Clinical validation of the DRoPH score: a preoperative tool for estimating reduction difficulty in proximal humerus fractures.

BACKGROUND: Proximal humerus fractures are common injuries that are frequently associated with considerable technical challenges during surgical reduction and fixation. Although anatomical reduction is crucial for fracture healing and functional recovery, the anticipated difficulty of achieving satisfactory reduction is difficult to estimate preoperatively. The Difficulty of Reduction of Proximal Humerus Fractures (DRoPH) score was recently developed through a structured expert consensus process to estimate anticipated reduction difficulty based on predefined fracture- and patient-related char

Donor site morbidity and patient satisfaction after arthroscopic superior capsule reconstruction using fascia lata autograft for irreparable rotator cuff tears.

BACKGROUND: Donor site morbidity-previously a major concern-remains a considerable issue in arthroscopic superior capsule reconstruction using a fascia lata autograft due to the limited scientific evidence. We aimed to evaluate donor site morbidity and patient satisfaction after arthroscopic superior capsule reconstruction using a fascia lata autograft for irreparable rotator cuff tears. We hypothesized that the procedure would result in certain donor site morbidities, would not have a significant impact on daily activities, and that improvements in shoulder function could compensate for donor

Chronic Neuropathy of the Lateral Antebrachial Cutaneous Nerve after Endoscopic Radial Artery Harvest: A Case Report.

INTRODUCTION: Neurologic complications after radial artery harvest for coronary artery bypass grafting (CABG) remain incompletely characterized, particularly after endoscopic radial artery harvest (ERAH). As the radial artery has become the preferred second arterial conduit due to its superior long-term outcomes, ERAH has gained popularity as a minimally invasive alternative to open harvest with favorable cosmetic and wound outcomes. Although prior studies suggest neurologic symptoms are often transient, persistent sensory deficits may be underrecognized. The lateral antebrachial cutaneous ner

Open Posterolateral Elbow Dislocation in an Adult after Low-Energy Trauma: A Case Report Highlighting Diagnostic and Operative Considerations.

INTRODUCTION: Open elbow dislocations are rare injuries in adults, accounting for <2% of cases, and may occur even after low-energy mechanisms. Their presentation can be subtle, particularly when periarticular wounds appear benign, leading to delayed recognition of joint violation. Early identification of intra-articular air and associated soft-tissue injury is essential to guide timely operative management. This case highlights the diagnostic challenges and surgical considerations associated with an uncommon open posterolateral elbow dislocation following a low-energy fall. CASE REPORT: A mi

Open reduction and internal fixation of Type I Monteggia Equivalent fractures: A case series with excellent functional outcomes.

INTRODUCTION AND IMPORTANCE: Monteggia equivalent fractures, a spectrum of proximal ulnar and radial injuries extending beyond classic Monteggia patterns, are rare and diagnostically challenging, particularly in adults. Delayed presentation further complicates management and increases the risk of long-term morbidity. This report details a case series of two adult patients with Type I Monteggia Equivalent fractures, one with a delayed presentation, successfully treated with open reduction and internal fixation (ORIF) of the radial fracture. We aim to elucidate effective diagnostic and treatment

Chronic Overuse Injuries in Young Athletes-Upper Extremity.

Young athletes are uniquely susceptible to overuse injuries of the upper extremity because their open physes and apophyses are structurally weaker than adjacent bone, ligament, and tendon. Repetitive sport-specific loading-torsional stress in throwers, axial compression in gymnasts and climbers-can outpace the repair capacity of growing tissues, producing a spectrum of physeal and apophyseal stress injuries from widening and sclerosis to frank epiphysiolysis. This review examines the mechanisms, clinical presentation, and imaging findings of chronic overuse injuries at key upper extremity site

Type II Supracondylar Humerus Fractures: To Fix or Not To Fix?

Extension-type supracondylar humerus (SCH) fractures are among the most common pediatric elbow fractures. Treatment algorithms for Gartland type I and type III fractures have been well established, yet managing type II SCH fractures has remained a topic of debate in the pediatric orthopaedic community. It is important to explore the controversies surrounding both the classification and treatment of type II SCH fractures. Radiographically, these fractures are difficult to reliably classify to guide decision making. In addition, the decision to pursue closed reduction and casting versus closed (

The Pulseless Supracondylar Humerus Fracture: To Explore or Not.

Supracondylar humerus fractures are the most common elbow fracture requiring surgical intervention in the pediatric population; therefore, any orthopaedic surgeon who treats pediatric orthopaedic trauma patients must be well versed in the appropriate evaluation and treatment of supracondylar fractures. It is important to focus on a subset of pediatric supracondylar humerus fractures in which the patient has an associated loss of a palpable radial pulse, termed the pulseless supracondylar. Essential considerations for these injuries include the initial evaluation and care in the emergency depar

Primary Stability of a New TendoClip Fixation for Tuberosity Refixation in Total Shoulder Arthroplasty.

Sufficient refixation and healing is crucial for the function of a reverse shoulder arthroplasty within the treatment of proximal humeral fractures. Conventional refixation with suture cerclage is time-consuming and leads to healing in only 60-70% of cases. This study used a biomechanical test to investigate two different refixation procedures for treating 4-fragment fractures of the proximal humerus. Eight paired humerus specimens were randomly assigned to two groups and treated with reverse shoulder prostheses. One group received conventional suture cerclage fixation for 4-fragment fractures

A Novel Methodology for Establishing Best Values for MCID, SCB, and PASS Thresholds for Rotator Cuff Repair.

BACKGROUND: The heterogeneity of threshold values for the minimal clinically important difference (MCID), substantial clinical benefit (SCB), and Patient Acceptable Symptom State (PASS) as reported in the rotator cuff repair literature undermines the utility of these concepts. This systematic review identifies studies with published threshold values and proposes a methodologic framework for determining which values should be used for rotator cuff repair moving forward. PURPOSE: To provide recommendations for the MCID, SCB, and PASS thresholds of commonly utilized patient-reported outcome meas

Structural Augmentation in Rotator Cuff Repair Decreases the Risk of Retear: A Systematic Review and Meta-analysis.

BACKGROUND: Rotator cuff tears are a common orthopaedic injury often resulting in shoulder pain and functional impairment. Despite the prevalence of rotator cuff tears, the incidence of retear after rotator cuff repair (RCR) remains high. There is growing interest in the use of structural augmentation in RCR to decrease retear rates and improve patient outcomes. PURPOSE: The purpose of this systematic review and meta-analysis was to compare the clinical and imaging outcomes of structural augmentation versus standard RCR in patients with full-thickness rotator cuff tears. The effectiveness of

Partial Tear of the Common Flexor Tendon and Medial Ulnar Collateral Ligament in a Six-Year-Old After a Minor Traumatic Injury: A Case Report.

Acute traumatic medial ulnar collateral ligament (MUCL) injury, associated with a high-grade sprain of the common flexor tendon and a low-grade sprain of the annular ligament, is an uncommon occurrence in the non-athletic pediatric demographic. At present, there is a lack of high-quality, evidence-based research that details comparable injuries. Furthermore, to our knowledge, there are no established guidelines that provide direction for the optimal treatment of this specific injury. We report a case involving a six-year-old boy who suffered an acute traumatic MUCL injury to his right elbow. T
