Neuromuscular signatures differentiating functional and impaired cases in full-thickness rotator cuff tears.

BACKGROUND: The relationship between structural damage and functional impairment in patients with full-thickness rotator cuff tears (FT-RCTs) remains inconsistent. While some patients maintain high function despite large tears, others experience profound disability, suggesting that compensatory neuromuscular mechanisms may be more closely associated with functional status than with anatomical integrity. This study aims to identify the clinical, structural, and neuromuscular factors that differentiate high-functioning from poor-functioning individuals with FT-RCTs. METHODS: Fifty-three patient

Greater tuberosity fragment volume does not predict clinical outcomes or tuberosity healing after reverse shoulder arthroplasty for proximal humerus fractures: a retrospective cohort study.

PURPOSE: Greater tuberosity (GT) fragment healing has been associated with improved outcomes following reverse total shoulder arthroplasty (RTSA) for proximal humerus fractures, yet the relationship between pre-operative GT fragment volume and clinical outcomes remains largely unexplored. We hypothesised that larger GT volumes would be associated with improved outcomes and larger anatomical dimensions. METHODS: We retrospectively studied 153 patients who underwent RTSA for proximal humerus fracture. GT fragment volume was measured on pre-operative computed tomography (CT). Clinical outcomes i

Is arthroscopic-assisted treatment applicable for the treatment of pediatric scaphoid nonunion?

Pediatric scaphoid nonunion is uncommon. Immobilization is initially attempted; however, it causes delays in both intervention and healing. Therefore, surgical intervention has gained broader acceptance and has been implemented in recent years. Among these, arthroscopic management has emerged as a contemporary technique. This study evaluated the clinical outcomes of pediatric scaphoid nonunion treated with arthroscopic-assisted bone grafting and Kirschner (K) wire fixation. Six pediatric patients with scaphoid nonunion treated with arthroscopic bone grafting and K-wire fixation between Novembe

Safe dosage and potential risks of chlorogenic acid: insights from in vitro and in vivo studies.

INTRODUCTION: As the economy grows, there is a growing emphasis on food safety. While the health benefits of chlorogenic acid (CGA) are recognized, safe dosages and potential liver cell damage from excessive CGA consumption are not well studied. This study aims to determine the safe and effective dose range of CGA and understand how it causes toxicity in hepatocyte at half-maximal inhibitory concentration (IC50). METHODS: This study assessed the impact of various CGA concentrations on liver cells, examining growth, viability, toxicity, energy metabolism, and colony formation using Real-Time C

Intramedullary K-Wire with Immediate Mobilisation versus Percutaneous Transverse K-Wire Fixation for Transverse Metacarpal Shaft Fractures.

Background: Transverse metacarpal shaft fractures commonly require surgical fixation, yet optimal postoperative management remains controversial. Although intramedullary (IM) Kirschner wire (K-wire) fixation allows early motion, many surgeons continue to apply postoperative splinting due to concerns about stability. We aim to determine if a single IM K-wire fixation without immobilisation yields better outcomes and faster recovery of hand function. Methods: A single-centre, prospective and randomised controlled trial was conducted between November 2024 and November 2025 involving isolated tran

Therapeutic Strategies and Predictors of Functional Impairment in Hand Burns: A Retrospective Single-Center Study.

Hand burns pose complex clinical challenges due to the intricate anatomy of the hand and its indispensable role in daily activities. In this retrospective cohort study, we evaluated 88 adult patients admitted to a specialized burn center with burns involving one or both hands. We collected demographics, burn characteristics, and clinical variables from medical records, and assessed functional outcomes 1 year after injury using the Disabilities of the Arm, Shoulder, and Hand (DASH) score. Nonparametric tests compared patient subgroups, a 6-factor multivariable regression (age, %TBSA, depth, dig

Articular impaction of olecranon fracture is associated with poor postoperative clinical outcome.

AIMS: Articular impaction in olecranon fractures has been under-recognized. This study aimed to evaluate the clinical implications of articular impaction, and to identify factors associated with poor postoperative outcomes following open reduction and internal fixation (ORIF). METHODS: Patients who underwent ORIF for olecranon fractures were retrospectively reviewed. Articular impaction was assessed using preoperative CT scans, and articular impaction reduction quality was categorized based on residual depression. Functional outcomes were assessed using the Mayo Elbow Performance Score (MEPS)

Operative management of self-inflicted upper extremity amputations: A retrospective case series.

INTRODUCTION: Self-inflicted upper extremity amputation is a rare injury which leaves surgeons with unique operative considerations. Although psychiatric decompensation is traditionally considered a contraindication to replantation, available evidence is limited to isolated case reports and small series. Hence, we sought to describe the injury characteristics, psychiatric comorbidities, operative decision-making, and clinical outcomes of patients with self-inflicted upper extremity amputations. METHODS: A retrospective review of traumatic upper extremity amputations and replantations was perf

Scapulothoracic Dissociation in Contemporary Practice: Revisiting Clinical Reality Through the Trauma Quality Improvement Program.

BackgroundScapulothoracic dissociation is a rare, limb-, and potentially life-threatening injury in which the scapula and shoulder girdle are violently detached from the thoracic cage. However, the published data on the condition is mainly composed of case reports and single institution samples, which limits the overall generalizability. The aim of the current investigation was consequently to use a multi-institutional data set to characterize the injuries observed in patients with scapulothoracic dissociation along with the treatment strategies selected.MethodsThe 2016-2021 Trauma Quality Imp

[Dislocation of the acromioclavicular joint].

Dislocations of the acromioclavicular joint (ACJ) are among the most common shoulder injuries, particularly in young, active patients. The cause is usually a direct trauma (fall, impact) or a fall on the outstretched arm. Injuries are classified according to the Rockwood system. For diagnosis in the acute case, in addition to clinical examination, panorama and axillary views are essential to assess vertical and horizontal instability. While types-I-II are commonly treated conservatively, types-IV-VI are generally considered indications for surgery. The management of types-III and -V remains co

SECEC Grammont Award 2025: the resting arm position influences impingement-free range of motion and the distalization and lateralization shoulder angle in reverse shoulder arthroplasty using Imascap preoperative planning software.

BACKGROUND: Preoperative planning software enables virtual implantation of reverse total shoulder arthroplasty (rTSA) and assessment of impingement-free range of motion (ROM). Recent studies have shown that scapular resting position varies significantly among individuals and impacts impingement-free ROM. The resting position of the arm also varies between individuals, yet its impact on simulated outcomes and whether this parameter should be integrated into planning programs remains unclear. The objective of this study was to assess the effect of the resting arm position in the coronal plane on
