Evaluation of Injuries Caused by Sharp Objects in Different Suicide Cases.

BACKGROUND: Suicide, defined as the act of taking one's own life, is a common cause of death worldwide, with more than 720,000 people dying by suicide each year. However, suicide by sharp objects is relatively rare, accounting for only a small percentage of all cases. In some cases, self-inflicted injuries may be mistaken for other causes, such as assault or accidental trauma. The mortality rate from self-inflicted injuries with sharp objects is low, and most such suicide attempts do not lead to death. The low fatality rate associated with self-inflicted sharp force injuries may prompt the use

Severe Spaghetti Wrist Injury With Complete Laceration of Median Nerve, Radial and Ulnar Arteries, and Multiple Flexor Tendons: A Case Report and Literature Review.

Spaghetti wrist injuries are complex distal forearm lacerations involving flexor tendons, nerves, and vessels, associated with significant functional morbidity. We present the case of a 64-year-old right-handed man with a deep cut wound to his left forearm and clinical signs of distal ischemia, complete loss of digital flexion, and palmar anesthesia. An urgent examination was performed, identifying sections of the radial and ulnar arteries, median nerve, and injuries to the palmaris longus, flexor carpi ulnaris, flexor carpi radialis, superficial and deep flexor tendons of the fingers, and fle

The use of external fixation in the treatment of nonunion and malunion of the upper extremity.

BACKGROUND: Nonunion and malunion of upper extremity fractures are uncommon but challenging complications, particularly when associated with infection, deformity, or multiple previous operations. Although internal fixation remains the standard treatment, external fixation based on Ilizarov principles offers advantages in complex situations including infection control, deformity correction, and gradual bone reconstruction. This study evaluates the indications, surgical technique, and outcomes of external fixation as a definitive treatment for nonunion and malunion of the upper extremity. METHO

Self-inflicted Thoraco-abdominal stabbing followed by bathtub drowning: Reconstruction of the terminal mechanism in a complex suicide.

Complex suicide involving sharp-force injury and drowning may closely mimic homicide and requires careful reconstruction of the terminal mechanism. We report an anonymized case of a 27-year-old man found prone in a bathtub containing water mixed with blood after emergency entry into his apartment. Blood traces were documented in several rooms. Autopsy identified 18 stab-incised wounds of the precordial and epigastric regions, hesitation-type superficial injuries, old linear forearm scars, and no defensive injuries. Seven wounds penetrated the thoracic cavity; however, the limited right ventric

The lacertus fibrosus substantially Alters flexion and supination moment arms: A cadaveric study.

This study evaluated the effect of the lacertus fibrosus on the biceps brachii moment arms to determine the lacertus importance in forearm strength. The work proceeded from the null hypothesis that the lacertus fibrosus has no effect on flexion/extension and supination/pronation. Ten fresh-frozen cadaveric specimens were mounted in an elbow simulator to measure tendon excursion, elbow flexion/extension (FE) and forearm pronation/supination (PS) angles. FE moment arms were calculated throughout FE excursion at multiple, constant PS angles and PS moment arms were measured throughout PS excursion

Hydrofluoric Acid, an Unexpected Surprise.

We report the case of a 37-year-old man presenting with pain out of proportion to the exam with hydrofluoric acid burns to his upper extremities after he spilled a wheel-stripping compound on his forearms while working at his powder coating business. His burns initially appeared mild and superficial, but over the course of several days, these evolved from simple erythema to significant partial thickness tissue destruction and ulceration. He required substantial topical, intradermal, and intravenous therapies to control the unseen burning process during his index visit to the emergency departme

[Ultrasound-guided peripheral venepuncture under poor venous conditions].

Placement of a peripheral indwelling venous catheter is a routinely performed invasive procedure, in which complications are often underestimated. In difficult venous conditions multiple puncture attempts are often required, which are time consuming, unnecessarily painful for the patients and nevertheless not always successful. Due to the close anatomical relationship between superficial veins and peripheral nerves in the arm, puncture-related nerve injury is not uncommon. Despite limited data it could be shown that ultrasound-guided peripheral venepunctures are superior to traditional landmar

Systematic review of nerves at risk at the wrist in common surgical approaches to the forearm: Anatomical variations and surgical implications.

Three commonly used approaches to the forearm in orthopedic surgery are Henry's, Thompson's, and the ulnar approach, each of which has the potential to cause injury to nerves around the wrist. Preserving these nerves is important to prevent complications such as neuroma formation and motor and sensory changes to the hand. We conducted a review of the literature to assess the nerves at risk and whether 'safe zones' exist to avoid these nerves. An independent reviewer conducted searches in Embase and MEDLINE of the literature from 2010 to 2020. A total of 68 papers were identified, with 18 artic

Pediatric trauma burden in Tanzania: analysis of prospective registry data from thirteen health facilities.

BACKGROUND: Trauma is among the leading causes of morbidity and mortality among pediatric and adolescent populations worldwide, with over ninety percent of childhood injuries occurring in low-income and middle-income countries. Lack of region-specific data on pediatric injuries is among the major challenges limiting the ability of health systems to implement interventions to prevent injuries and improve outcomes. We aim to characterize the burden of pediatric health injuries, initial healthcare interventions and outcomes seen in thirteen diverse healthcare facilities in Tanzania. METHODS: Thi

Anatomical Study of the Innervation of Triangular Fibrocartilage Complex and Distal Radioulnar and Radiocarpal Joints: Implications for Denervation.

PURPOSE: Open and percutaneous denervation is an emerging technique for joint pain. This study investigated the course and distribution of the articular branches innervating the triangular fibrocartilage complex (TFCC), distal radioulnar joint (DRUJ), and radiocarpal joint (RCJ) relative to bony and soft tissue landmarks to guide wrist denervation procedures. METHODS: Fourteen formalin-embalmed specimens were serially dissected to expose the origin, course, and distribution of articular branches innervating the TFCC, DRUJ, and RCJ. Bony and soft tissue landmarks to localize each articular bra

Anterolateral thigh free flaps with T-shaped pedicles and multiple venous anastomosis for extremity reconstruction.

The anterolateral thigh free flap is one of the most preferred options for reconstructing soft tissues of the extremities and vascular anastomosis is one of the most important factors for flaps survival. T-anastomosis and double venous anastomosis have been widely used for increasing flap survival. This report shows both application of T-shape pedicle and multiple venous anastomosis to each 43 cases for extremity reconstruction that have not been described so far in the literature and it showed the necessity of multiple anastomosis. The locations of the lesions were 8 upper extremities (4 hand

Acute distal biceps tendon repair using cortical button fixation results in excellent short- and long-term outcomes : a single-centre experience of 102 patients.

AIMS: Acute distal biceps tendon repair reduces fatigue-related pain and minimizes loss of supination of the forearm and strength of flexion of the elbow. We report the short- and long-term outcome following repair using fixation with a cortical button techqniue. METHODS: Between October 2010 and July 2018, 102 patients with a mean age of 43 years (19 to 67), including 101 males, underwent distal biceps tendon repair less than six weeks after the injury, using cortical button fixation. The primary short-term outcome measure was the rate of complications. The primary long-term outcome measure

Current indications for abdominal-based flaps in hand and forearm reconstruction.

Extensive soft tissue loss or injury of the hand and upper extremity is a challenging reconstructive problem traditionally treated with abdominal-based pedicled flaps. Options for coverage included the groin flap based on the superficial circumflex iliac artery, the Scarpa's fascia flap based on the superficial inferior epigastric artery, and the paraumbilical perforator flap from the deep inferior epigastric artery perforators. Despite the ability to provide consistent and pliable soft tissue coverage with ease of elevation, these flaps have several disadvantages including restriction of mobi

Preoperative Computer Simulation and Patient-specific Guides are Safe and Effective to Correct Forearm Deformity in Children.

BACKGROUND: Posttraumatic and congenital forearm deformities in children can be difficult to appreciate in all planes. In cases of distal radioulnar joint instability and loss of forearm rotation, surgical correction is challenging. Advances in 3-dimensional printing allow creation of custom guides at a reasonable cost, enabling precise correction of the deformity in all planes. METHODS: Nineteen children with deformity of the forearm had corrective osteotomies performed using preoperative 3-dimensional computer modeling and patient-specific surgical guides. Surgicase software was used for 3-

Extended Posterior Interosseous Artery Flap: Anatomical and Clinical Study.

PURPOSE: A reverse-flow pedicled flap from the posterior interosseous artery (PIA) has been used to cover defects on the dorsal and volar aspects of the hand. However, the original description of this flap does not reach further than the metacarpophalangeal joints of the 4 ulnar digits. In the present study, we describe a distal variant (type 2) of the PIA flap, which changes the pivot point of the classic variant (type 1) and which can provide full coverage of single or multiple digits in the entire dorsum and palmar surface of the fingers. METHODS: An anatomical study was performed on 26 ca

Fluorescent Imaging Evaluation of Lidocaine Distribution Following Bier Block in the Upper Extremity.

INTRODUCTION: Bier block regional anesthesia has been used as an alternative to general anesthesia for years. Despite multiple theories and multiple techniques to delineate the location of the action of lidocaine during Bier block anesthesia, there has not been a consensus on the location of action. The purpose of this study was to use fluorescent imaging to further investigate the site of action of lidocaine during Bier block. MATERIALS AND METHODS: Three patients with carpal tunnel syndrome underwent open carpal tunnel release with Bier block anesthesia performed in the standard fashion wit

Idiopathic Guttate Hypomelanosis: A Review of its Etiology, Pathogenesis, Findings, and Treatments.

Idiopathic guttate hypomelanosis is a common acquired leukoderma characterized by multiple, discrete round or oval, porcelain-white macules on sun-exposed areas, especially on the extensor surface of forearms and pretibial areas. It usually affects individuals aged over 40 years and the likelihood of acquiring it increases with age. The exact pathogenesis remains controversial. However, there are several factors that are believed to be involved such as aging, ultraviolet exposure, trauma, genetic factors, autoimmunity, and local inhibition of melanogenesis. Despite the benign course of progres
