Minimum 24-Month Outcomes of Minimally Invasive Paratenon Protection Repair vs Open Giftbox Repair of Ruptured Achilles Tendon.

BACKGROUND: The paratenon has been shown to promote Achilles tendon healing, but the evidence supporting the role of paratenon protection technique in Achilles tendon repair is sparse. We retrospectively assessed the results of a paratenon-sparing repair technique vs an open giftbox repair of Achilles tendon ruptures. METHODS: Patients with Achilles tendon rupture who underwent surgical treatment at our hospital between January 2015 and August 2021 were retrospectively reviewed. Among them, 61 patients underwent surgical repair using the minimally invasive paratenon protection technique (MI g

Quantifying Muscle Volume Deficits Among 38 Lower Extremity Muscles in Collegiate Football Athletes After Anterior Cruciate Ligament Reconstruction.

BACKGROUND: Quadriceps dysfunction is ubiquitous after anterior cruciate ligament (ACL) reconstruction (ACLR). Addressing quadriceps dysfunction is crucial to improve function, reduce the reinjury risk, and maintain long-term knee health. While deficits specific to the quadriceps are well documented, less is known about the effect of an ACL injury on other lower extremity muscle groups. PURPOSE/HYPOTHESIS: The purpose of this exploratory analysis was to quantify and rank lower extremity muscle volume deficits using magnetic resonance imaging in collegiate football athletes after ACLR. It was

Effect of foot position during plantarflexion on the neural drive to the gastrocnemii in runners with Achilles tendinopathy.

Runners with Achilles tendinopathy have reduced neural drive to the gastrocnemius lateralis. Positioning feet-inwards (feet-in) can influence gastrocnemii activity in healthy individuals. Therefore, this study investigated if pointing (feet-in) during isometric plantarflexion would increase gastrocnemius lateralis electromyography root mean square amplitude (RMS) and motor unit discharge rates (MUDR), compared to feet-neutral (feet-neutral), in Achilles tendinopathy. High-density electromyograms were recorded from gastrocnemius lateralis and medialis, during 20-s feet-in and feet-neutral contr

A Novel Method to Assess Subject-Specific Architecture of the Achilles Tendon In Vivo in Humans.

The Achilles tendon (AT) comprises three subtendons whose relative locations, and respective lines of action, vary individually. This study was aimed to demonstrate the efficacy of a novel method, combining Ultrasound and electrical STIMulation (USTIM), to identify the in vivo location of individual subtendons in cross-sections of the AT. We individually stimulated the triceps surae muscle heads and imaged localized tissue movement on a transverse plane 1 cm proximal to the calcaneus using B-mode ultrasonography. Movement induced by muscle stimulation was presumed to arise from movement in the

Problems in Diagnosis and Treatment of Soleus Muscle Injuries-Narrative Review and Case Report.

Injuries to the soleus muscle are often unrecognized, which increases the risk of complete tearing. Consequently, it results in the need for a long break in sports. This is mainly because the soleus muscle is complex, and the clinical signs of injury are difficult to capture, which can mimic Achilles tendinopathy and tennis player's calves. This muscle has a complex connective tissue structure with three intramuscular tendons, which makes it challenging to interpret pathological muscle conditions. Injuries to the soleus muscle can be acute or chronic and are usually considered to be a minor di

Effects of Blood Flow Restriction Training on the Upper Extremities: A Scoping Review.

High-intensity training leads to muscle growth in the upper extremities but at the expense of a greater risk of damaging joints, ligaments, or tendons due to this region being more prone to injury. In contrast, low-intensity resistance training implements a low load with high repetition, which is safer but yields less muscle hypertrophy and greater time consumption. Blood flow restriction (BFR) training potentially provides a solution as it achieves the muscle growth and strengthening of high-intensity resistance training while safely performing low-intensity resistance training. This techniqu

Achilles Subtendons Stiffness Differ in People with and without Achilles Tendinopathy.

PURPOSE: Previous work showed altered mechanical properties of the Achilles tendon in the presence of tendinopathy, considering the Achilles tendon as a homogeneous structure with the gastrocnemius medialis (GM) subtendon representative of it. However, the Achilles tendon consists of three semi-independent structures: the GM, gastrocnemius lateralis (GL), and soleus (SOL) subtendons, each independently pulled by their respective muscle. The aim of this study was (i) to compare the mechanical properties of the different Achilles subtendons in humans in vivo by considering the force of each musc

Surgical Options for the Restoration of Elbow Extension After Brachial Plexus Injury.

Brachial plexus injuries (BPIs) are devastating and lead to marked functional disability. Injuries involving the proximal radial nerve, posterior cord, or lower cervical nerve roots can lead to triceps palsy and inability to actively extend the elbow. Although not as functionally important as elbow flexion, active elbow extension is required for some activities of daily living, and discussion of reconstructive options with the patient is warranted. Various nerve transfers have been successfully used and may be available, depending on the extent of injury and other surgeries planned or performe

Understanding the structure and mechanics of the sheep calcaneal enthesis: a relevant animal model to design scaffolds for tissue engineering applications.

Tendon or enthesis injuries are a worldwide clinical problem. Along the enthesis, collagen fibrils show a progressive loss of anisotropy and an increase in mineralization reaching the bone. This causes gradients of mechanical properties. The design of scaffolds to regenerate these load-bearing tissues requires validation in vivo in relevant large animal models. The sheep tendon of triceps surae muscle is an optimal animal model for this scope with limited knowledge about its structure and mechanics. We decided to investigate in-depth its structure and full-field mechanics. Collagen fibrils mor

Load and muscle-dependent changes in triceps surae motor unit firing properties in individuals with non-insertional Achilles tendinopathy.

Non-insertional Achilles tendinopathy (NIAT) induces morpho-mechanical changes to the Achilles tendon (AT). Evidence on how triceps surae motor unit firing properties are influenced by altered tendon mechanics in NIAT is limited. This study investigated motor unit firing properties (mean discharge rate (DR), recruitment and de-recruitment thresholds, and discharge rate variability (COVisi)), motor unit firing-torque relationships (cross-correlation coefficient between cumulative spike train (CST) and torque, and neuromechanical delay (NMD)) and neural drive distribution (connectivity strength

Morphological Analysis of the Latissimus Dorsi Tendon Insertion: Implications for Optimizing Tendon Transfer Surgeries in Rotator Cuff Repair.

Latissimus dorsi (LD) tendon transfer is widely used to treat irreparable rotator cuff tears, particularly in cases with posterosuperior and anterosuperior tear patterns. We conducted a comprehensive anatomical analysis of the attachment of the LD tendon to the humerus, focusing on its morphological patterns and morphometric features, which are critical for optimizing the surgery. Dissection of 32 LD tendons in 16 Korean cadavers revealed three insertion patterns: fully combined (40.6%), partially combined (34.4%), and fully separated (25.0%). The mean width, length, and thickness of the tendo

Acute Lateral Ankle Sprain Impairs Function and Strength Without Altering Muscle or Tendon Stiffness: A Controlled Observational Study.

INTRODUCTION: Acute lateral ankle sprain (LAS) frequently results in persistent functional limitations. Understanding changes in calf muscle and Achilles tendon (AT) stiffness after LAS may shed light on mechanisms underlying impaired function. OBJECTIVE: To investigate the effects of acute LAS on the mechanical properties of the calf muscles and the Achilles tendon, ankle function, pain, edema, and strength. METHODS: This controlled observational study was conducted from August 2023 to January 2025. Fourteen participants with acute LAS and 14 healthy controls were evaluated twice, 6 weeks a

Is soleus intrinsic motor neuron excitability contributing to motor deficits in runners with Achilles tendinopathy?

OBJECTIVE: Soleus weakness is suggested to contribute to Achilles tendinopathy (AT) in runners. Since muscle force relies on the ability of motor units firing at high frequencies, and intrinsic motor neuron excitability contributes to firing rate modulation, soleus inhibition and hypoexcitability may contribute to AT soleus weakness. This study investigated soleus motor neuron excitability by comparing: (i) estimates of persistent inward currents (Δf and Δf/k), which is known to modulate excitability; (ii) brace height, which is related to neuromodulatory input onto motor neurons; (iii) attenu

Transfer of Middle and Posterior Deltoid Extended by the Lateral Intermuscular Septum for Elbow Extension Reconstruction.

PURPOSE: Restoring elbow extension in tetraplegia and brachial plexus injuries is critical for improving hand function. In chronic lesions, the traditional method is based on the transfer of the posterior deltoid elongated by a tendon graft to the triceps. Tendon grafts need a prolonged immobilization period to avoid stretching at the muscle-tendon junction. To avoid the use of tendon grafts, we propose harvesting the middle and posterior deltoid muscle along with the entire length of the lateral intermuscular septum, facilitating direct suturing to the triceps tendon. METHODS: Before surgery

Contralateral Lower Trapezius to Triceps Transfer for Restoring Elbow Extension in Patients with Brachial Plexus Palsy - A Technical Note.

Restoration of elbow extension is seldom considered in the reconstructive plan for patients with extensive brachial plexus injury because of the scarcity of the nerve and tendon transfer options. However, restoring elbow extension could increase the outreach of hand and give better control of the elbow flexion. We propose a new technique for restoring elbow extension using the readily available contralateral lower trapezius (CLT). By extending the CLT with a fascia lata graft and transferring it to the triceps, we were able to restore elbow extension. In all four patients who underwent this pr

Correlation between functional and structural variables in patients undergoing achilles tendon surgical repair.

Achilles tendon (AT) rupture promotes deficits in the plantar flexors and AT's structural properties, which have been related to deficits in the ankle joint range of motion (ROM), in the plantar flexors' ability to produce force and in functionality. The objective of this study was to evaluate possible correlations between the triceps surae's structural variables with performance in functional tests, and to compare these correlations between the uninjured limb and the limb that underwent AT surgical repair. Both legs from 28 participants (37 ± 5 years) were evaluated after tendon surgical repa

Medial gastrocnemius muscle and aponeurosis shear wave velocity and morphological changes after Achilles tendon rupture: A 1-year follow-up study.

Achilles tendon rupture (ATR) alters stiffness of the tendon and other structures within the triceps surae muscle-tendon unit. Although stiffness of the tendon has been studied after rupture, regional adaptations of the medial gastrocnemius (MG) muscle and aponeurosis mechanical properties are unknown. Therefore, we assessed changes in MG muscle and aponeurosis shear wave (SW) velocity and morphology during a 1-year follow-up after unilateral ATR. Twenty-three (17 males, 6 females) participants were assessed for SW velocity of MG muscle and aponeurosis and morphological properties at 2, 6 and

Differences in triceps surae muscle dynamometry and electromyography between adult males and females.

The triceps surae performs vital functions during locomotion and possesses shock-absorbing capacity. The injury rate of the Achilles tendon is higher in males than females. Quantification of the triceps surae muscle force outputs across sexes has not been determined. This study aimed to investigate and quantify sex-related triceps surae isokinetic parameters and activation magnitudes. A repeated-measures comparative design is implemented. A total of 20 males and 20 females joined this study. An isokinetic dynamometer and a synchronized electromyography system were employed to measure isokineti

Interlimb differences in gait kinematics, kinetics, and muscle activation during walking and running one year after acute unilateral Achilles tendon rupture.

BACKGROUND: Approximately 30% of individuals with Achilles tendon rupture do not fully restore normal gait function, regardless of the treatment chosen. Limited knowledge exists about the long-term kinematic, kinetic, and neuromuscular adaptations after repair of an acute Achilles tendon rupture and their impact on muscle/tendon function. This exploratory cross-sectional study assessed between-limb differences in terms of lower-limb kinematics, kinetics, and muscle activation during walking and jogging stance phases one year after an Achilles tendon rupture. METHODS: Thirty-seven participants
