Isolated injuries to the short head of the biceps (SHB) are uncommon, and the existing literature mainly includes case reports documenting tears at the muscle belly or myotendinous junction. This case report presents the first reported SHB tendon injury at the coracoid insertion in a 34-year-old healthy male patient after trauma while playing with his daughter. The injury occurred with the arm forced into hyperextension, abduction, and external rotation, with the elbow fully extended. Diagnosis was confirmed by MRI, showing an isolated insertional tear of the SHB tendon with intact coracobrach

PURPOSE OF THE STUDY: The authors examined the anatomic condition and the function of the rotator cuff obtained after an average period of four years following surgical repair in a series of 100 full thickness rotator cuff tears. The aim was to assess the validity of Constant's scoring method and to analyse risk factors and the frequency of recurrent tears.
MATERIAL AND METHODS: The series comprised 98 patients, 62 men and 36 women whose average age was 56 years. It included 69 tears of less than 2 cm in size (39 cases) or between 2 to 4 cm (3 cases) of the supra-spinatus, 22 tears of the sup

INTRODUCTION: Magnetic resonance imaging (MRI) is highly accurate for diagnosing full-thickness rotator cuff tears but inaccurate in detecting small anterosuperior cuff tears near the rotator interval. Expanding on the concepts of long head of biceps tendon (LHBT) instability and the anterior sentinel sign, the present study evaluated the posterior sentinel sign, a novel arthroscopic indicator of supraspinatus tears.
METHODS: This monocentric prospective cohort study included patients undergoing arthroscopic repair of rotator cuff tears at a single institution. The integrity and stability of

To overcome the drawbacks of these conventional suturing methods, we have developed a novel suture technique. This new technique combines the advantages of the traditional Mason-Allen technique and the suture bridge technique, which enhances the tendon-to-bone contact area and prevents suture cutout, among other benefits. This study is to investigate the method, technique, and clinical efficacy of this novel suture technique in the treatment of rotator cuff injuries. Between January and December 2023, 21 patients (9 male, 12 female; age range: 46-71 years, mean: 51) underwent rotator cuff repai

Rotator cuff tears are frequent, mainly involving the anterosuperior shoulder tissue, such as the supraspinatus, subscapularis, and the long head of the biceps tendon (LHBT). The double-row repair was designed to provide better footprint coverage and a lower retear rate than a single-row repair for supraspinatus tears. Traditionally, a double-row repair needs two medial-row suture anchors and two lateral-row suture anchors to provide a wide compression area. However, the medical expenses associated with this technique are quite high. We developed a modified knotless independent double-row repa

Sprinting and the Nordic hamstring exercise (NHE) are common training modalities used to reduce hamstring injury risk, but the differences in the biomechanical demands of sprinting and the NHE are unclear. We conducted an exploratory analysis comparing knee and hip joint kinematics and kinetics, and biceps femoris long head (BFLH) muscle-tendon unit length and velocity during the flight phase of sprinting and the NHE. We collected motion capture and force data from fourteen athletic participants (8 males, 6 females) running (4-8 m/s) and performing the NHE. We used this experimental data and m

BACKGROUND: Prolonged Nordic hamstring exercise (NHE) training reduces hamstring injury risk, but the underlying mechanisms remain unclear. This study examined how NHE training alters biceps femoris long-head (BFlh) muscle-tendon unit (MTU) mechanics during the exercise, focusing on how muscle adaptations influence fascicle and sarcomere lengths during the NHE after training.
METHODS: Twelve recreationally active participants completed 9 weeks of supervised NHE training and 3 weeks of detraining. Knee flexor torque was measured using ankle-mounted load cells and estimated from the knee joint

Massive irreparable rotator cuff tears (MIRCTs) present a particularly complex and difficult challenge for the orthopedic surgeon. Fatty infiltration of muscles, chronic excessive retraction of tendons, and overall tissue degeneration predispose these cases to high failure rates following surgical treatment. Several approaches have been introduced for transferring the long head of the biceps tendon (LHBT) to help stabilize the humeral head and reestablish shoulder muscle strength. This article proposes a technical modification of capsular reconstruction using arthroscopic LHBT rerouting and fi

The challenge of managing massive irreparable rotator cuff tears is not solved through technical complexity alone but through achieving balance between biological restoration and mechanical reinforcement. Superior capsule reconstruction using the long head of the biceps tendon offers static and dynamic advantages, whereas tendon transfers provide additional posterior force vectors to restore external rotation. The combined "static-dynamic" strategy may represent an evolving biologically based alternative for selected patients who are not candidates for arthroplasty.

BACKGROUND: Hamstring injuries are the leading cause of time lost in professional soccer. Among these, tendon-related lesions of the biceps femoris (BF) are associated with longer recovery and higher reinjury risk. Emerging magnetic resonance imaging (MRI) findings suggest the existence of morphological subtypes with different prognostic implications.
PURPOSE: To describe an MRI pattern of proximal BF tendon injury-the "Bunny Tail lesion"-and report its management and outcomes in elite soccer players.
STUDY DESIGN: Case series; Level of evidence, 4.
METHODS: Over 2 competitive seasons (July

PURPOSE: To investigate the underlying regeneration mechanisms of biological augmentation using pressed long head biceps tendon (LHBT) in a rat model.
METHODS: Bilateral infraspinatus tendons were torn and repaired 1 week later in 32 Sprague-Dawley rats. In the biologic augmentation (BA) group, the LHBT was extracted, pressed, and augmented on the repair site. Histological evaluation was performed at 3 and 6 weeks to measure the thickness of the repaired tendon, number of chondrocytes and non-chondrocytes, percentage of aligned chondrocytes, areas of fibrocartilage and collagen fiber, and COL

The Nordic hamstring exercise (NHE) and high-speed running are widely used training modalities to prevent hamstring strain injuries, yet the differences in the muscle lengths, forces, power, work, and impulse between these training modalities remain unclear. Thus, in this study, we compared model-estimated dynamics of the most frequently injured hamstrings muscle, biceps femoris long head (BFLH), for 14 participants (8 male and 6 female) performing the NHE and running between 4 and 8 m/s. We collected motion capture data to drive musculoskeletal simulations to estimate muscle fiber lengths and

BACKGROUND: The semitendinosus muscle, a common hamstring injury site, often sustains damage at its myotendinous junctions. Understanding these connections is crucial for comprehending injury mechanisms and guiding treatment. This study aimed to investigate the semitendinosus muscle's architecture in adults and fetuses.
MATERIALS AND METHODS: 50 semitendinosus muscles from adult cadavers and 20 from fetal specimens were analyzed using standard anatomical dissection methods. An ultrasound examination was conducted on 20 semitendinosus muscles in living individuals.
RESULTS: The proximal origi

BACKGROUND: The treatment of large to massive rotator cuff tears leads to a high failure rate and several techniques have been proposed to improve these results. Rerouting the long head of the biceps tendon (LHBT) appears to be a biological reinforcement to increase tendon healing. Therefore, this study aimed to compare the clinical and radiological outcomes of open repair for large and massive rotator cuff tears reinforced with the LHBT to conventional open repair.
METHODS: A prospective randomized study was conducted with patients diagnosed with large to massive rotator posterosuperior rota

BACKGROUND: When an adult with rotator cuff tendinopathy reports feeling or hearing a pop along with new pain, both the clinician and the patient may inaccurately and unhelpfully expect (framing heuristic) an injury and consider it a compensable work claim.
METHODS: We retrospectively reviewed medical records of 118 people that filed work claims for new shoulder pain where aspects of their care or recovery trajectory triggered a peer review. We collected data on age, sex, reports of hearing or feeling a pop (25%; 29 of 118 patients), and reports of new numbness or tingling (11%; 13 of 118 pat

The Popeye sign is a classic eponym describing the cosmetic deformity following long head of the biceps tendon (LHBT) retraction. While frequently cited, its diagnosis remains subjective and standardized criteria are lacking. Muscle mass, body mass index, and surgical treatment of LHBT pathology as well as patient perceptions can all influence the prevalence and functional effect of the Popeye sign. This article reviews the historical origin of the eponym and its clinical relevance. Furthermore, we propose a novel, standardized diagnostic grading system to quantify the Popeye sign, facilitatin

AIM: Rotator cuff tears and long head of the biceps tendon (LHBT) injuries are primary causes of shoulder pain. During rotator cuff repair, concurrent LHBT injuries are frequently identified. Surgical management of LHBT injuries can effectively relieve pain and improve shoulder function. Two commonly used surgical approaches for LHBT repair are interference screw compression fixation and suture anchor ligation fixation. However, no definitive conclusion has been reached regarding which technique yields superior clinical outcomes. This study compared the efficacy of arthroscopic rotator cuff re

To determine whether elite collegiate athletes with a prior hamstring strain injury (HSI) display persistent deficits in hamstring muscle morphology and muscle-tendon unit (MTU) mechanics during sprinting, and whether MRI-informed, subject-specific musculoskeletal models detect limb differences that scaled-generic models overlook. This study analyzed data from the Hamstring Injury (HAMIR) study. Seventy-nine NCAA Division I male football players (106 previously injured vs. 52 uninjured control limbs) completed maximal-velocity over-ground sprints instrumented with inertial measurement units (1
