[Problem of fracture of the radius head in the child].

Guidelines for the choice of conservative or operative treatment of fractures of the proximal radius are derived from a study comprising 21 children. Postoperative complications and long-term failures are specifically stressed. True intraarticular fractures of the radial head are rarely observed in children. Major problems arise in the treatment of comminuted fractures of the radial head associated with dislocation of the elbow, since these show the worst prognosis. Displaced marginal fractures below 3 mm are suited for early functional treatment using conservative procedures. 18 patients suff

Monteggia fracture-dislocations in children.

Thirty-three Monteggia fracture-dislocations occurring in patients aged 2 to 15 years were reviewed. A follow-up of 2 to 7 years in 25 patients revealed that 88% had good to excellent results and 12% had results which were fair or poor. Closed reduction was successful in 24 of 28 cases and appeared to be very effective. Open reduction was required only for older children or when treatment was begun late. A mild hyperextension deformity at the elbow noted on follow-up of patients with anterior dislocation of the radial head seemed to support the theory that the injury is caused by hyperextensio

Fractures of the capitellum.

Forty intact cadaver elbows were studied to determine the contribution of the capitellum to elbow stability. With the elbow at 10 degrees of flexion, valgus motion of the elbow after capitellum excision demonstrated a minimal increase. Although some increase in valgus motion did occur after capitellum excision and radial head resection it was not until the ulnar collateral ligament was released that a severe valgus deformity was produced. In addition, isolated capitellum excisions occurring with release of the medial collateral ligament produced severe valgus motion, demonstrating the importan

Lateral humeral condylar fractures in children.

Fifty-three patients with 56 fractures of the lateral humeral condyle were reviewed and the results were assessed to determine the amount of displacement that could be relied on to produce satisfactory results. Closed treatment resulted in satisfactory results if the initial displacement did not exceed 2 mm. This required close follow-up for detection of displacement. Percutaneous pinning of nondisplaced and minimally displaced fractures is an acceptable alternative in any situation in which close scrutiny cannot be ensured. Those fractures with greater than 2 mm of displacement should be redu

[Indications for the surgical and conservative treatment of juvenile supracondylar humeral fractures and their results].

In the period from 1977 to 1982 78 children were treated for supracondylar fractures of the humerus at the Berufsgenossenschaftliche Unfallklinik Frankfurt/Main. Indications and methods for operative and conservative treatment are described. The results of therapy were reviewed by means of a questionnaire or reexamination in 68 patients with regard to function, elbow deformity and nerve lesions. Fractures of a grade I severity are to be treated conservatively by immobilisation in a POP-cast. Only in exceptional cases of grade II severity can a closed reduction be recommended. All remaining typ

Varus deformity following lateral humeral condylar fractures in children.

Lateral humeral condylar fractures are quite common in children. This article shows that varus deformity is a common sequela following such injuries. Twenty-four patients were studied, with a mean follow-up of 4.8 years. A loss of carrying angle, seen in 10 cases, occurred with comparable frequency in undisplaced (50%) and displaced (39%) fractures. The deformity was usually mild, averaging 10 degrees in the undisplaced fractures and 9.6 degrees in the displaced fractures treated with open reduction. We conclude that varus deformity is common in this injury, and occurs whether or not the fract

Fracture separation of the olecranon ossification center in adults.

Traumatic disruption of an incompletely fused olecranon physis in adults has been reported on only one occasion by O'Donoghue in 1942, while stress fractures of the olecranon physis have been well-recognized and reported. A series of three adult males with traumatic disruption of an incompletely fused olecranon physis was studied. Two were competing in football at the time of the injury, while a third patient sustained the injury during a fall. All of our patients were former throwing athletes and all sustained a direct blow to the dominant elbow at the time of the fracture. In our study, as w

Bony injuries about the elbow in the throwing athlete.

Tremendous valgus stress during the acceleration phase of pitching is the primary cause of elbow injury in the pitcher. Lateral compression injuries are usually found in pitchers in their early teens, and osteochondritis dissecans is the primary underlying cause. Severe flexion contractures and even partial ankylosis of the elbow may occur. Prognosis after surgery is generally poor. Medial tension injuries with either soft-tissue damage or bony damage usually require surgery. The medial collateral ligament should be considered the primary stabilizer against the valgus-vector overload. The medi

Clinical application of basic research on continuous passive motion for disorders and injuries of synovial joints: a preliminary report of a feasibility study.

Since 1970, when the concept of continuous passive motion (CPM) was originated by one of the authors (R.B.S.), he and a succession of his research fellows have investigated its biological effects on the healing and regeneration of articular tissues in a wide variety of experimental models in rabbits. From this basic research he concluded that CPM is well tolerated, seems to be painless, stimulates the healing and regeneration of articular tissues, prevents joint stiffness, and permits the normal healing of arthrotomy incisions. Beginning in 1975, one of the authors (H.W.H.), and in 1978, the r

[Operative treatment of distal intra-articular humerus fractures; results of 412 follow-up cases (AO-collected statistics)].

Intraarticular fractures require a surgical therapy. The functional results are depending on the anatomical reconstruction of the joint surfaces and on early kinesitherapy. Remaining disabilities may be controlled in 2/3 of the cases to such an extent that no alterations are necessary in private or professional life. The overall result was very good in 23.8%, good in 36.0%, moderate in 22.3% and bad in 17.9%. These bad results are due to the rate of polytraumatized patients (18%) and the high rate of open comminuted fractures (27%).

Percutaneous probe reduction of frontal fractures of the humeral capitellum.

Nine frontal fractures of the humeral capitellum treated by percutaneous probe reduction are reported. Most of the fractures were percutaneously reduced with a Steinmann pin inserted along the lateral border of the biceps tendon. To prevent redisplacement of the fragment, the elbow joint was first flexed more than 90 degrees with plaster immobilization for two weeks. The favorable results obtained with this method are due to reduced surgical trauma to local tissues and good reduction of the fracture. The percutaneous probe reduction should be considered for frontal fracture of the humeral capi

Simultaneous dislocation of the elbow and the proximal radio-ulnar joint.

Simultaneous dislocation of the elbow and the proximal radio-ulnar joint is rare. Two children with this uncommon injury are reported. One child had a transverse divergent dislocation that was treated successfully by closed reduction. The other had a convergent dislocation (or translocation of the radius and ulna) which needed open reduction. Convergent dislocation is even rarer than transverse divergent dislocation, and this is believed to be the first time it has been reported.

Traumatic laceration of the radial nerve following supracondylar fracture of the elbow. A case report.

Traumatic laceration of the median, ulnar, or radial nerve is an extremely rare complication of supracondylar fracture of the humerus in children. A recent review of the literature disclosed only two reported cases of a laceration involving the radial nerve; a third such injury in an 8-year-old boy is reported in the present paper. Traumatic neuropraxia of one or more of the three adjacent peripheral nerves is a more common complication of supracondylar fracture, with the great majority of these lesions responding to conservative treatment. Vascular compromise secondary to traumatic laceration

Management of concomitant ipsilateral fractures of the humerus and forearm.

We treated nineteen patients with concomitant ipsilateral fractures of the humerus and forearm resulting in a so-called floating elbow. The injuries that were treated without open reduction and internal fixation had a high incidence of non-union of the humerus. This has led us to prefer open reduction and internal fixation of both the humerus and the forearm fracture.

Intramedullary fixation of humeral shaft fractures.

In this paper we document seventy fractures of the humeral shaft that were treated by intramedullary fixation between 1970 and 1981. Complications developed in forty-seven (67 per cent) of the fractures, and forty-five (64 per cent) required at least one additional operative procedure. Of the sixty fractures that were internally fixed within six weeks after injury, nine (15 per cent) had a delayed union and five (8.3 per cent) had a non-union, two of which persisted despite subsequent surgery. Three of the ten fractures that were internally fixed more than six weeks after injury never united d

Retrospective analysis of 29 fractures of the olecranon process of the equine ulna.

Twenty-nine fractures of the olecranon process of the ulna in horses treated at The Ohio State University College of Veterinary Medicine from 1973 through 1983 were evaluated. The objectives of the study were to determine the prognosis for various configurations of fractures of the olecranon process of the ulna, to determine what preoperative factors influenced the prognosis, and to evaluate the efficacy of the treatment methods used. The fractures were placed in one of 4 categories on the basis of severity of the injury: type-1 fractures involved the physeal plate; type-2 fractures involved t

Articular cartilage sleeve fracture of the lateral humeral condyle capitellum: a previously undescribed entity.

Salter-Harris type IV fractures of the lateral humeral condyle are well-recognized injuries that have been previously described and classified. The case of an 11-year-old boy who sustained an unusual fracture of the distal humerus with the free fragment composed of 75% of the articular cartilage of the capitellum is presented. The cartilage was attached to only a thin shell of subchondral capitellar bone in continuity with the lateral aspect of the distal humeral physis and metaphysis. Radiographs did not show the magnitude of the articular cartilage component of the fracture, although physica

[Chronic polyarthritis and polytrauma. A case report].

Rheumatoid arthritis already can lead to an extreme limited range of motion of the whole patient in an early age. A female patient of 39 years has been immobilized for a period of about 6 years. By accident she was suffering too from multiple fractures, especially of both femora and both elbow-joints. Rheumatoid arthritis and traumatic lesions lead to incommon surgical procedures. Both hip-, both elbow- and both kneejoints, regarding to the fractures, were replaced during three operation periods. Within half a year the patient was far extending independent of nursing and able to walk again.
