Pitfalls in the radiologic evaluation of extremity trauma: Part I. The upper extremity.

Family physicians often are required to evaluate patients who present with acute skeletal trauma. The first of this two-part series discusses the features and evaluation of some commonly missed fractures and dislocations of the upper limb, excluding the hand. Dislocations of the sternoclavicular joint are infrequent and often missed. Clavicular fractures in adults usually are not hard to diagnose. Acromioclavicular joint dislocations represent about 10 percent of all dislocation injuries to the shoulder girdle. Forty percent of all dislocations occur at the glenohumeral joint. Scapular fractur

Lower thoracic spinal cord injury--a severe complication of shoulder dystocia.

Fundal pressure as a maneuver for the relief of shoulder dystocia is associated with up to a 77% fetal injury rate. The usual injuries involve the brachial plexus or orthopedic injuries. We now report a severe lower thoracic spinal cord injury with permanent neurological injury when fundal pressure was applied in an attempt to relieve shoulder dystocia. Shoulder dystocia occurred in a 28-year-old nulliparous woman. A series of manual maneuvers to include episiotomy extension, McRoberts, suprapubic pressure, Woods screw, and extraction of the posterior arm all failed to achieve delivery. During

Neurologic deficit and recovery in the donor limb following cross-C7 transfer in brachial-plexus injury.

Two adult patients with post-traumatic complete brachial plexus injury were treated with cross-C7 nerve-root transfer from the contralateral side. A detailed documentation of the motor and sensory functional status of the donor upper limb was carried out before and after division of the C7 nerve root. Gross motor deficits were observed immediately after surgery in shoulder extension and adduction, elbow extension, forearm pronation, wrist flexion, and hand movement. Sensory deficit involving the hand was maximal immediately after division of the C7 root. Functional recovery was rapid starting

Major ablative procedures in orthopaedic surgery.

In the presence of the notable progress in limb-sparing techniques afforded by the developments in microsurgery and musculoskeletal oncology, major ablative surgery of the extremities still remains a last-resort, yet powerful tool in managing patients with primary tumors in whom wide excision is not possible, as well as in cases with severe trauma to the limbs. During the last thirteen years, eight major ablative procedures were performed at the Department of Orthopaedic Surgery of the University of Ioannina Medical School. Seven out of the eight procedures were performed in patients with prim

Rehabilitation of patient with brachial plexus lesion and break in axillary artery. Case study.

This paper describes the physiotherapy and occupational therapy used in treating a 74-year-old woman with a left brachial plexus lesion, a break in the axillary artery, dislocation of the acromioclavicular joint, a broken scapula and clavicula, serial left rib fractures, and lacerations on the upper and lower arm. After testing the patient, the following goals were set: reduce pain, soften scar tissue, and improve joint motion, muscle strength, and functionality of the hand. A 12-month outpatient program was used. Various analgesics were used to reduce pain, and a special aid was made to unwei

Shoulder arthroscopy in the throwing athlete.

The throwing athlete is a unique patient. High demands on the shoulder, combined with poor tolerance for slight changes in performance, make this a particularly difficult group of patients to treat successfully. In addition, there is overlap between multiple pathologic conditions in the shoulder, including rotator cuff tendinopathy, shoulder instability, SLAP lesions, acromioclavicular arthrosis, and scapulothoracic problems. These disorders rarely occur in isolation in the throwing athlete. Arthroscopy of the shoulder may be invaluable as a diagnostic tool to confirm clinical and radiographic

Upper limb musculoskeletal disorders in highly repetitive industries: precise anatomical physical findings.

Physical assessment of 146 female workers in highly repetitive jobs found 54% to have evidence of musculoskeletal disorders in the upper limb that are potentially work-related. Many workers had multiple problems, and many were affected bilaterally (33% of workers). Muscle pain and tenderness was the largest problem, both in the neck/shoulder area (31%) as expected and in the forearm/hand musculature (23%), a previously unreported site. Most forearm muscle problems were found on the extensor side. Carpal tunnel syndrome was the most common form of neuritis with 16 people affected (7 people affe

Imaging of shoulder instability.

Multiple imaging modalities are used in the evaluation of the patient with known or suspected instability. Plain films are often the initial study and are useful in detecting fractures and assessing alignment. Magnetic resonance imaging, with its multiplanar capabilities, such as that of visualizing soft-tissue structures, is rapidly becoming the study of choice for shoulder instability. Ultrasonography and computed tomography arthrography have limited roles in the evaluation of the patient with instability.

Complex fracture patterns of the upper extremity.

Combined skeletal injuries to the upper limb present complex management problems because they may result in dysfunction of the terminal unit of the upper limb-the hand. This article explores complex patterns of skeletal and soft tissue injuries, including complex soft tissue trauma with closed and open fractures; the floating shoulder, elbow, and forearm; and the role of skeletal fixation in conjunction with the complex fracture patterns. As the functional capacity of the upper limb reflects an integration of all its component parts, a combined approach to skeletal and soft tissue trauma is of

[Lesions of the supra-aortic arterial trunks by closed trauma].

Thirteen patients treated for 14 arterial lesions of the supra-aortic trunks from blunt trauma between 1985 and 1993 were retrospectively reviewed. In 2 cases, the lesion was on the innominate artery, in 1 case on the right common carotid artery and in 11 cases on the subclavian artery. It consisted of rupture in 8 cases, pseudoaneurysm in 5 and occlusion from an intimal flap in 1 case. Acute ischemia of the upper limb was present in 5 patients, and severe local hemorrhage in 1 patient. Four patients (5 lesions) did not have symptoms on initial hospitalization. Two later suffered embolisms fro

Blunt injury to the subclavian or axillary artery.

BACKGROUND: This study is an analysis of our experience with blunt injuries to the subclavian or axillary artery in order to delineate the immediate challenges encountered and to assess immediate and long-term outcome. STUDY DESIGN: A retrospective analysis was performed of 15 consecutive patients treated for a blunt injury to the subclavian or axillary artery in our hospital between 1988 and 1992. RESULTS: Ten patients had multiple injuries, and six were in a state of shock at admission. Bleeding from the artery contributed significantly to shock in three patients. Ischemia of the upper ext

The neurovascular outcome of scapulothoracic dissociation.

PURPOSE: The purpose of this study was to define the role of acute revascularization in the management of scapulothoracic dissociation (SD), a rare injury of the upper extremity in which severe traction to the shoulder girdle results in combined arterial, brachial plexus, and musculoskeletal injury. METHODS: The results of a retrospective study of 11 patients with SD who were compared with 41 patients from the literature are presented. RESULTS: All 11 patients were involved in high-speed accidents. All patients had chest wall hematoma, absent radial pulse, and complete brachial plexus palsy.

Histo-pathological findings of abraded skins in the cases of automobile dragging.

We histologically examined skin which had been abraded by automobile dragging. In the first case, the victim was a 57-year-old drunken male. He was hit by a passenger car while walking on a road and dragged for a distance of 30 m. He soon died from the pelvic bone fractures. Extensive and deep abrasions were observed on both shoulders and the back. In the second case, a 20-year-old drunken male was lying on a road. He was run over by a passenger car and dragged for a distance of 800 m. During the dragging the car's underbody strongly compressed his thoraco-abdominal region and killed him by ca

Posttraumatic arteriovenous fistula and subclavian vein thrombosis: treatment by percutaneous arterial embolization and vein angioplasty.

A 71-year-old woman had edema and venous dilatation of her upper right limb that caused painful functional disability following a shoulder injury. Arteriograms demonstrated an arteriovenous fistula between the subclavian artery and vein associated with thrombosis of the vein at the same level. The arteriovenous fistula was found to have multiple arteriovenous communications. Because of associated distal venous thrombosis, venous drainage was retrograde through the brachial vein. The inflow arteries of the fistula were embolized and then the subclavian vein thrombosis was recanalized, dilated,

[Traumatic paraplegia with injury of the upper extremity--effects on the success of rehabilitation].

The evaluation of a greater number of patients with traumatic paraplegia and tetraplegia showed more than 20% of associated injuries of the upper limbs. In view of the fact that arms and shoulders of the paraplegic person take over a decisive weight-bearing function, we were interested in the effects of these special injuries on rehabilitation outcome. Injuries of the distal arm were found to have more serious repercussions for achievement of the rehabilitation aim than the relatively more impressing injuries of the shoulder girdle and the upper arm. Serious limitations were found in the range

[The arterial complications of closed injuries to the shoulder girdle].

After a period of time of 5 years, we made a review of all the patients registered at our Center. Eight cases of closed traumatism at the scapulohumeral belt, with vascular injury associated (0.2% of all closed traumatisms at this level registered in our center during this time) were reported. Six patients presented also an osteoarticular injury associated. Three different vascular injuries were showed: three cases of complete arterial section, two arterial contusions and a partial arterial section. From the two cases without osteoarticular injuries, one of them showed a complete section and t

Traumatic scapulothoracic dissociation: case report.

A 23-year-old man suffered traumatic scapulothoracic dissociation (TSD) in a car-vs.-bicyclist accident. TSD is a devastating forequarter injury characterized by brachial plexus damage, major upper extremity musculoskeletal disruption, and exsanguinating hemorrhage: our patient survived a hematocrit of 7. Prompt recognition and aggressive management of TSD's multiple injuries are crucial.

Work posture, workstation design, and musculoskeletal discomfort in a VDT data entry task.

Self-report data on musculoskeletal discomfort were collected from several hundred VDT users in two agencies of a state government. Aspects of worker posture and workstation design were objectively assessed for 40 of the VDT users. Multiple regression analyses were used to examine the relationship between these ergonomic variables and musculoskeletal discomfort. Effects of ergonomic factors on musculoskeletal discomfort were clearly evident in the analyses. Regression models explained up to 38% of the variance in discomfort at different body sites. Of special interest was that leg discomfort i

[Results of treatment of compartment syndrome of the upper arm].

Only a small number of cases of compartment syndrome in the upper arm has been reported in the literature. The authors have reviewed 14 patients with 14 cases of compartment syndrome treated at their institution from 1980 to 1988. In the majority of cases in this series, compartment syndrome was caused by blunt, high-energy trauma. There were 9 patients with multiple trauma, 7 of whom were motor cyclists, and fracture of the upper arm was present in most of these. In 5 patients scapulothoracic dissociation with disruption of the neurovascular bundle of the upper extremity concerned was present
