BACKGROUND AND STUDY AIMS: Elastic stable intramedullary nailing (ESIN) enables stable osteosynthesis with a low rate of complications in pediatric both-bone forearm fractures. However, repeated unsuccessful attempts at closed reduction can cause unnecessary soft tissue trauma and increased radiation exposure. We conducted a retrospective study of pediatric forearm fracture patients to identify predictors for the need for open reduction when using ESIN.
PATIENTS AND METHODS: We included 65 pediatric patients who underwent ESIN osteosynthesis for both-bone forearm fractures. We analyzed gender

Extensor hallucis longus (EHL) tendon rupture is an uncommon injury that usually results from trauma and often requires surgical intervention. In the subacute phase, primary tendon repair becomes challenging because of tendon retraction, and tendon transfer or graft reconstruction is often necessary. Anatomical variations of the EHL, particularly the extensor hallucis capsularis (EHC), may influence tendon retraction and reparability, although their clinical significance has rarely been reported. We present a rare case of subacute EHL rupture in which primary end-to-end repair was feasible owi

Pediatric forearm fractures, particularly involving the ulna and radius, are among the most common childhood injuries. However, the lack of standardized and openly available datasets has limited progress in artificial intelligence research and constrained clinical validation. To address this issue, we present the Pediatric Ulna and Radius Fractures (PediURF) dataset, a first-of-its-kind, publicly available collection of over 10,000 de-identified images. Each image is carefully annotated by expert radiologists and categorized into three clinically relevant types: proximal, midshaft, and distal

INTRODUCTION: Impalement injuries of the upper limb represent a rare and striking form of penetrating trauma, often associated with significant risk of neurovascular and tendinous damage due to the compact anatomical arrangement of the forearm. Despite their alarming external appearance, actual morbidity is determined by the trajectory relative to critical structures. Effective management requires careful pre-hospital stabilization, thorough pre-operative assessment, appropriate imaging, and meticulous surgical extraction under direct visualization to minimize complications.

Extension-type supracondylar humerus (SCH) fractures are among the most common pediatric elbow fractures. Treatment algorithms for Gartland type I and type III fractures have been well established, yet managing type II SCH fractures has remained a topic of debate in the pediatric orthopaedic community. It is important to explore the controversies surrounding both the classification and treatment of type II SCH fractures. Radiographically, these fractures are difficult to reliably classify to guide decision making. In addition, the decision to pursue closed reduction and casting versus closed (

Deep tissue and bloodstream yeast infections are associated with poor outcomes due to rapid progression, antifungal resistance, and immune evasion. Differentiating between wound contamination and true fungal infection remains a clinical challenge, with limited guidance on the use of prophylactic antifungal therapy in this setting. This article presents a rare case of an immunocompetent patient who sustained a degloving hand injury and radius fracture from a bread kneading machine, with subsequent deep tissue cultures growing Saccharomyces cerevisiae. The case underscores the potential risk of

Supracondylar humerus fractures are the most common elbow fracture requiring surgical intervention in the pediatric population; therefore, any orthopaedic surgeon who treats pediatric orthopaedic trauma patients must be well versed in the appropriate evaluation and treatment of supracondylar fractures. It is important to focus on a subset of pediatric supracondylar humerus fractures in which the patient has an associated loss of a palpable radial pulse, termed the pulseless supracondylar. Essential considerations for these injuries include the initial evaluation and care in the emergency depar

Paediatric distal forearm fractures are a common injury in children and adolescents, often requiring surgical intervention. Traditional techniques involve forceful manipulation and hyper-exaggerating the defect to overcome haematoma resistance, increasing the risk of complications including iatrogenic displacement, neurovascular injury, and growth plate damage. This technical note describes an alternative intra-operative reduction technique aimed at addressing these limitations. A minimally invasive approach with a small incision allows for precise identification and gentle evacuation of the h

BACKGROUND: Diagnosing simple valgus instability of the elbow currently involves time- and cost-intensive imaging modalities such as magnetic resonance imaging or magnetic resonance arthrography. Previous studies have demonstrated that stress radiography represents an alternative diagnostic tool for such conditions. The aim of this study was to investigate whether standardized valgus stress radiography can identify soft-tissue lesions of the medial elbow.
METHODS: A telos stress device (telos GAIII/E; telos Arzt- und Krankenhausbedarf GmbH, Woelfersheim-Bernstadt, Germany) was used to apply 5

BACKGROUND: Tension band wiring (TBW) is a widely used technique for displaced olecranon fractures but carries a substantial risk of complications, often due to technical errors. Despite being a designated core competency in orthopedic curricula, no standardized, procedure-specific tool exists to assess technical performance in TBW. This study aimed to establish international expert consensus on assessment parameters for evaluating surgical competence in TBW of simple transverse olecranon fractures.
METHODS: A modified 4-round Delphi process was conducted with 98 orthopedic trauma surgeons, p

The aging process is associated with many functional decrements. It is important to consider the additive acute functional decreases that may occur following eccentric exercise-induced muscle damage (EIMD). This study aimed to examine the effect of age on muscle damage metrics following EIMD. Ten younger (age=20±2 years, BMI=22.49±3.97 kg/m2) and six older (age=68±2 years, BMI=24.45±2.82 kg/m2) females participated in this study. Participants performed a maximal knee extensor EIMD protocol. Additionally, participants completed various assessments before, immediately after, 24-, 48-, and 72-hou

OBJECTIVES: During transradial percutaneous intervention (PCI), sharp edge of guide catheter tip may produce "razor blade effect" leading to radial vascular injury. Pigtail assisted tracking (PAT) technique has been described to overcome it, but has not been systematically evaluated. We prospectively evaluated the safety and efficacy of PAT technique in transradial PCI.
METHODS: Consecutive patients undergoing transradial-PCI were randomized into two groups (Group 1: transradial navigation using PAT technique, Group 2: transradial navigation without PAT technique). Patients were observed for

BACKGROUND: The knee arthroplasty terrible triad (TT) is a combination of periprosthetic joint infection, extensor mechanism insufficiency, and soft tissue defect. The TT is a limb-threatening condition that has historically poor functional results and high probability of amputation or fusion. The rotational gastrocnemius-Achilles rotational flap (GARF) holds promise as native soft tissue to provide coverage and muscular power to assist with knee extension. We hypothesized that application of the GARF in conjunction with two-stage revision improves functional and limb-salvage outcomes compared

BACKGROUND: Prognostic indicators of lack of strength progression or poor self-reported function may alter rehabilitation decision-making following anterior cruciate ligament reconstruction (ACLR). The torque-velocity relationship is a noninvasive measure of muscle function that is altered following ACLR, but its prognostic value has not been explored.
PURPOSE: To compare the torque-velocity relationship of knee extensors (quadriceps) and flexors (hamstrings) between the ACLR and uninvolved limbs and to determine whether the torque-velocity relationship was prognostic of subsequent achievemen
