BACKGROUND: Artificial intelligence-assisted radiographic fracture detection tools (AI-RFDT) have the potential to support confident fracture exclusion by physicians in the paediatric emergency department (ED), where presentations for suspected fractures substantially outnumber confirmed fractures. This study aimed to evaluate whether implementation of AI-RFDT in a paediatric ED is associated with reduced length of stay (LOS) among patients presenting with suspected fractures, and to determine whether any observed effect is specifically mediated through the fracture-exclusion pathway.

OBJECTIVE: Paratonia is defined as an inability to relax the muscles during tone assessment, resulting in participants either opposing or facilitating the examiner's movement involuntarily. This phenomenon is frequently found in patients with cognitive impairment and has been attributed to frontal lobe dysfunction, although a direct correlation has not been established. This study aimed to explore the correlation between frontal lobe function, as assessed by functional brain imaging, and paratonia, as assessed by electromyography.
METHODS: Memory clinic outpatients with suspected Alzheimer's

PURPOSE: Tendon injuries frequently affect the Achilles, rotator cuff, and the tendons of the wrist extensors originating at the lateral epicondyle. Until recently, terms for tendon degeneration, disease, and injuries were debated between the basic research and clinical communities, creating barriers to advances in knowledge. Aside from acute traumatic rupture in healthy tendon, in the current 'omic age, most tendon disease, including ruptures in chronic disease, exists along a tendinopathy spectrum. While intrinsic factors such as sex, genetics, and aging are unmodifiable, extrinsic factors i

Pediatric acute compartment syndrome (PACS) results from increased pressure within a fibro-osseous space, reducing local limb perfusion. It can be traumatic or non-traumatic, often due to fractures, burns, or iatrogenic compression. PACS is challenging to diagnose in children due to insidious onset. While lower extremity PACS is well studied, upper extremity PACS remains underexplored. This study analyzes characteristics and inpatient outcomes of pediatric upper extremity PACS. The National Inpatient Sample (2015 - 2019) was queried for patients under 18 diagnosed with ACS. ICD-10 codes identi

BACKGROUND: Ulnar neuropathy at the elbow (UNE) is the second most common entrapment neuropathy of the upper limb, involving physical medicine and rehabilitation, neurology, orthopedics, and plastic surgery. It results from static or dynamic ulnar nerve instability, cubital tunnel syndrome, or space-occupying lesions. Accurate diagnosis requires thorough history, clinical examination, and selection of appropriate diagnostic tests.
SEARCH STRATEGY: PubMed/MEDLINE, Scopus and Web of Science were searched (January 2000-August 2026) using combinations of "ulnar neuropathy", "cubital tunnel syndro

Traumatic segmental bone defects of the hand are uncommon injuries that may require complex reconstruction to restore skeletal stability and function. Calcium sulfate is widely used as a biodegradable bone void filler and local antibiotic carrier, although its use during the acute management of traumatic metacarpal defects has been rarely reported. We present the case of a 55-year-old man who sustained a severe open injury to his dominant hand caused by a radial saw, resulting in dissociation of the first and second rays, segmental bone loss involving approximately 40% of the first metacarpal

Elbow pain arises from diverse tendinous, ligamentous, neural, and bursal disorders involving the lateral, medial, anterior, and posterior compartments. Because clinical symptoms frequently overlap - such as epicondylitis, distal biceps tendon injury, cubital tunnel syndrome, and olecranon bursitis - precise imaging is essential for accurate diagnosis and targeted management. Ultrasonography has become a first-line modality for elbow evaluation due to its high-resolution visualization, dynamic capability, and real-time interventional guidance. This article presents the Ultrasound Musculoskelet

Background/Objectives: Radial nerve palsy complicates 8-16% of humeral shaft fractures, and because more than 90% of primary palsies recover spontaneously, the clinically decisive question is not whether a nerve will recover but when. That timeline has been defined almost entirely from blunt-mechanism cohorts, yet is routinely applied to ballistic injury. No study has directly compared radial nerve recovery between gunshot wound (GSW) and blunt trauma. We compared time to full radial nerve recovery between mechanisms within a cohort of radial nerve injuries with continuity either operatively c

RATIONALE: There have been several reports of lumbar yellow ligament hematomas leading to cauda equina and/or nerve root compression. However, based on clinical history and histological findings, most of these cases involve chronic hematomas. Acute lumbar yellow ligament hematomas causing nerve compression are relatively rare.
PATIENT CONCERNS: A 71-year-old male presented with an 8-day history of sudden-onset lower back pain without an identifiable trigger, which progressively worsened with new-onset pain and numbness in both lower extremities. Physical examination revealed muscle strength o

INTRODUCTION: Autonomic dysreflexia (AD) is a potentially fatal syndrome in people with spinal cord injury (SCI) marked by episodic hypertension with flushing, diaphoresis, and bradycardia. Common etiologies include bladder distension and bowel impaction. Compressive neuropathies lack attention as a potential precipitant of AD. This report describes an individual with cervical SCI and with recurrent AD treated with ulnar nerve decompression surgery.
CASE PRESENTATION: A 74-year-old woman with a remote history of C6 SCI (American Spinal Injury Association Impairment Scale Grade B) presented to

BACKGROUND: Distal biceps tendon ruptures significantly impair forearm flexion and supination strength, particularly in active individuals. Suspensory fixation using cortical button devices is a widely accepted surgical approach. Adjustable-loop techniques allow maximal tendon-to-bone apposition, while fixed-loop methods are simpler but may leave a gap. Biomechanical differences between these techniques specific to the distal biceps tendon remain poorly investigated, especially given the unique loading patterns of the elbow compared to other joints.
METHODS: Finite element analysis was perfor

Patella alta is a major anatomical risk factor for patellar instability. Although a tibial tubercle osteotomy with distalization has been described in this context, a patellar tendon imbrication more directly addresses the pathoanatomy and avoids the risk of physeal and apophyseal injury in skeletally immature patients. This technical note describes a transection-free patellar tendon imbrication technique using a double-row configuration that allows for predictable correction of patella alta, avoidance of risks associated with osteotomy, and the ability to medialize the patellar tendon, optimi

Distal radius fractures (DRFs) are among the most common upper extremity fractures encountered in clinical practice. Closed reduction followed by external immobilization remains a fundamental treatment approach; however, the procedure is highly operator-dependent and lacks standardization. This article presents a step-by-step protocol for manual reduction and four-splint immobilization using volar, dorsal, radial, and ulnar splints for acute DRFs. The protocol encompasses patient selection, anesthesia administration, patient positioning, sustained longitudinal traction with countertraction, re

Distal radius fracture (DRF) usually adopts static fixation method, which often leads to complications such as abnormal fracture healing and pressure injury. The reason lies in the lack of real-time regulation of fixed pressure and biomechanical feedback, which affects the functional recovery. Our team designed an intelligent and adaptive orthopedic equipment. Based on the patient's personalized three-dimensional (3D) printing architecture, the equipment system realizes light and accurate adaptation, and embeds a series of thin film pressure sensors in key anatomical parts to continuously quan
