Postero-lateral elbow dislocation with traumatic brachial artery disruption.

Dislocations of the elbow are the second most common large joint dislocation, following shoulder dislocations. Most are closed reductions, especially posterior or posterolateral types, and do not require surgical intervention. When an elbow dislocation involves a vascular structure-usually the brachial artery-this injury becomes a rare and life-threatening emergency. Collateral circulation can maintain distal pulses, so these injuries can be underdiagnosed. Without rapid intervention, complications of limb ischemia, compartment syndrome, nerve injury, and even amputation can occur. This report

Associations Between Fracture Characteristics and Surgical Outcomes in Distal Digit Replantation of Tamai Zones 1-2.

BACKGROUND: Although numerous risk factors were reported for the failure of digit replantation, there was a lack of studies investigating the association between replantation outcomes and fracture characteristics. The purpose of this study was to evaluate the survival rate and functional outcomes of distal digit replantation based on the fracture patterns. METHODS: We retrospectively reviewed patients with Tamai zones I-II amputated digits who underwent replantation between 2018 and 2022. The fracture characteristics of the distal phalanx were compared to outcomes, including the survival rate

Evaluating the Ability of Brachial Plexus-Injured Patients to Control an Externally Powered (Myoelectric) Hand Prosthesis.

BACKGROUND: Restoration of hand function after traumatic brachial plexus injury (BPI) remains a formidable challenge. Traditional methods such as nerve or free muscle transfers yield suboptimal results. Advancements in myoelectric prostheses, characterized by novel signal acquisition and improved material technology, show promise in restoring functional grasp. This study evaluated the ability of adults with a BPI injury to control an externally powered prosthetic hand using nonintuitive signals, simulating the restoration of grasp with a myoelectric prosthesis. It also assessed the effectivene

Free fillet flap transplantation using amputated upper limbs for shoulder defect coverage and joint function salvage in emergency treatment-A case report.

INTRODUCTION AND IMPORTANCE: Severe limb trauma results in mangled extremities, amputation, uncovered wound and delayed healing. The rapid development of flap transplantation concept and technique leads to applications of free flap in the salvage of limb and articular appearance and functions. This report discusses the case of a patient with acute shoulder avulsion and smashed injuries and evaluates the feasibility and safety of free fillet flap transplantation in emergency treatment. CASE PRESENTATION: A 44-year-old man presented with acute traumatic severing injury to the left arm. We perfo

Treatment of Nonreplantable Total Thumb Amputation at the CMC Level Using Index Finger Pollicization.

BACKGROUND: Pollicization of the index finger is a rarely performed reconstructive option for patients with total thumb amputations with nonsalvageable carpometacarpal (CMC) joint and thenar muscles. Successful pollicization can provide basic grasp and pinch to help patients carry out activities of daily living. We present a retrospective review of 4 patients who underwent index finger pollicization for traumatic total thumb amputations. METHODS: A retrospective review of 4 cases of pollicization using an injured index finger for traumatic thumb amputation was performed. Patients available fo

Utility of "reposition-flap" in the reconstruction of the avulsed thumb.

Injuries that result in thumb amputation cause a loss of 50% of hand function. Microsurgical replantation remains the gold standard of thumb reconstruction techniques. The non-microsurgical technical variants of thumb reconstruction described so far aim to create a neo-thumb of adequate length, stable, opposable, sensitive, and last but not least esthetically pleasing appearance. Avulsion of the distal phalanx and the absence of the nail will determine a functional deficit but also an unesthetic appearance. When replantation is not possible or the patient refuses to "sacrifice" another anatomi

Detection of traumatic elbow arthrotomies: computed tomography scan vs. saline load test.

BACKGROUND: Traumatic elbow arthrotomies are common injuries evaluated for by orthopedic services; however, failed identification of a traumatic arthrotomy leads to a high risk of developing septic arthritis. Currently these injuries are evaluated by either a saline load test or a computed tomography (CT) scan, yet there is little published evidence regarding detection of traumatic elbow arthrotomies. HYPOTHESIS: In our study, we hypothesized better sensitivity and specificity of detecting a traumatic elbow arthrotomy with a CT scan over a saline load test. STUDY DESIGN: Descriptive cadaveri

Use of single-staged transmetatarsal trimmed great toe transfer for reconstruction of a thumb amputation at the carpometacarpal joint.

BACKGROUND: Reconstruction for proximal thumb amputation at the carpometacarpal (CMC) joint level is extremely challenging. All thenar muscles are lost and other fingers might be lost or injured. Transmetatarsal trimmed great toe (TGT) transfer may be an option in such cases. METHODS: Between 2012 and 2018, 11 patients who had amputation of the thumb at the CMC joint level were reconstructed by transmetatarsal TGT transfer in a single stage. There were seven cases in which the right hand was affected and four cases in which the left hand was affected. Three cases involved a contracture scar a

Modified single pedicle reverse adipofascial flap for fingertip reconstruction.

BACKGROUND: Homodigital adipofascial flap is a well-established method for treating a distal fingertip defect; nonetheless, its use has some limitations. Reconstruction of fingertip injuries, with radial or ulnar tissue loss, may lead to some difficulties in providing an adequate bone coverage. The standard bipedicled technique did not allow the flap to cover the bone exposure without excessive tension. In our series, the reverse adipofascial flap had a single pedicle. The modified technique, because of its improved degree of rotation, granted the flap to reach either radial or ulnar tissue lo

Scapulothoracic Dissociation: Evaluation and Management.

Scapulothoracic dissociation is a rare, potentially limb- and life-threatening injury of the shoulder girdle. The injury is characterized by lateral displacement of the scapula resulting from traumatic disruption of the scapulothoracic articulation. The typical physical examination findings consist of substantial swelling of the shoulder girdle, along with weakness, numbness, and pulselessness in the ipsilateral upper extremity. Radiographic evaluation includes measurement of the scapular index on a nonrotated chest radiograph and assessment for either a distracted clavicle fracture or a disru
