Examining Motor Unit Properties of Upper Limb Muscles Near the Zone of Injury in Chronic Cervical Spinal Cord Injury.

Spinal cord injury (SCI) causes muscle weakness or paralysis, with functional deficits partly due to motor neuron damage near or below the injury level. Understanding how damage affects the morphology and function of motor neurons and the muscle fibers they innervate (i.e., motor units [MUs]) is important for characterizing motor pathophysiology. We assessed MU electrophysiological properties in muscles innervated near the injury zone (anconeus: C7; extensor indicis [EI]: C8-T1) in motor-incomplete cervical SCI and controls. The EI of 14 SCI participants (48.2 ± 14.6 years; 1 female; C2:1, C4:

Floating Knee Severity Score (FKS): A Novel Multidimensional Prognostic Model for Predicting Functional Outcomes After Floating Knee Injuries.

Purpose: Floating knee (FK) injuries are complex high-energy traumas associated with poor functional outcomes. This study aimed to identify independent predictors of functional prognosis and to develop a novel, multidimensional scoring system to predict long-term functional outcomes. Methods: A retrospective analysis was performed on 182 adult patients with ipsilateral femur and tibia fractures treated between January 2010 and December 2023. Functional outcomes were assessed using the Karlström-Olerud criteria and dichotomized as excellent-good versus fair-poor. Variables significant in univar

Serial Changes in Knee Muscle Strength and Functional Performance After Anterior Cruciate Ligament Reconstruction: A Retrospective Cohort Study of 107 Patients.

Background and Objectives: Anterior cruciate ligament (ACL) reconstruction (ACLR) is widely performed to restore knee stability and facilitate return to activity. However, recovery of muscle strength, balance, functional performance, and patient-reported outcomes does not occur uniformly over time. The longitudinal recovery trajectory across various functional areas during the first year after ACLR remains insufficiently characterized. Materials and Methods: We included 107 patients who underwent isolated unilateral ACLR using a hamstring autograft. Isokinetic knee extensor and flexor strength

Comparing kinematic and kinetic demands on the knee joint during selected physiotherapy exercises and activities of daily living.

BackgroundThe knee is one of the most common areas to suffer injuries or be affected by surgery. Physiotherapy rehabilitation was shown to support recovery, but evidence guiding optimal rehabilitation practices is limited. To recommend appropriate exercises, it is essential to understand the musculoskeletal requirements involved in both physiotherapy and activities of daily living (ADLs).ObjectiveThis study aimed to evaluate and compare the knee joint kinematics, joint forces and muscle activity in knee flexors and extensors during selected rehabilitation exercises and ADLs.MethodsKinematic an

Delayed extensor pollicis longus tendon rupture after minimally displaced distal radius fracture: a report of two cases.

INTRODUCTION: Extensor pollicis longus (EPL) tendon rupture is an uncommon but serious complication following nondisplaced or minimally displaced distal radius fractures. CASE PRESENTATION: We present two cases of delayed EPL tendon rupture. A 61-year-old woman and a 41-year-old man both developed inability to extend the thumb several weeks after conservative treatment of minimally displaced distal radius fractures. Intraoperative findings revealed poor tendon quality and significant retraction. Both patients were managed with palmaris longus tendon graft using the Pulvertaft weave technique.

Automatic measurements of volar tilt and radial inclination of the distal radius on 3D models: validation against manual methods.

Assessing distal radius malunions traditionally relies on measuring volar tilt and radial inclination in 2D radiographs. Reliability of these parameters is limited by projection errors and landmark ambiguity. Using 3D models allows more precise quantification of these parameters. This study validated an algorithm for measuring volar tilt and radial inclination on 3D virtual models of malunited and healthy radii. The algorithm establishes a 3D coordinate system and anatomical landmarks replicating conventional 2D definitions to maintain clinical compatibility. Automatic 3D measurements on model

Trans-tibial tension band wiring versus heavy suture fixation for type II periprosthetic patellar fractures after total knee arthroplasty: A comparative cohort study.

BACKGROUND: Periprosthetic patellar fractures (PPPFs) following total knee arthroplasty (TKA) remain a challenging complication with high rates of fixation failure and functional impairment. Optimal surgical management for type II PPPF remains unclear. This study compared outcomes between transtibial tension band wiring (TTTBW) and heavy suture fixation (HSF). METHODS: A retrospective review identified 18 patients with type II PPPF treated between 2020 and 2023 (HSF, n = 12; TTTBW, n = 6). Primary outcomes included implant survival and extensor mechanism function, assessed by extension lag, f

Influence of foot arch height on isokinetic knee muscle strength at varying angular velocities.

BACKGROUND: The height of the foot arch is highly related to knee joint function. Knee joint strength is a significant contributing factor to knee joint injuries, while existing research has not yet clearly established the correlation between arch height and knee joint strength. OBJECTIVE: This study aims to investigate the influence of arch height on isokinetic knee muscle strength at varying angular velocities, thereby providing a potential scientific basis for the prevention and rehabilitation of associated conditions. DESIGN: Cross-sectional study. PARTICIPANTS: Sixty healthy young adul

Further Displacement After Initial Nonsurgical Treatment of Minimally Displaced Olecranon Fractures in Children.

INTRODUCTION: Pediatric olecranon fractures are uncommon periarticular injuries with unclear treatment guidelines for varying magnitudes of intra-articular displacement. Similar to other pediatric elbow fractures, minimally displaced fractures are treated nonsurgically, and potential for further displacement following nonsurgical treatment exists. This study assesses the incidence and risk factors for further displacement after nonsurgical treatment of minimally displaced pediatric olecranon fractures. METHODS: A retrospective review was conducted on patients aged 0 to 15 years with isolated

Beyond The Sprain, Unmasking a Rare Anterolateral Ankle Pathology in a Young Athlete.

INTRODUCTION: Chronic anterolateral ankle pain in young athletes is commonly attributed to ligament sprains, stress injuries, or tendinopathies. However, uncommon underlying pathologies may occasionally be responsible for persistent symptoms. Calcaneonavicular coalition is a congenital condition that can lead to chronic ankle pain and restricted hindfoot motion. The coexistence of calcaneonavicular coalition with a superficial ankle lipoma is extremely rare. CASE REPORT: An 18-year-old female cricket athlete presented with gradually progressive swelling and pain over the anterolateral aspect

Biomechanical study on different internal fixation methods for treating Mayo type IIA olecranon fractures of the ulna.

The aim of this experiment is to compare the biomechanical strength of six distinct internal fixation techniques for Mayo type IIA olecranon fractures using biomechanical analysis. This study utilized tensile tests on artificial, shape-mimicking olecranon bones to assess their biomechanical properties. A tensile test was performed on the artificial, shape-mimicking olecranon bone at a 90° angle, with the tensile load applied at a rate of 2 mm/min until the test displacement reached 2 mm, at which point the test was halted. Throughout the test, the testing system was able to collect load and di

Validation of the Acute Lower Limb Activation Mechanism Test: A Reliable Bedside Diagnostic Tool for Extensor Mechanism Ruptures.

Modern clinical practice has become increasingly reliant on imaging modalities to diagnose the extent of soft tissue injuries. We describe and then validate a novel bedside clinical test, the acute lower limb activation mechanism test (ALAM), that can be used to identify an extensor mechanism rupture in those unable to straight leg raise (SLR). Patients who presented with an acute knee injury and were unable to SLR were identified over a 6-year period. Patients meeting the inclusion criteria were examined utilizing the ALAM test and diagnosis subsequently confirmed with ultrasound, magnetic re

Combined Open Sagittal Hood Repair and Metacarpophalangeal Joint Arthroscopy: A Comprehensive Approach to Complex Knuckle Injuries.

Isolated management of metacarpophalangeal (MCP) joint injuries or sagittal band disruptions often leads to incomplete symptom resolution and suboptimal functional outcomes, particularly in complex trauma. We present a combined surgical technique integrating MCP joint arthroscopy with open sagittal hood repair for traumatic extensor mechanism instability and intra-articular pathology. This paper describes a stepwise approach to address both intra-articular and soft tissue components of MCP joint injuries. Arthroscopy facilitates direct visualization, synovectomy, debridement, and capsular shri

Changes in the mechanical properties of myotonometric muscles following manual therapy during archery: a single-group, single-session repeated measurement study.

In precision sports like archery, muscle control, fatigue reduction, and efficient movement execution are essential to an athlete’s performance. This study aimed to (i) analyze variations in muscle tone, stiffness, elasticity, creep, and relaxation at baseline, after archery shooting, and at a final post-session time point obtained after a standardized 30-min rest period followed by a manual-therapy session, and (ii) examine the relationship between acute fatigue after 100 shots and the magnitude of subsequent post-session change scores (not treatment effects). Twenty experienced archery athle

A Bear of a Case: Multidisciplinary Reconstruction Following a Grizzly Bear Mauling.

Grizzly bear attacks are rare but often result in devastating craniofacial injuries requiring staged, multidisciplinary reconstruction. A 61-year-old male sustained extensive facial trauma, including complete lower lip avulsion and anterior mandibular loss, after a grizzly bear attack. He underwent early stabilization followed by virtual surgical planning-guided fibula free flap reconstruction with immediate dental implants, and a radial forearm free flap incorporating palmaris tendon to restore oral competence. The patient's postoperative course was uncomplicated and he was decannulated and d
