Limb Salvage and Reconstruction of a Complex Upper Extremity Injury: A Case Report.

INTRODUCTION: Complex upper extremity trauma poses us with significant challenges in orthopedic and reconstructive surgery due to the complex anatomy and functional importance of the extremities. These injuries are often the result of high-energy mechanisms like motor vehicle collisions, which makes their management particularly demanding. Making the decision between limb salvage and amputation in such cases requires careful consideration of multiple factors such as functionality, risk of complications, and recovery. CASE REPORT: We describe the case of a 39-year-old male who had sustained an

Utilization of a Skin Graft From an Amputated Hand to Cover Reservent Below-elbow Amputation Wound.

This case involves a 58-year-old man who sustained a severe left forearm injury from a motor vehicle accident. Imaging revealed comminuted fractures and arterial occlusions in the left forearm. As the injury was crush type and exploration revealed vessels with avulsion, the decision was made to perform amputation due to nonsalvageability. The patient underwent irrigation, debridement, and amputation, followed by coverage via a full-thickness skin graft from the amputated hand. Postoperatively, the patient received antibiotics and wound care, resulting in successful graft integration and healin

Impact of Surgery Timing on Outcomes After Nerve Transfer to Restore Elbow Flexion.

BACKGROUND: Nerve reconstruction following brachial plexus injury (BPI) is a time-sensitive procedure, and surgical delay may negatively impact muscle reinnervation and outcomes. This study investigated the impact of surgical timing on elbow flexion strength in patients with BPI undergoing nerve transfer to restore elbow flexion. METHODS: Following PRISMA guidelines, MEDLINE, Embase, and the Cochrane Library databases were systematically searched. English-language studies investigating the single fascicular transfer (SFT) or double fascicular transfer (DFT) to restore elbow flexion in BPI wer

Routine evaluation of supracondylar humerus fracture percutaneous pin sites does not decrease complications: a retrospective comparative cohort.

BACKGROUND: Supracondylar humerus (SCH) fractures are the most common type of pediatric elbow fractures that require surgical intervention. Studies suggest limiting the first postoperative visit to the time of pin removal or limiting radiographs at first postoperative follow-up may not increase complication rates, but few comparative studies exist. This study aims to determine if routinely examining surgical pins sites at the first postoperative visits affects the complication rates or outcomes compared to only evaluating pin sites if a clinical indication exists. METHODS: This cohort study r

Fatigue of the Dynamic Stabilizers of the Medial Elbow in Baseball Pitchers.

BACKGROUND: The flexor carpi ulnaris and flexor digitorum superficialis are thought to provide dynamic stability to the medial elbow during pitching. High medial elbow stress during pitching fatigues these dynamic stabilizers of the medial elbow. This fatigue of dynamic stabilizers could leave the ulnar collateral ligament more vulnerable to the stress of pitching. PURPOSE/HYPOTHESIS: The purpose of this study was to identify the onset of fatigue in the dynamic elbow stabilizers during a pitching performance. It was hypothesized that middle finger (MF) and ring finger (RF) fatigue during a pi

Irreducible Isolated Radial Head Dislocation in a Child Due to Annular Ligament Interposition: A Case Report.

The annular ligament of the elbow is essential for its stability, playing a key role in both the proximal radioulnar and humeroradial joints, as well as supporting surrounding muscles and ligaments. Radial head dislocation is rare in children, and when isolated they can be challenging to reduce and may require surgical intervention. An 11-year-old boy presented with an anteromedial dislocation of the radial head after a fall. Initial, closed reduction was attempted and failed, requiring surgical intervention, where we found a rupture of the annular ligament and interposition, which was repaire

Surgical Management of Humeral Avulsion of the Glenohumeral Ligament Injuries: Indications, Treatment Strategies, and Outcomes.

PURPOSE OF REVIEW: This review aims to synthesize current knowledge on humeral avulsion of the glenohumeral ligament (HAGL) lesions, emphasizing clinical presentation, operative indications, and surgical outcomes. RECENT FINDINGS: HAGL lesions play an important role in shoulder instability, occurring in 7.5%-9.4% of surgically treated cases, with a high propensity to cause recurrent instability if left untreated. The sensitivity of magnetic resonance imaging for detecting HAGL lesions remains imperfect (50%- 83%), making arthroscopy the diagnostic gold-standard. Primary instability is the mos

Posterior approaches for articular distal humeral fractures.

Approaching the elbow joint posteriorly is the most used method to visualize closed fractures of the distal humerus in adults. Treatment can span from open reduction and internal fixation to elbow hemiarthroplasty and total elbow arthroplasty. Different approaches were developed in the necessity of balancing an adequate visualization with minimizing complications and these surgical exposures can be divided into four main categories: paratricipital, triceps reflecting, triceps splitting and olecranon osteotomy. In this article are described the positive and negative features of the principal po
