twenty-six (84%) of the patients had satisfactory results with the remaining 5 (16%) patients having unsatisfactory results. A classification system for the s

A cadaver study was performed to determine the effect of arm position and capsular release on rotator cuff repair. Artificial defects were made in the rotator cuff to include only the supraspinatus (small) or both supraspinatus and infraspinatus (large). The defects were repaired in a standard manner with the shoulder abducted 30 degrees at the glenohumeral joint. Strain gauges were placed on the lateral cortex of the greater tuberosity and measurements were recorded in 36 different combinations of abduction, flexion/extension, and medial/lateral rotation. Readings were obtained before and aft

The complex anatomy of the shoulder often impedes the orthopedic surgeon's evaluation and understanding of injuries to the shoulder joint, particularly the rotator cuff muscle-tendon complex. Arthroscopy, MRI, and digital data analysis are several techniques now at the surgeon's disposal. This article reviews the application of these techniques in the evaluation of rotator cuff injuries.

Abnormalities of the long tendon of the biceps brachii muscle commonly accompany other lesions about the shoulder, especially rotator cuff rupture, and are a frequent cause of a painful shoulder. The spectrum of abnormalities associated with medial displacement of the biceps tendon seen at magnetic resonance (MR) imaging was reviewed in six patients. MR imaging was performed on a 1.5-T system with use of a dedicated surface coil. The findings at MR imaging were correlated with those at surgery. Two types of medial displacement of the biceps tendon were identified. More frequently observed was
