Outcomes of intra-articular distal humerus open reduction and internal fixation based on severity of articular comminution.

BACKGROUND: The advent of modern, precontoured locking plates has improved treatment of distal humerus fractures (DHFs) by providing an anatomic fit and a reliable locking mechanism with interdigitating screws. However, severe articular comminution may predispose surgical failure despite anatomic reduction and a biomechanically strong construct. METHODS: A retrospective review was performed to identify patients who underwent distal humerus open reduction internal fixation (ORIF) between 2015 and 2020. Fractures were classified using the AO/OTA classification and further categorized by anatomi

Bouldering-related trauma: Injury patterns and operative burden over 10 years at a UK major trauma centre.

INTRODUCTION: Bouldering is a rapidly growing discipline within climbing, characterised by short ascents performed without ropes, typically at heights of up to several metres above protective crash mats. While overall injury burden among climbers has been described in the literature, detailed analyses of severe orthopaedic trauma presenting to major trauma centres remain limited. As participation continues to increase, particularly within indoor climbing facilities, a clearer understanding of injury patterns associated with bouldering is needed. This study aimed to characterise the anatomical

Medial Epicondyle Fractures Treated with Diverse Fixation Techniques: A Case Series.

INTRODUCTION: Medial epicondyle fractures of the humerus, account for ~12-20% of pediatric elbow fractures and are often associated with elbow dislocations. Management is debated, especially for moderately displaced injuries. Conventionally, displaced fractures were sometimes treated non-operatively, yielding acceptable function but high rates of fibrous non-union. Recent evidence shows surgical fixation provides ~9-fold higher union rates without significant differences in long-term pain or nerve function. We present five patients treated with different fixation techniques to highlight manage

Forearm Compartment Syndrome Following a Thermal-Crush Injury Treated With a Dorsal Ulnar Artery Flap: A Case Report.

Compartment syndrome (CS) is a condition that requires immediate attention; if diagnosis is delayed and treatment is not provided promptly, it can lead to complications resulting in permanent functional disability or limb loss. As the name suggests, once a muscle compartment is affected by an injury, this syndrome may develop, manifesting as progressive inflammation and edema, leading to reduced blood flow to the limb. Diagnosis is primarily clinical, based on signs of impaired perfusion. Fasciotomy is the treatment of choice, helping to prevent amputations when performed in a timely manner. H

Suture Bridge Fixation for Ulnar Sublime Tubercle Avulsion Fractures in High School Baseball Players: A Report of Two Cases.

Ulnar sublime tubercle avulsion fractures caused by repetitive valgus stress are rare injuries predominantly observed in adolescent overhead throwing athletes. Although conservative management is typically the first-line treatment, surgical intervention may be required in cases of nonunion, displaced fragments, or persistent medial elbow pain. We report two high school baseball pitchers with symptomatic ulnar sublime tubercle avulsion fractures that were unresponsive to conservative treatment and were surgically treated using a suture bridge fixation technique. In both cases, the small, avulse

Open Reduction and Internal Fixation of a Distal Humerus Fracture Through an Elbow Pseudoarticulation After Untreated Pediatric Dislocation: A Case Report.

UNLABELLED: An 89-year-old man presented with right elbow pain after a low-energy fall. At age 2 years, he sustained an untreated distal humeral fracture dislocation, which resulted in progressive remodeling and formation of a functional pseudoarticulation. Despite permanent deformity, he maintained stable functional motion for decades and performed heavy manual labor. Imaging revealed a displaced distal humeral fracture through the chronically remodeled elbow. Open reduction and internal fixation with a variable-angle locking plate was performed, while the pseudoarticulation was intentionally

High-velocity gunshot-related humeral fractures managed with circular external fixation in a resource-limited setting, Samburu County, Kenya: Surgical technique highlights from a case series.

INTRODUCTION: High-velocity gunshot-related humeral fractures are complex injuries often associated with a high rate of nonunion, infection, and poor functional outcome. In low- and middle-income countries these injuries are further complicated by delayed presentation and implant unavailability. Staged management with spanning external fixation followed by internal fixation with intramedullary nail or plate has been well described in the literature; however, the use of a circular frame as a definitive treatment has been very seldom mentioned. MATERIALS AND METHODS: Six high-velocity gunshot-r

Pre-operative soft tissue injuries as independent predictors of elbow stiffness after radial head fracture fixation: a pilot study.

BACKGROUND: Post-operative elbow stiffness is a common and debilitating complication following open reduction and internal fixation for displaced radial head fractures. The relative contributions of post-operative immobilization and intrinsic soft tissue damage remain debated. The purpose of this pilot study was to generate preliminary data on this relationship to inform the design of future, definitive research. METHODS: This study retrospectively reviewed 45 patients who underwent open reduction and internal fixation for displaced radial head fractures. Pre-operative magnetic resonance imag

Does the operative technique (MIPO vs. ORIF) influence surgical outcomes and complications in proximal humerus fractures? A matched-pair analysis.

INTRODUCTION: Proximal humerus fractures account for approximately 4-6% of all fractures and are among the most common fractures in the elderly. Surgical treatment is increasingly applied for displaced and unstable fracture patterns. The two most commonly used techniques are open reduction and internal fixation (ORIF) via a deltopectoral approach and minimally invasive plate osteosynthesis (MIPO) using a deltoid-split or anterolateral approach. ORIF allows direct fracture visualization and anatomical reduction but is associated with more extensive surgical exposure. MIPO aims to preserve soft

Which Technique is the Optimal Option for the Treatment of Displaced Far-Lateral (Cho Type IIC) Distal Clavicle Fractures? A Comparative Analysis of Coracoclavicular Fixation Versus Anatomical Locking Plate Fixation.

BACKGROUND: As achieving stable fixation in unstable far-lateral distal clavicle fractures is challenging, the optimal surgical treatment remains controversial. The aim of this study was to compare the clinical and radiological results of coracoclavicular fixation (CCF) versus anatomical locking plate fixation (ALPF) in patients with Cho type IIC fractures. METHODS: Thirty-eight patients who underwent surgical treatment for Cho type IIC fracture were included. Open CCF using a suture button device was performed in 25 patients and ALPF was performed in 13 patients. The mean follow-up period wa

Manipulation under anesthesia for posttraumatic elbow stiffness: a propensity score matched retrospective cohort study.

Post-traumatic elbow stiffness is a common complication limiting range of motion and function. Open surgical release is invasive and may not improve patient outcomes. Manipulation under anesthesia (MUA) is a less invasive way to break adhesions and restore motion, though high-level comparative evidence is lacking. This study evaluated whether early MUA plus rehabilitation improves elbow motion, pain, and function compared to rehabilitation alone in patients with postoperative elbow stiffness. This retrospective propensity score-matched cohort study included 240 post-ORIF elbow patients (120 re

Does Combined Intravenous and Topical Tranexamic Acid Offer Additional Benefit Compared With Intravenous or Topical Alone in Open Elbow Arthrolysis-A Pilot Randomized Controlled Trial.

PURPOSES: Open elbow arthrolysis (OEA) is a common method for treating post-traumatic elbow stiffness, which may lead to severe bleeding when resecting heterotopic ossification (HO) and fibrous scar tissue. The purpose of this study is to compare the efficacy and safety among three different ways of using tranexamic acid (TXA) in reducing total blood loss and drainage volume in OEA. PATIENTS AND METHODS: This was a pilot, double-blinded, randomized controlled trial involving a total of 60 patients of post-traumatic elbow stiffness. Patients in the IV group (n = 20) received TXA intravenously,

Imaging of pediatric musculoskeletal trauma: Age-specific considerations and challenges.

Pediatric musculoskeletal trauma differs significantly from adult injuries due to the dynamic and evolving nature of the growing skeleton. The presence of open growth plates (physes), secondary ossification centers, and a more elastic bone structure contributes to unique injury patterns and imaging findings in children. Proper interpretation of pediatric imaging requires an in-depth understanding of age-specific anatomy, biomechanics, and normal developmental variants. This review provides a comprehensive overview of pediatric bone physiology and fracture types, including the Salter-Harris cla

Defining patient acceptable symptom state (PASS) following reverse total shoulder arthroplasty to treat proximal humeral fractures using the Advita Ortho database: a retrospective cohort study.

BACKGROUND: Proximal humeral fractures (PHFs) are common in the elderly population, and the use of reverse total shoulder arthroplasty (rTSA) as treatment has risen substantially in recent years. As opposed to statistical significance, clinical value has increasingly been used to evaluate outcomes; thus, this study aims to define patient acceptable symptom state (PASS) thresholds, Minimal Clinically Important Difference (MCID), and substantial clinical benefit (SCB) values for American Shoulder and Elbow Surgeons Standardized Shoulder Assessment Form (ASES) score, Constant-Murley score (CMS),

Revision Open Reduction and Internal Fixation with Autologous Iliac Crest Bone Grafting for Atrophic Olecranon Nonunion in a Middle-aged Woman.

Olecranon nonunion is an uncommon complication of proximal ulna fractures that can cause pain, deformity, and functional limitation. It usually arises from inadequate fixation, comminution, or impaired healing biology. Timely surgical intervention is often required to restore elbow function. A 34-year-old female, right-hand dominant homemaker presented with persistent deformity, weakness in extension, and difficulty performing overhead activities 4 months after a conservatively managed right elbow injury. Clinical examination revealed a palpable olecranon defect, and radiographs confirmed an a

Postoperative Stiffness After Upper Limb Surgery: Prevention and Management.

The upper limb experiences postoperative stiffness as a frequent disabling complication that restricts range of motion, extends recovery time for work and sports activities and diminishes patient contentment. The condition develops from an intensified wound-healing response which primarily involves inflammation and fibrosis. The clinical presentation of joint stiffness manifests as frozen shoulder (adhesive capsulitis) with capsular thickening and rotator-interval/coracohumeral ligament contracture but elbow contracture includes both capsular fibrosis and heterotopic ossification (HO). The pre
