The pediatric T-supracondylar fracture.

T-supracondylar fractures of the humerus occur rarely in children compared with the adult variety. Sixteen such fractures treated at the Children's Hospital of Eastern Ontario between 1975 and 1983 are described. There were nine male and seven female patients, 7-17 years old. All fractures were displaced, two were open, and one patient had an associated radial nerve palsy. All cases were managed by open reduction, internal fixation, and early mobilization. Follow-up revealed no cases of functional disability despite significant loss of motion in two patients. The conservative approach should b

Untreated posterior dislocation of the elbow in children.

Fifteen children with an untreated posterior dislocation of the elbow were seen between 1965 and 1980. Three had a useful range of painless flexion and were not treated by operation. Twelve had a stiff elbow and had an open reduction between three weeks and three years after injury. The triceps was lengthened when it prevented reduction and Kirschner wires were used when necessary to stabilize the elbow. Complications included a transient paralysis of the hand in one patient and myositis ossificans with a rigid elbow in another. The length of follow-up ranged from one to six years. In eleven p

Posterior dislocation of the ulna at the elbow with associated fracture of the radial shaft.

An exceptional case of posterior dislocation of the ulna at the elbow, associated with fracture of the radial shaft, is reported. Diagnosis was established with anteroposterior and lateral radiographs of the forearm, elbow and wrist. Treatment consisted of closed reduction of the ulnar dislocation and open reduction and internal fixation of the radius with an AO plate. A long arm cast was applied for 3 weeks with the elbow held at 90 degrees. Six months later full function of the limb had been achieved.

Open radiocarpal fracture-dislocations.

Ten open radiocarpal fracture-dislocations in nine patients were treated by wound debridement, open reduction, and maintenance of reduction by casting supplemented with internal or external fixation. Eight patients had associated fractures or dislocations of the ipsilateral extremity: elbow fracture or dislocation (three patients), shoulder fracture (one patient), fractured humerus (two patients), and phalangeal fractures (three patients). In seven of ten cases the median and/or ulnar nerve was severely contused and required immediate decompression. Six patients were followed at least 15 month

Post-traumatic posterior radial head subluxation. Two case reports.

Two women, 33 and 55 years of age, with post-traumatic posterior radial head dislocation were treated by open reduction. Normal apposition of the radius to the capitellum was achieved. Although excellent range of motion was restored in both patients, the possibility of post-traumatic arthritis must be determined by long-term follow-up observations.

Ulnar fracture and dislocation of the proximal radial epiphysis (Monteggia lesion) in the dog and cat: a review of 28 cases.

Ulnar fracture with dislocation of the proximal epiphysis of the radius (Monteggia lesion) was diagnosed in 16 dogs and 12 cats. The lesions were classified into 4 types on the basis of direction of the dislocation of the radial head and angulation of the ulnar fracture. Cranial dislocation of the radial head, with cranial angulation of the ulnar fracture (type 1 lesion) was the most common. Radial head chip fractures were frequently associated with the lesions. In 25 cases, open reduction and internal fixation were required. The most common postoperative complications were restricted range of

The efficacy of flurbiprofen versus piroxicam in the treatment of acute soft tissue rheumatism.

Fifty patients suffering from acute soft tissue rheumatism entered an open parallel group study of flurbiprofen 200 mg to 400 mg daily versus piroxicam 20 mg to 40 mg daily for 4 weeks. Both treatments caused statistically significant improvements in pain, pain on active movement, restriction of passive movement and reduction of strength. Global assessments of the patients' progress also improved significantly with both compounds. Flurbiprofen was significantly superior to piroxicam with regard to relief of pain at Day 28, pain on active movement at Days 14 and 28, pain on passive movement at

Trauma on eight wheels. A study of roller skating injuries in Seattle.

Thirty-five fractures secondary to roller skating accidents from March 15 to October 1, 1979 were treated in the University of Washington-affiliated hospitals. Twenty-eight involved the wrist and elbow. Only three were in skeletally immature individuals. Sixty-three percent of the patients were female with the 20- to 34-year-old age group the most commonly involved. A fall, usually backwards, onto outstretched arms and hands was the most frequent mechanism of injury. A detailed study of the popular outdoor skating area in Seattle, Green Lake, indicated that more than 75% of the injuries occurr

Surgical treatment of T and Y fractures of the distal humerus.

Intercondylar T and Y fractures of the distal humerus in adults present a challenge. Open reduction and internal fixation in 19 cases over a 7-year period gave excellent or good results in 15 cases. Campbell's posterior approach to the elbow joint gave a satisfactory exposure. Anterior transposition of the ulnar nerve is a very useful adjunct to prevent late neuritis. A new method of classification is suggested, taking into account the importance of the supracondylar ridge for adequate stability.

Multicenter study of piroxicam in the treatment of acute musculoskeletal diseases involving 3011 patients.

An open non-comparative study was conducted to assess the efficacy and tolerability of piroxicam in patients with acute musculoskeletal disease. A total of 3011 patients of both sexes, aged between 18 and 70, were treated with piroxicam: 4 capsules (40 mg) after dinner, on the first two days and two capsules (20 mg) on the following days. Patients were evaluated for a maximum period of 14 days. Assessment of efficacy was made including local pain and limitation of mobility. The patients' self-assessment of pain was also analyzed. A clear improvement in each of the parameters analyzed was found

The isolated fracture of the ulnar shaft. Treatment without immobilization.

The isolated fracture of the ulnar shaft is notorious for prolonged healing time. Over a twenty-seven-month period, seventy-one such fractures were treated at our institution. The initial twelve fractures were immobilized with the standard axilla-to-palm plaster cast. They had an average healing time of 10.5 weeks and a non-union rate of 8 per cent. The remaining fifty-nine fractures were treated without a cast or with a cast or splint for no longer than two weeks after injury and then mobilization as tolerated. In this group the average healing time was 6.7 weeks and there were no non-unions.

Lateral Monteggia fracture in children (case report).

A lateral Monteggia fracture in a child is reported. Between 1941 and 1977, only 16 such cases were reported in the literature. Of these, 2 cases needed open reduction of the fractured ulna. The present case needed osteotomy of a nearly healed fracture of the ulna and reconstruction of the orbicular ligament to stabilise the reduction of the radial head. No similar case has previously been described in a child. Prompt diagnosis and early reduction should yield good results by closed methods of treatment, but open reduction is necessary if the diagnosis is missed and treatment is delayed. Even

[Diagnosis and therapy of capitulum humeri fractures].

Report of four cases of fracture of capitulum humeri treated by operation. The necessity of an exact preoperative radiological diagnostic work-up including lateral tomography of the elbow is pointed out. It is described how difficulties occurring during and after surgery can be avoided by choice of an adequate approach and a suitable internal fixation. Presented is the radio-volar approach and temporary fragment fixation with K-wires from ventral to dorsal. Three cases treated in a similar way show the efficiency of the procedure.

Very deep high temperature burns associated with blunt abdomino-thoracic trauma.

A 14-year-old boy with deep burns, blunt abdominal trauma and extensive muscle damage is described. The patient was operated on for a ruptured liver, developed acute tubular necrosis, had 3220 gr of necrotic tissue removed in serial tangential excisions, had an above elbow amputation of the left hand, removal of necrotic ribs and excisions of necrotic spinal processes, developed sepsis twice, empyema of right pleura, had open infected knee joints and was successfully treated until complete healing over a period of four months. The successful outcome seems to be due to early and aggressive surg

[Injuries of the proximal humeral epiphysis. Indications for surgical therapy and results].

Lesions of the proximal humeral epiphyseal plate occur most frequently as epiphysiolysis with metaphyseal fragment between the age of 12 and 16 years. The conservative treatment with Desault plaster cast is recommended for stable fractures. In unstable fractures the percutaneous K-wire osteosynthesis after reduction is the adequate procedure. Hypertrophic scars were found after open reduction in half of all patients treated. Only when closed reduction was impossible, the fracture should be exposed and stabilised with K-wires and/or lag screws. All 21 patients who were followed-up have been fre

[Supracondylar fractures of the elbow in children and osteosynthesis by "open sky" double wiring using J. Séror's method. Apropos of 100 cases].

Emphasis is placed on the value of treating supracondylar fractures of the elbow with displacement (stages III and IV) in children by means of the technique described, in 1959, by J. Séror, J. Rives, J. Frailong, and Cl. Azoulay: "open sky" reduction through a twofold internal and external paratricipital posterior approach; maintenance of reduction by two Kirschner wires entering by the epicondyle and epitrochlear, with fixation into the opposite diaphysial cortex, and sectioned and curved inwards in order to remain subcutaneous. Analysis of the results of 100 cases treated in this manner betw

Forearm fractures in the head-injured adult.

Forty-seven of 661 head-injured adults sustained 50 forearm fractures. Eight extremities exhibited elbow flexion contractures greater than 55 degrees. Traumatic heterotopic ossification at the elbow developed in ten (20%) extremities: four with Monteggia fractures, two with olecranon fractures, and four with no known trauma. Only two of 18 (11%) extremities treated by plaster immobilization achieved good or excellent results, while 17 of 32 (53%) extremities treated by open reduction and internal fixation achieved good or excellent results. Union of fractures of one or both bones occurred at t
