According to a follow-up of 50 fractures of the proximal end of the radius in adolescence the cause and prevention of later persisting functional disturbances, posttraumatic valgisation of the elbow axis and posttraumatic growth disturbances are investigated. Twenty-three patients, aged from 10 to 60 years, underwent open reduction for untreated posterior dislocations of the elbow. The dislocations had been unreduced for periods varying from one month to two years. All patients underwent a standard procedure based on the Speed technique. Complications after operation included one case of gross sepsis and five cases of ulnar neuritis. Most patients gained a useful range of flexion-extension of the elbow. Neither the age of the patient nor the duration of the unreduced dislocation influenced the result.

Between 1968 and 1978, 32 patients were seen with nonunion of distal humerus fractures in close proximity to the elbow: 25 were treated with open reduction and fixation of the nonunion, and seven patients were treated with excision of the distal fragments and total elbow arthroplasty. An outbreak of Staphylococcus aureus furuncles (boils) occurred among 26 members of a central Illinois high school football team. A total of 55 boils were reported with a diameter of induration ranging from 0.25 to 6 inches (mean = 1.25 inches). A 7-month-old girl sustained a deep and extensive burn of the posterior aspect of the left upper arm and forearm, complicated by an open wound of the elbow joint with exposed bone and articular cartilage. A bipedicled chest wall flap was used to cover the defect. The limb was preserved along with good function in the elbow joint. An above-elbow amputation was thus avoided.

Twenty-five children with combined elbow injury and forearm bone fractures have been reviewed. The incidence of serious complications was low, and only one patient was significantly handicapped. Open reduction and internal fixation of the supracondylar fracture with closed treatment of the forearm fracture gave the best results.

Two cases of fracture of the ulna with fracture-separation of the radial neck in children are presented. We reviewed the cases of 107 patients who underwent surgical treatment of an isolated fracture of the olecranon. Fifty-three patients were treated by primary excision and fifty-four, by open reduction and internal fixation by various methods. A surgical technique of open drainage, using penrose drains and pressure bandages, was used for the treatment of hygromata in four horses. In 3 cases with hygroma of the carpus, one with a secondary infection, there was complete resolution of the condition whereas in the one case with hygroma of the elbow resolution was more protracted because of difficulties in bandaging. The cosmetic and functional results after operation were most satisfactory.

This investigation reports on 30 female gymnasts with 32 elbow injuries acquired in gymnastics. Thirty of the injuries were acute fractures and dislocations, and two developed Panner's disease of the humeral capitellum. The vascular anatomy and the cutaneous territorial definition of the brachioradialis myocutaneous flap has been studied by cadaver dissection and intra-arterial injection of methylene blue eye. A case is presented in which a proximal brachioradialis myocutanenous flap, based on the vascular pedicle arising from the radial recurrent artery just distal to the elbow level, was used to cover an exposed open elbow joint. The advantages and disadvantages of this flap are briefly discussed.

The anterior oblique component of the medial collateral ligament of the elbow is the mainstay of joint stability. Fractures of the medial epicondyle must be anatomically reduced, open if necessary. A fibrous union of a minimally displaced fractured medial epicondyle may result in lengthening and functional compromise of the medial collateral ligament. Chronic elbow instability is an unusual lesion. Repair of chronic elbow instability is best performed by restitution of medial collateral ligament function.

The results of 193 conservatively treated acute fractures of the carpal navicular are presented. Ninety-eight (50.7%) had horizontal oblique fractures, 82 (42.5%) transverse, and 13 (6.8%) vertical oblique fractures.
