Upper Extremity Return to Sport Functional Testing: A Systematic Review.

BACKGROUND: Upper extremity return to sport (RTS) assessments are not standardized with respect to which metrics to use, if the metrics are appropriate for all populations and levels of competition, and what constitutes good or poor test performance. Subsequently, clinicians may utilize suboptimal metrics to evaluate RTS readiness, or forgo objective criteria altogether. The purpose of this study was to examine the psychometric properties (reliability, agreement/measurement error, hypothesis testing/construct validity, criterion/predictive validity, responsiveness) of upper extremity functiona

Reframing diagnostic reasoning: the Bayesian imperative in shoulder examination.

Despite the widespread use of clinical shoulder tests such as the Jobe or Neer test, diagnostic reasoning often remains binary and insufficiently quantitative. Moreover, reported diagnostic accuracy varies substantially across studies, with sensitivity, specificity, and likelihood ratios typically presented as ranges that do not adequately reflect study weighting or uncertainty. In this Perspective, we apply a Bayesian framework to formalize diagnostic reasoning by integrating pre-test probability with pooled likelihood ratios, using published data from Hegedus et al. (2012). This approach all

Closed Reduction Internal Fixation with Titanium Elastic Nailing System for a Proximal Humerus Shaft Comminuted and Segmental Fracture.

Segmental fractures of the proximal humeral shaft are rare and challenging to manage, particularly in elderly osteoporotic patients with multiple co-morbidities. Traditional open fixation methods carry risks of soft tissue complications, neurovascular injury, and delayed recovery. Minimally invasive techniques such as Closed Reduction and Internal Fixation (CRIF) with Titanium Elastic Nailing System (TENS) offer an alternative, though their use in segmental proximal humeral fractures remains underreported. We report the case of a 72-year-old female with diabetes mellitus, hypertension, and cor

Comparative Study of Functional and Radiological Outcomes of Antegrade Versus Retrograde Titanium Elastic Nailing for Pediatric Ulnar Diaphyseal Fractures.

Background Pediatric forearm fractures account for approximately 40% of childhood skeletal injuries, with ulnar fractures being particularly prevalent. Titanium Elastic Nailing (TEN) offers an effective minimally invasive option for these injuries; however, the optimal nail entry point - antegrade (olecranon) versus retrograde (distal ulna) - remains debated. This study compared functional and radiological outcomes, complication profiles, and implant-related morbidity between the two approaches. Methods A retrospective comparative study was conducted from May 2023 to May 2025 at the R.L. Jalap

Segmental Momentum Sequencing: Implication for Injury and Performance Between Collegiate Pitchers With Different Arm Slots.

BACKGROUND: Baseball pitching requires precise coordination of the upper extremities to generate and transfer angular momentum efficiently while minimizing joint loading. Although the importance of temporal sequencing is recognized, few studies have examined how segmental momentum sequences vary across arm slot styles. Therefore, the purpose of this study was to evaluate the temporal sequence of peak angular momentum across segments and determine its capacity to predict ball velocity and upper extremity joint loading among overhead (OH), three-quarter (QT), and sidearm (SA) collegiate pitchers

[Management of humeral shaft fractures in childhood and adolescence : Consensus report of the Pediatric Traumatology Section of the DGU].

Humeral shaft fractures are rare in childhood, accounting for less than 5% of all pediatric fractures. Indirect trauma predominates in younger children, whereas direct trauma is more common in older patients. To date, no uniform recommendations exist regarding diagnostic evaluation or treatment management. Therefore, during the 13th and 14th scientific meetings of the Pediatric Traumatology Section (SKT), the available evidence was reviewed, the management strategies were critically discussed within an expert panel and a consensus was formulated. The management remains predominantly conservativ
