Magnetic resonance imaging of shoulder impingement.

MRI is an accurate technique for diagnostic evaluation of a broad spectrum of shoulder pathology. In particular, rotator cuff injuries are a frequent cause of pain and disability. The mechanism of injury to the supraspinatus tendon usually begins with microscopic tears of fibers of the triple helix collagen molecule. This leads to increased motion of the bound water within the collagen molecule that lengthens the effective T2 of the tendon, allowing short TE imaging sequences to detect signal in abnormal tendons. Clinically significant disease probably does not occur without frank collagen rup

Current concepts and recent advances in the athlete's shoulder.

The demands placed on the shoulder girdle during athletics can often exceed its physiologic limits and result in significant injury. The goal of orthopedists, therapists, trainers, and ancillary personnel who care for athletes involved in sports which require extreme demands of the shoulder is to enhance athletic performance, extend their longevity, and prevent injury. With the new concepts available from basic science and clinical research, a clear understanding of normal architecture and biomechanical function of the shoulder girdle is better appreciated. A greater understanding of sporting

Electromyographic analysis of muscle action about the shoulder.

The application of dynamic integrated EMG and motion analysis to the shoulder has improved the understanding of dynamic shoulder biomechanics during athletic activities and rehabilitation protocols. These studies exemplify a symbiotic merger of basic science research and clinical application. EMG and motion analysis has produced specific objective quantifiable data concerning the muscles in and about the shoulder during normal planar motion, athletic motions, and rehabilitation exercises. This expanded understanding of intricate muscular interrelationships during athletic activities and rehabi

Open surgical techniques in the athlete's shoulder.

Treatment of the athlete's shoulder is one of the greatest challenges facing the orthopedic specialist. Surgical procedures, and in particular, the open surgical techniques presented in this article, are always a last resort of treatment. The primary treatment is always nonoperative with a particular emphasis toward physiotherapy. In selected instances in which patients have failed rehabilitation, the techniques that are presented can achieve a relatively high degree of success if particular attention is paid to the specific indications, approaches, and details of each technique. Open surgical

Isokinetic torque imbalances in the rotator cuff of the elite water polo player.

The specific repetitive activity of water polo, like baseball pitching, emphasizes adduction and internal rotation. This study used the Cybex II to evaluate the isokinetic strength of the rotator cuff in elite water polo players and in a group of control subjects. The water polo players were significantly stronger than the controls. Of greater importance was the confirmation of imbalances in the rotator cuff force couples of adduction/abduction and external/internal rotation. These changes are similar to those reported for pitchers. The adductors in the water polo group had gained in relative

[Factors affecting the surgical results of treatment of rotator cuff tears in the shoulder].

The work presents the outcomes of clinical, radiological and mathematical analyses of 42 patients with the rupture of the whole width of the cuff of shoulder rotators treated operatively. The group comprised 34 male and 8 female patients, the average age 55.5 years. The mean period between the operation and final clinical evaluation was 23 months. According to the method chosen for the evaluation of the results by means of 100-point score acceptable result was achieved in 90 per cent of cases (excellent in 64 per cent, good in 26 per cent). Double t-test showed that significantly participating

[Rupture of the rotator cuff].

Spontaneous or traumatic ruptures almost always occur in a cuff that has degenerated due to overuse. Despite the lack of absolute anatomico-clinical correlation, two clinical types can be described: a conflictual, often hyperalgesic shoulder with its painful arch corresponding to a small anterosuperior rupture, or a pseudo-paralytic shoulder betraying a major posterosuperior rupture. Standard radiography with Leclercq's manoeuvre is performed primarily to find out whether the cuff is continent or incontinent. Modern medical imaging (ultrasonography or MRI) adds precision to the lesions when su

Cross-country skiing injuries and biomechanics.

Cross-country skiing exercises most of the joints, muscles and tendons in the body giving the skier an all around workout. This, in combination with a low incidence of injury, makes cross-country skiing an ideal recreational and competitive sport. The new skating techniques developed during the last decade have resulted in greater velocity. The maximum speed during the diagonal stride technique is 6 m/sec compared to 8 to 9 m/sec when skating and double poling. Top-level skiers today use strong and ultra light skis of fiberglass and graphite. The ski weight is less than 500g. Today's skating t

[Tendon ruptures of the shoulder].

Common sports, involving raising the arms above the head, i.e., throwing, racquet games and swimming, often result in rotator cuff tendinitis. During the throwing motion, the humeral head and its overlying biceps tendon and rotator cuff must pass rapidly under the coraco-acromial arch. Damage to these structures can occur by several mechanism. First, an increase in the size of the structures passing underneath the arch may lead to impingement. This can occur either by way of hypertrophy of the musculotendinous cuff or by way of inflammation of the cuff. Second, a decreased space available unde

Rotator cuff tear--relationship between clinical and anatomopathological findings.

Rotator cuff tears (RCT) are frequent and increase with age. Why do only a relatively small percentage cause a permanent severe handicap justifying surgery? Is there a relationship between size and site of the tear and the clinical symptoms? How do they influence the postoperative result? These questions were answered in a prospective study of 76 operated patients. We adopted D. Patte's classification of the RCT into four groups according to the site and size of the lesion. Our figures were compared with those obtained by Patte in an analogous study of 256 cases. Of the tears in our series, 56

Magnetic resonance imaging of glenohumeral joint diseases.

Through the application of oblique planes and flexible surface coil techniques, magnetic resonance imaging (MRI) promises to be of great clinical value in the evaluation of a variety of pathologic conditions affecting the shoulder. In patients with joint effusions, the tendinous portion of the rotator cuff, glenoid labrum, and bicipital tendon can be readily visualized. This capability has particular relevance in patients with inflammatory disease and traumatic conditions. Rotator cuff atrophy and impingement of the coracoacromial arc upon the supraspinatus muscle and tendon can also be demons

Isokinetic strength of the shoulder after repair of a torn rotator cuff.

The strength of the shoulder was measured isokinetically in forty patients who had undergone surgical repair of a torn rotator cuff. Thirty-three patients were tested six months postoperatively and twenty-four, after a year. There was an increase in strength between the two intervals (p less than 0.05) for abduction measured at 180 degrees per second, external rotation measured at 60 and 180 degrees per second, and flexion measured at 60 degrees per second. On the average, strength reached 80 per cent of normal in abduction and 90 per cent of normal in external rotation at one year postoperati

Prevalence rates and odds ratios of shoulder-neck diseases in different occupational groups.

The aim of the present study was to evaluate the association and impact of occupational exposure and diseases of the shoulder and neck. Prevalence rates, odds ratios, aetiological fractions, and their confidence intervals were computed for pooled and non-pooled data of previous published reports. By comparison with office workers and farmers, dentists had an increased odds ratio for cervical spondylosis (two studies) and for shoulder joint osteoarthrosis. Meat carriers, miners, and "heavy workers" also had significantly higher rates of cervical spondylosis compared with referents. Compared wit

Shoulder pain and heavy manual labor.

The problem of the increasingly common shoulder pain syndromes in industry was investigated by biomechanic and epidemiologic methods. Rotator cuff tendinitis constitutes a major problem in people with arduous occupations, i.e., shipyard welders (prevalence ratio, 18.3%), and steel plateworkers (16.2%). Static work seems to induce symptoms at an earlier age. The clinical diagnosis was supraspinatus tendinitis. The biomechanic studies confirmed the view that the shoulder muscles are heavily loaded when the arm is elevated. Excessive hand tool mass increases the strain markedly in some muscles, p

Subluxation of the shoulder in athletes.

Shoulder subluxation is an extremely common problem confronting the sports medicine physician and may present in a variety of ways, with the patient complaining of pain of a sense of instability. In throwing spots, tendinitis secondary to impingement is common, but it may also be secondary to instability as a result of traction or compression of the rotator cuff. In addition, subluxation may develop initially, then progress to dislocation, or the converse may occur. However, some patients may have subluxation over an extended period of time. The physician must consider the entity and take a ca

"Supraspinatus slide" for rotator cuff repair.

The supraspinatus slide procedure described by Debeyre et al. (1965) offers an opportunity for the surgeon to repair certain large and late ruptures of the rotator cuff. In order to re-appraise this procedure and clarify its indications, the authors performed the following study: Firstly 37 rotator cuff ruptures repaired by this method were reviewed. Most of the patients were supplied by the Workmen's Compensation Board of Ontario. The review period was from 1968 to 1977. Secondly, the surgical anatomy of the area was reviewed. Twenty-two (60%) of the patients obtained satisfactory results. Th

[Combined traumatic rupture of the rotator cuff and nerve lesions (author's transl)].

Traumatic damage to the rotator cuff was diagnosed in 13 patients by clinical, electromyographic and arthrographic studies. The main features were failure to rotate, elevate or abduct the affected shoulder joint. Eight patients also had peripheral nerve lesions: axillary nerve lesion was present in five, partial brachial plexus lesion in three.
