Unexpected Casualty: The Accidental Fatality of a Glass Injury.

Fatal sharp-cut injuries are often associated with violent acts and are typically considered homicidal in nature. This case report presents a rare instance of an accidental fatality caused by glass, initially suspected as a homicide due to the injury pattern and preliminary crime scene scenario. It describes a 20-year-old male found dead on the roadside with a deep, wide, sharp wound to his right elbow and significant blood loss. An autopsy confirmed hemorrhagic shock due to a major vessel rupture. Further investigation, including CCTV footage, revealed that the intoxicated victim had accident

Influence of Tilt and Rotation on Coracoclavicular Distance Measurements and Rockwood Classification in Panorama View Radiographs in the Diagnosis of Acromioclavicular Dislocations.

BACKGROUND: The severity of acromioclavicular (AC) joint dislocation is evaluated through bilateral anterior-posterior radiographs of the AC joint. AC joint dislocations are graded based on the classification system of Rockwood, which is the foundation for further decision-making regarding therapy regimen. PURPOSE/HYPOTHESIS: The purpose of this study was to simulate technical irregularities in obtaining panoramic views and the effect they might have on the measured coracoclavicular (CC) distance. It was hypothesized that vertical tilt and horizontal rotation of the radiographic panoramic vie

Type 1 Monteggia Equivalent Fracture with Ipsilateral Distal Radius Fracture: A Case Report.

In this case report, we present a 13-years-old patient who sustained a Monteggia equivalent fracture along with an ipsilateral distal radius fracture following a fall on the elbow. Comminuted ulnar fracture was treated with open reduction and internal fixation with a bridging plate. After restoring the ulnar length, the radial neck fracture was successfully reduced. The distal radius fracture was managed conservatively. Our literature review shows that, the patient is one of the comparatively older patients treated with open reduction and internal fixation in this area and that a successful ou

Nonoperative Management of Ulnar Collateral Ligament Injuries in the Throwing Athlete: A Framework for Return to Throwing.

BACKGROUND: Ulnar collateral ligament (UCL) injuries in throwing athletes result from repetitive valgus stress to the elbow in a flexed position. Muscular dysfunction of the forearm and shoulder musculature is commonly associated with UCL injuries. Nonoperative rehabilitation should generally start with a rest period, followed by progressive strengthening of the upper extremity, and eventually, a systematic throwing program. INDICATIONS: Nonoperative management is first line for UCL injuries. Trained rehabilitation professionals, including physical therapists or athletic trainers, should be i

Combined Pediatric T-condylar Humeral Fracture and Monteggia Fracture-Dislocation in the Upper Extremity: A Case Report.

Monteggia fractures are characterized by a fracture of the ulna associated with radial head dislocation. T-condylar fractures of the humerus are intra-articular injuries that disrupt the distal humeral epiphysis. This report describes a rare case of a pediatric patient with an ipsilateral T-condylar humeral fracture and a Monteggia fracture-dislocation. A 10-year-old girl presented with elbow pain and deformity after falling while playing basketball. No open wounds or neurological deficits were observed, and the radial artery was palpable. The radiograph revealed a pediatric T-condylar humeral

Distal Triceps Tendon Ruptures: A Case Series Highlighting Diagnostic Challenges, Surgical Management, and Functional Outcomes.

Triceps tendon ruptures are uncommon but significant injuries, often presenting diagnostic challenges due to nonspecific clinical features. Delayed or missed diagnoses can result in prolonged functional impairment. We present a retrospective case series of five patients who sustained distal triceps tendon ruptures through various mechanisms, including direct trauma and heavy lifting. Clinical presentations ranged from elbow swelling and pain to palpable tendon gaps, with radiographic "flake signs" consistently observed. Ultrasonography effectively identified tendon ruptures and retraction in f

An Inferential Investigation Into Countermovement Jump Determinants of Ulnar Collateral Ligament Injuries in Collegiate Baseball Pitchers.

BACKGROUND: Countermovement jump (CMJ) analyses can predict ulnar collateral ligament (UCL) injuries in professional baseball pitchers, yet a biomechanical determinant linking CMJ analytics to UCL sprains is unknown. PURPOSE/HYPOTHESIS: The purpose of this study was to evaluate CMJ parameters in collegiate pitchers with high and low elbow varus torque (EVT) and investigate multilinear regression relationships between CMJ and EVT kinetics. It was hypothesized that pitchers with greater EVT would have greater CMJ measures, and CMJ kinetics would explain the variance in EVT kinetics. STUDY DESI

Arthroscopic Pectoralis Minor Release and Infraclavicular Brachial Plexus Decompression for Neurogenic Thoracic Outlet Syndrome: A Novel Treatment for an Old Problem.

BACKGROUND: Neurogenic thoracic outlet syndrome (nTOS) of infraclavicular etiology is a complex condition involving the compression of the brachial plexus through the interscalene triangle and costoclavicular, infraclavicular, and pectoralis minor space. New insight into nTOS of infraclavicular etiology and its association with scapular dyskinesia has enabled minimally invasive treatments: endoscopic pectoralis minor release (PMR) and infraclavicular brachial plexus neurolysis. The purpose of this study was to analyze clinical outcomes of this technique compared with historically published out

The clinical outcomes of open reduction and internal fixation for Mason-Johnston type IV fractures of the radial head.

BACKGROUND: The treatment of displaced or comminuted Mason-Johnston type IV radial head fractures is challenging. These fractures often involve complex injuries to the ligaments surrounding the radial head, necessitating careful consideration of treatment strategies that prioritize ligament repair while preserving the integrity of the radial head whenever feasible. The primary objective of this study was to assess the clinical outcomes of open reduction and internal fixation (ORIF) in managing complex radial head fractures associated with transolecranon fractures, Monteggia fractures, and terr

En-Bloc separation of the distal humerus articular appendage.

The unique anatomy of the distal humerus predisposes it to coronal shear injuries of its articular appendages. These injuries often require an open reduction and internal fixation of articular fragments. The existing classification systems often overlook the depth of sub-chondral bone involvement. This case report discusses a rare variant of a distal humerus articular injury which involved an 'en-bloc separation of the anterior articular cartilage'. A young male in his 20s presented with a coronal shear fracture of the distal humerus, fixed using combined lateral and medial surgical approaches

Trends, complications, and comorbidity risk in total elbow arthroplasty versus open reduction and internal fixation for distal humerus fractures.

BACKGROUND: Treatment of distal humerus fractures (DHF) in older patients presents challenges due to high complication rates. Total elbow arthroplasty (TEA) has emerged as an alternative to open reduction and internal fixation (ORIF). This study investigates trends, outcomes, and risk factors associated with TEA and ORIF for the treatment of DHF. METHODS: Using the PearlDiver Mariner database, patients aged 55 years and older treated with either TEA or ORIF for DHF between 2010 and 2021 were identified. Trends in procedure volume, 90-day complications (postoperative bleeding, wound disruption

Outcomes and challenges in the surgical treatment of trans-olecranon fracture-dislocations: A case series study.

INTRODUCTION: Transolecranon fracture-dislocations represent a complex subset of elbow injuries characterized by concomitant fractures of the olecranon, coronoid process, and radial head, often associated with posterior dislocation. These injuries pose significant surgical challenges due to their inherent instability and high risk of poor functional outcomes. Despite previous studies describing different fixation techniques, there is no standardized surgical protocol to optimize outcomes. This study aims to evaluate the clinical and functional results of a structured surgical approach in the m

Does resident involvement impact postoperative outcomes after proximal humerus fractures?

BACKGROUND: Proximal humerus fractures (PHFs) are a prevalent orthopedic injury and often require surgical intervention. These surgeries frequently involve resident participation as part of their training, but the impact of this involvement on surgical outcomes remains unclear. This study evaluates the effect of resident participation on perioperative complications and mortality in PHF surgeries. METHODS: The National Surgical Quality Improvement Program database was retrospectively analyzed from 2006 to 2012. Data were extracted from inpatient records of patients with a PHF who received open

Atypical Injuries of the Elbow in Children: A Case Series.

Fractures occurring in the elbow region of children and adolescents pose a notable challenge for orthopedic surgeons due to the complex anatomy and developmental stages of the humerus. Capitulum fractures, a rare form of elbow injury, are classified into three types according to the Bryan and Morrey classification system. Monteggia fractures, a rare occurrence in children, involve a combination of ulna break and radial head dislocation, with Monteggia equivalent fractures expanding upon traditional classifications. The distinction lies in Monteggia equivalent fractures (MEFs) not always involv
