Irreducible Volar Subluxation of the Distal Interphalangeal Joint: An Unrecognised Cause.

This report describes a rare case of irreducible closed volar subluxation of the distal interphalangeal (DIP) joint of the small finger following a sports injury. Closed reduction failed, leading to open surgery, which revealed ulnar condyle entrapment between a partially torn extensor tendon and volarly displaced oblique retinacular ligament (ORL). Flexing the proximal interphalangeal (PIP) joint to 90° while extending the DIP joint relaxed the ORL, enabling dorsal repositioning and successful reduction. Awareness of this mechanism may help hand surgeons achieve effective reduction. Level of

Effects of Biomechanical Testing Using a Synthetic Ligament Fabricated from Polyhydroxyalkanoate Biopolyesters for Lateral Ulnar Collateral Ligament Reconstruction in Cadaver.

An injury to the elbow's lateral ulnar collateral ligament (LUCL) is an orthopedic emergency that can impair joint stability and functional biomechanics throughout the upper extremity. The development and application of synthetic ligament substitutes, particularly short-chain-length and medium-chain-length polyhydroxyalkanoate (SCL-PHA and MCL-PHA) co-polymers, represent a promising innovation for lateral elbow stabilization. This experimental cadaveric study aimed to (1) compare biomechanical parameters of torque and angular rotation among control, damage, repair, and reconstruction groups an

Nerve and Tendon Transfers for Upper Extremity Reconstruction in Spinal Cord Injury Patients: A Systematic Review.

Background: Spinal cord injury (SCI) often results in upper extremity paralysis, significantly limiting independence and quality of life. Surgical interventions, including nerve and tendon transfers, offer the potential to restore critical functions such as hand grasp and elbow extension. However, the utilisation of these techniques remains limited despite promising outcomes. As such, this study aimed to systematically review the literature on the clinical outcomes of nerve and tendon transfers for upper extremity reconstruction in patients with cervical SCI. Methods: A systematic search was c

Implant Survivorship and Functional Recovery Utilizing a Modern Knee Fusion Nail Spacer for Periprosthetic Joint Infection.

BACKGROUND: Traditional indications for static spacers include substantial bone loss, ligamentous insufficiency, extensor mechanism dysfunction, or severe soft-tissue compromise. However, conventional designs are associated with high complication rates and morbidity. A knee fusion nail spacer (FNS) construct combines mechanical stability with targeted antibiotic delivery. This study aimed to (1) describe outcomes of FNS use in terms of total knee arthroplasty (TKA) reimplantation, infection eradication, and limb salvage; (2) determine FNS survivorship with reinfection as the endpoint; and (3)

Relative Motion Extension Orthosis Versus Classic Immobilization in Extensor Tendon Repairs (Zones IV-VI of the Hand): A Randomized Controlled Trial.

BACKGROUND: Traditional treatment for extensor tendon injuries typically involves tenorrhaphy followed by a 4- to 6-week period of immobilization in a plaster orthosis. This study compares the efficacy of relative motion extension (RME) orthoses versus classic immobilization (CI) in promoting recovery after such injuries. METHODS: We conducted a randomized, controlled, blind clinical trial approved by the Galicia ethics committee. Participants were individuals with single tendon injuries in the long fingers. They were randomly assigned to either the RME or the CI treatment group. Outcome meas

Enhancing Neuromuscular Conditioning in Football Players Through Single-Leg and Double-Leg Cycling: A Randomized Controlled Trial.

BACKGROUND: Football requires high levels of neuromuscular conditioning to meet the demands of explosive actions such as sprinting, cutting, and kicking, while minimizing the risk of lower limb injuries-especially hamstring strains linked to muscle imbalances. Single-leg cycling (SLC), a form of unilateral training, may offer superior neuromuscular adaptations compared to traditional double-leg cycling (DLC). This study aimed to compare the effects of SLC and DLC on lower limb strength, anaerobic power, fatigue resistance, agility, and sprint performance in competitive football players. METHO

The effect of mixed fatigue on knee biomechanics and muscle activation during sidestep cutting in elite soccer players.

BACKGROUND: Football is one of the most popular sports in the world, and it is also a sport with a high rate of injury. The study aims to investigate the effects of physical and mental mixed fatigue (PMF) on knee biomechanics during sidestep cutting maneuvers in elite male soccer players, thereby assessing the potential mechanisms underlying non-contact knee injuries. METHODS: Thirty-six elite male soccer players were recruited (age: 21.61 ± 1.22 years; body mass: 75.16 ± 6.34 kg; height: 175.8 ± 3.53 cm; shoe size: 41–44 EUR). Following a targeted fatigue induction protocol, key lower limb bi

Extensor Indicis Proprius Transfers Versus Tendon Graft in Extensor Pollicis Longus Reconstruction: A Systematic Review and Meta-Analysis.

BACKGROUND: In extensor pollicis longus ruptures not amenable to primary repair, secondary reconstruction of the tendon has traditionally been performed using either extensor indicis transposition or free tendon graft techniques. This paper constitutes a systematic review and meta-analysis of the current literature with a focus on Geldmacher's scoring to identify the effectiveness of these procedures. METHODS: We performed a literature search of medical databases to identify papers fulfilling the inclusion criteria. A meta-analysis abiding by Preferred Reporting Items of Systematic Reviews an

24-week multidimensional predictors of return to play post-ACLR in high-sports demanders: a randomized trial.

Predicting successful return to play (RTP) during mid-stage rehabilitation after anterior cruciate ligament reconstruction (ACLR) remains a critical, unresolved clinical challenge. High-sports demanders (HSDs)-non-professional athletes participating in level-1 sports (cutting, pivoting, jumping) ≥ 3 times/week with Tegner score > 5-represent a distinct subgroup who face unique rehabilitation challenges yet lack evidence-based mid-stage prediction tools. The primary objective of this study was to identify multidimensional predictors at 24 weeks post-ACLR for successful RTP at 48 weeks, and to c

Pull Out Suture Technique for Fixing Coronoid Fractures using a Single Lateral Extensor Digitorum Communis Split Approach and Fiberwire Endobutton Fixation by Retrograde Drilling - A Case Series.

INTRODUCTION: Coronoid fractures, while less common than other elbow injuries, present with significant challenges in maintaining elbow stability and function. Restoring the normal anatomy of the coronoid process (CP) is crucial for stabilizing the elbow joint, with surgical intervention often necessary for displaced or comminuted fractures. Suture button fixation offers advantages, such as ease of use, bone-sparing fixation, and reduced reliance on intraoperative X-rays. It is especially effective for small or fragmented fractures, with tools, such as aiming devices and repositioning pliers a

Reconstruction of the Extensor Carpi Ulnaris Subsheath Using a Fascia Lata Allograft.

Extensor carpi ulnaris subsheath injuries range from instability, painful snapping, to chronic tenosynovitis. While acute cases often respond to immobilization, surgical intervention is required for refractory cases. This study introduces a novel reconstruction technique using a fascia lata allograft to create an anatomical, radial-based sling under the extensor reticulum. This approach avoids autograft harvesting, preserves the original tendon gliding pathway, is applicable to all types of injury, and enables simultaneous treatment of intrinsic extensor carpi ulnaris pathologies and ulnocarpa

Cross-education effects of healthy-side lower-limb strength training on neuromuscular function recovery following anterior cruciate ligament reconstruction: a randomized controlled trial.

OBJECTIVE: Based on cross-education theory, this study aimed to investigate whether Healthy-Side Lower-Limb Strength Training could promote neuromuscular function recovery of the injured limb in patients following anterior cruciate ligament reconstruction (ACLR) through central nervous system-mediated contralateral adaptive mechanisms. METHODS: A single-masked, randomized controlled trial was conducted with 40 patients at 5-6 weeks post-ACLR, who were randomly allocated to intervention and control groups (n = 20). The control group received conventional rehabilitation, while the intervention

Immobilization versus Early Active Mobilization after Zone 5-6 extensor tendon repair.

BACKGROUND: Extensor tendon injuries in the dorsal hand are prevalent due to the superficial location of the tendons and limited surrounding soft tissue. Early active mobilization (EAM) protocols are increasingly being adopted; however, comparative data with traditional immobilization methods remain scarce. PURPOSE: This study aims to compare the short-term functional outcomes and return-to-work times between EAM and immobilization following zone 5-6 extensor tendon repairs. STUDY DESIGN: A retrospective cohort study. METHODS: We analyzed twenty-six patients with isolated zone 5-6 extensor

Pre- and postsurgery measurements of continuous muscle compartment pressure in patients with extremity trauma.

BACKGROUND: Continuous pressure measurement of traumatized muscle has been shown to be an accurate method of diagnosing acute compartment syndrome (ACS), but no baseline data have been released to show the reaction to surgery in extremity fracture. We sought to establish normal values for perioperative pressures. METHOD: We obtained pressure tracings from prospective clinical trials of forearm and tibia fractures requiring surgery. We standardized data and produced graphs using the ggplot2 library in R version 4.2.2. We generated generalized pressure curves based on the mean pressure and quan

The extensor digitorum brevis muscle flap in lower limb reconstruction: A long-term follow-up.

BACKGROUND: The extensor digitorum brevis flap (EDB) is a small local muscle flap used primarily for the reconstruction of small tissue defects in the lower leg. Few studies investigate the long-term outcomes of the EDB flap, and concerns have been raised regarding long-term morbidity. The aim of this study was to investigate the long-term functional outcomes of the EDB flap. METHODS: All patients who had a lower limb reconstruction using a pedicled EDB muscle flap between 1997 and 2021 were identified and asked to participate in the study. Twenty-one patients agreed to participate and were i
