Nonoperative management of terrible triad injuries: clinical outcomes of 39 cases.

BACKGROUND: Terrible triad injury (TTI) poses therapeutic challenges in joint stability and functional recovery. While surgery has traditionally been preferred, recent interest in nonoperative management for select cases has emerged, with studies suggesting positive outcomes for carefully chosen patients. METHODS: This retrospective study, conducted at a tertiary medical center, included patients treated conservatively for TTI between 2014 and 2022. Patients younger than 18 years with open or pathological fractures or multiple injuries were excluded. Conservative treatment was chosen for elbo

A Prospective Comparative Study of Clinical Outcomes of Hook Plate and Endobutton Fixation Techniques for Acromioclavicular Joint Dislocation in Jharkhand, Eastern India.

BACKGROUND: Acromioclavicular joint dislocations (ACJDs) are prevalent shoulder injuries that often lead to significant disability, necessitating surgical repair to regain joint stability and function. The evolution of surgical techniques and the development of various implants have introduced multiple treatment modalities, among which the hook plate (HP) and the endobutton (EB) have gained prominence. The HP and EB are prominent implants used for this purpose, each offering distinct advantages and potential complications. AIM AND OBJECTIVE: This study aimed to evaluate the clinical outcomes

Patient characteristics are more important than preoperative radiographic factors in predicting the risk of acromial stress fracture following reverse total shoulder arthroplasty.

BACKGROUND: Acromial stress fractures (ASFs) after reverse total shoulder arthroplasty (rTSA) can have a devastating effect on a patient's overall function, causing decreased range of motion and patient satisfaction. The purpose of this study was to evaluate preoperative patient demographics and radiographic findings associated with risk for ASF after rTSA. METHODS: A retrospective review was performed on patients who underwent primary rTSA by a single surgeon between December 2011 and December 2022. Inclusion criteria were primary rTSA with minimum 6-month postoperative radiographs. Preopera

Fracture type, treatment, and outcome of humeral shaft fractures in polytraumatized patients: a case series.

Since most clinical studies on humeral shaft fractures exclude polytraumatized patients, the epidemiology in this population is largely unknown. The aim of this study was to describe the fracture type, treatment, and outcome of humeral shaft fractures in adult polytraumatized patients. A case series with a single follow-up questionnaire was performed in patients aged 16 years or older with a humeral shaft fracture and an injury severity score of 16 or higher, admitted to a level 1 trauma center between January 1, 2007, and July 31, 2021. Details on injuries, treatment, and clinical outcome wer

Relationship between the elasticity of the forearm flexor-pronator muscles and elbow valgus torque in young baseball pitchers: a descriptive laboratory study.

BACKGROUND: The forearm flexor-pronator muscles act as a dynamic elbow stabilizer against elbow valgus load during baseball pitching. The elasticity of these muscles increases with pitching. However, it is unclear whether increased muscle elasticity is associated with greater elbow valgus torque during pitching. This study aimed to determine the relationship between the elasticity of the forearm flexor-pronator muscles and elbow valgus torque during pitching in young baseball pitchers. METHODS: We recruited 124 young baseball pitchers aged 9 to 12 years. The exclusion criteria included curren

Efficacy of Neuromuscular Ultrasound in Diagnosing and Treating a Mammogram-Induced Brachial Plexus Injury-A Case Report.

A 40-year-old woman presented with chronic left anterior shoulder, upper arm, and axillary pain following a routine mammogram 3 years prior. Despite multiple interventions, her pain persisted significantly affecting her quality of life. Ultrasound examination revealed fascicular edema in the medial brachial cutaneous nerve (MBCN), intercostobrachial cutaneous nerve (ICBN), and a positive sono-Tinel. Ultrasound-guided hydrodissection of these nerves provided complete axillary pain relief and significant improvement in anterior arm pain, sustained for 6 months. This case highlights the diagnosti

Thirty-Eight-Year Follow-Up After Multiple Toe-to-Hand Transfers.

Replantation of damaged digits is the preferred treatment for traumatic pediatric hand injuries, but, when replantation is not feasible, second toe-to-hand transfers have been reported since the 1960s as a reconstructive method. This case report discusses a 38-year follow-up of a pediatric patient who underwent bilateral second toe-to-hand transfer after a riding lawn mower accident resulted in traumatic amputations of her right index through little fingers. At follow-up, the patient's hand was assessed for functionality, including range of motion, grip strength, and sensation. The Disabilitie

Ligament Sparing Elbow Hemiarthroplasty: A Novel Technique for the Management of Distal Humeral Fractures.

Intra-articular distal humerus fractures present various challenges with a wide array of treatment options. Open reduction internal fixation remains the treatment of choice. In older patient populations with poor bone quality and short-end segment fractures with articular comminution, open reduction internal fixation, however, may bring on unsurmountable technical challenges. Total elbow arthroplasty and elbow hemiarthroplasty (EHA) may offer superior functional outcomes in these cases. During EHA for fractures, the medial and lateral columns are reconstructed with the collateral ligaments to
