BACKGROUND: Proximal humerus fractures (PHFs) are becoming more common, especially among older adults, and often require surgical management. Two widely used surgical techniques are intramedullary nailing (IMN) and locking plate fixation (open reduction and internal fixation), yet there is ongoing debate regarding their comparative effectiveness. This study aims to systematically evaluate and compare functional, clinical, and complication-related outcomes of IMN and plating for adult patients with displaced PHFs.
METHODS: A systematic review and meta-analysis was conducted in accordance with

Hook plates are low-profile implants commonly used for olecranon fractures and osteotomies, offering stable fixation especially in complex or osteoporotic fractures. This study evaluates the clinical and functional outcomes of hook plate fixation around the elbow. We retrospectively analyzed 20 patients (mean age 52.7 years) treated between 2016 and 2023 for olecranon or distal humerus fractures using hook plates alone or combined with other implants. Inclusion criteria were patients aged 16 years or older with at least 12 months of follow-up. Exclusion criteria included ASA ≥ IV, open fractur

Distal radius fractures are among the most prevalent fractures, accounting for approximately 17% of all fractures treated. Given the high prevalence of these fractures, particularly in the older adults population, determining the optimal treatment approach is of utmost clinical importance. While previous research has highlighted both the advantages and drawbacks of surgical and conservative treatment strategies, there remains a lack of consensus regarding which approach yields superior outcomes in terms of functional recovery and quality of life (QoL). This study was designed to determine whet

OBJECTIVE: The optimal treatment strategy for intra-articular distal radius fractures with loss of reduction in elderly patients remains unclear, particularly in AO/OTA 23-C1 and C2 fracture types. This study aimed to compare the clinical and radiological outcomes of surgical versus conservative treatment in elderly patients who developed secondary displacement after initial reduction.
METHODS: In this retrospective multicenter study, 127 patients aged ≥ 65 years with AO/OTA 23-C1/C2 distal radius fractures and secondary displacement during follow-up were analyzed. Patients were managed eithe

Titanium volar locking plates are widely used in distal radius fractures (DFRs) fixation, but their induction of radiographic shielding hinders fracture assessment. Carbon fiber-reinforced polyether ether ketone (CFR-PEEK) plates are radiolucent and biomechanically comparable to titanium, potentially improving intraoperative visualization and postoperative follow-up. This study aims to compare clinical and radiographic outcomes between CFR-PEEK and titanium plates.MethodsThis retrospective study included 39 patients with DRFs treated with either CFR-PEEK (n = 19) or titanium plates (n = 20) be

OBJECTIVE: This finite element analysis compared the biomechanical performance of two surgical approaches for Neer Type II distal clavicle fractures: isolated locking plate fixation versus locking plate fixation combined with anatomical double-bundle coracoclavicular ligament reconstruction.
METHODS: A three-dimensional finite element model of the shoulder was developed from the CT scan of a 30-year-old healthy male. Three conditions were simulated: an intact joint (Group A, control), a fractured joint fixed with a locking plate alone (Group B), and a fractured joint fixed with a locking plat

BACKGROUND: Proximal humerus fractures are common injuries in the elderly, frequently treated with plate osteosynthesis. Intraoperative 2D fluoroscopy provides only limited visualization of the humeral head's complex anatomy, often leading to undetected intra-articular screw perforations. Intraoperative 3D imaging provides enhanced assessment but is technically limited by the confined operating field and the risk of collisions between the C-arm, patient, and operating table. This cadaveric study systematically investigated how progressive reduction of orbital rotation during intraoperative 3D

BACKGROUND: Heterotopic ossification (HO) around the shoulder is common in patients with clavicle fracture or acromioclavicular joint (ACJ) dislocation. Periarticular HO sometimes leads to poor joint function. HO of the elbow and hip requires active prevention. However, studies on the incidence, risk factors, characteristics, and shoulder joint function of HO around the shoulder after surgical treatment of clavicle fracture or ACJ dislocation are still lacking.
STUDY HYPOTHESIS: ACJ dislocation, clavicular hook plate fixation and excessive screw length are independent high-risk factors for po

This study reviews literature on corrective osteotomies without bone grafting for malunion of distal radius fractures (MDRF). A systematic review was performed on the treatment of MDRF without bone graft. The primary functional outcome measures were time to radiological union, Disabilities of the Arm, Shoulder, and Hand (DASH) scores, Visual Analogue Scale (VAS) scores, Mayo Wrist scores, grip strength, and range of motion (ROM) at final follow-up. A total of 285 patients across 11 studies met search criteria. Radiological union occurred in 98.9% of cases with a mean time of 3.9 months. Postop

BACKGROUND: The advent of modern, precontoured locking plates has improved treatment of distal humerus fractures (DHFs) by providing an anatomic fit and a reliable locking mechanism with interdigitating screws. However, severe articular comminution may predispose surgical failure despite anatomic reduction and a biomechanically strong construct.
METHODS: A retrospective review was performed to identify patients who underwent distal humerus open reduction internal fixation (ORIF) between 2015 and 2020. Fractures were classified using the AO/OTA classification and further categorized by anatomi

Bilateral clavicle fractures represent one of the rarest patterns of shoulder girdle trauma, occurring predominantly after high-energy mechanisms and posing unique challenges in diagnosis and management. Their simultaneous disruption compromises both clavicular struts, resulting in impaired shoulder biomechanics and necessitating timely stabilization for optimal functional recovery. A 25-year-old male sustained high-velocity trauma following a motorcycle accident and presented with severe bilateral shoulder pain, deformity, swelling, and markedly restricted arm elevation. Neurological and dist

BACKGROUND: It is undetermined whether the quality of surgical technique in a humeral shaft fracture (reduction and fixation) affects outcomes. The aim of this study was to score the quality of surgical technique performed in patients with humeral shaft fractures and to assess whether the quality of surgical technique was associated with complications and (functional) outcomes.
METHOD: A panel of expert orthopedic trauma surgeons, experienced in upper extremity surgery, assessed the quality of surgical technique (good, sufficient, or poor) in patients who participated in the HUMMER study on p
