Described is a case of bilateral epicondylar entrapment following fracture dislocations of elbows in a 13-year-old girl who fell while performing on parallel bars. Close manipulative reduction was unsuccessful. Open reduction and internal fixation yielded excellent results. Ulnar nerves were not transposed anteriorly as has been recommended by some authors. Long-term follow-up in five previous cases has shown no evidence of tardy ulnar nerve palsy. The need for transposing the ulnar nerve is questioned.

We have illustrated instances in which open reduction with internal or external fixation should be used in the care of fractures in children. These methods should be used when closed reduction fails or in other circumstances in which they are the initial treatment of choice. At no time should the surgeon hesitate to utilize these operative options if they represent the best method of achieving the most functional end result in a pediatric fracture.

Medical rehabilitation was fulfilled in 75 patients with intra-articular fractures of the distal metaepiphysis of the shoulder bone. The authors came to the conclusion that the main methods of treatment of fresh intra-articular fractures of the distal metaepiphysis of the shoulder bone consisted in conservative measures (a single reposition, skeletal extension and compression-distractional osteosynthesis). In cases of the open reposition followed by osteosynthesis a sparing technique of surgical intervention should be used.

Fractures of the neck of the radius may cause considerable permanent limitation of elbow and forearm movement. This complications resulted from failure to achieve adequate reduction or from vascular disturbance. The authors make use of a Kirschner wire introduced in the medullary canal by its lower end. It is possible to reduce and stabilise the head of the radius without open reduction.

Fracture-separation of the distal humeral epiphysis in young children is frequently misdiagnosed as a fracture of the lateral humeral condyle or a dislocation of the elbow. The roentgenographic relationships between the ossification center of the capitulum humeri and the radius are important in distinguishing between the three conditions. Arthrographic studies in our patients showed that this is an extracapsular supracondylar fracture. In six fracture-separations treated either by open reduction and wire fixation or in overhead traction, the results were excellent.

Radial nerve compression palsies at the elbow and forearm result in a lower radial nerve palsy whose clinical data are generally a hardly diagnosed dissociated radial palsy, or a rough paresis in the range of epicondylalgia. Apparently spontaneous non traumatic compressions are rare. They are essentially due to lipomas, to fibromas, or sometimes to synovial cysts of the elbow. In the course of rheumatoid arthritis, palsies must be isolated. Besides, there is an actual inflammatory canal syndrome of epicondylalgias range, in which radial nerve paresis must be investigated. Traumatic compression

Eighty cases of supracondylar fracture of the humerus at the elbow in children have been analysed. The post-reduction radiograph of the fracture was classified depending on the residual deformity present and related to the loss of carrying angle and cubitus varus deformity at the time of follow-up. Fifty-four percent of the displaced fractures showed a loss of carrying angle greater than 5 degrees, and of these 23 per cent developed cubitus varus deformity. Eighty-five per cent of those cases with a loss of carrying angle greater than 10 degrees showed either residual tilt or medial rotation o

A series of 1,329 patients with 1,327 fractures and 100 dislocations has been studied. Boys under 10 years of age predominated. The majority of the injuries resulted from falls or road traffic accidents. Upper limb fractures occurred more commonly than those of the lower limbs or axial skeleton. Femoral shaft fractures were 2-1/2 to 5 times more common while femoral neck fractures were three to five times less common than in Americans. Dislocations were most common at the elbow, followed by the shoulder. Only 11.5% of the patients had open operations; the others were treated conservatively. Th

Injuries of the distal forearm region demand careful examination in children if green stick fractures or dislocations of the epiphysis with spontaneous reduction are not to be overlooked. The treatment should be principally conservative with a plaster splint including the elbow joint following reduction. Exact and repeated radiological controls are necessary because of loss of reduction is not infrequent. Operative treatment is indicated in cases of any obstruction to reduction, inadequate reduction of fractures with epiphysis involvement, open fractures with marked dislocation and for the cor

A consecutive series of 2579 patients with 2571 fractures and 189 dislocations was studied prospectively. Males and children, especially under 10 years of age, predominated. The majority of the injuries resulted from falls either in the home or outdoors and from road traffic accidents. Fractures occurred more commonly in the upper limb than in the lower limb or the axial skeleton. Fractures of the shaft of the femur were three times more common, while fractures of the neck of the femur were less common by the same factor, in Africans than in Caucasians. Dislocations were commonest at the shoul

Six patients with soft and hard tissue injuries of the elbow region during the 1973 Yom-Kippur war are discussed. These six patients were treated with two types of procedures. One group was initially treated with application of a split-thickness skin graft followed by a thoracoabdominal, abdominal, or chest skin flap. The second group was initially treated with the application of a thoracoabdominal flap. There are few descriptions in the literature or in textbooks of the plan design method, nor are there comparisons of two types of procedures. In both groups the end results were the same. The

The management of an anterior supracondylar fracture by closed reduction, traction, percutaneous pinning or open reduction is seldom satisfactory, especially for adults and the aged with a lesser ability to remodel and a slower functional recovery. A new, safe, and simple technique is described in which the condylar mass is pushed posteriorly along the axis of the forearm and the hand is rotated to full supination while the elbow is held in flexion to correct deformities. Fixation is divided into two parts: the circular cast around the upper arm provides a firm buttress onto which the lower fr

A follow-up study of 20 patients operated upon for fracture of the capitulum humeri is reported. In 8 cases the fracture fragment was removed while in 12 others open reduction was performed. Open reduction gave the best results and is therefore recommended as the treatment of choice.

Of the 115 elbow dislocations treated at two New Orleans hospitals between 1959 and 1969, 78% were posterior. Sixty-one percent of the patients were 21 years of age or younger, and the male-female ratio was 1.7:1.0. The most common associated injuries were fractures of the medial condyle or epicondyle. All uncomplicated dislocations were treated by closed reduction and plaster immobilization. Of the 35 cases complicated by medial condylar or epicondylar fractures, 29 required open reduction and internal fixation. Results were excellent or good in 91% of the cases. The three patients (2.6%) wit

Because of its technical improvement percutaneous pinfixation of bones is now a successful procedure of osteosynthesis. The indications of this method are: Open fractures of second and third degree, emergency treatment of polytraumatized persons with debris fractures, fractures in combination with vascular injuries or burns, elongating osteotomy, pseudarthrosis, osteomyelitis and arthrodesis. External pin-fixation offers the following advantages: High range of stability with the possibility of early exercises. In case of resorption of the bone fragments it is possible, to correct the axial com

A series of supracondylar fractures of the humerus in children is presented. The majority was reduced by closed manipulation. The difficult fractures were defined as those in which adequate reduction either could not be achieved by manipulation or was not maintained, or those in which neurological or vascular complications occurred. Such cases were treated by open reduction and internal fixation. The results were assessed with regard to loss of elbow movement, deformity and symptoms. The results of the operative series were comparable with those achieved by closed manipulation in the easier ca

An analysis of 144 sports accidents of 275 tennis players in Switzerland (203 men, 72 women) showed fractures of bones in 5,5% open wounds in 14,5%, torsions in 33% and other injuries in 47%. The head was injured in 9,5 and the legs in 76%, the arms in 11,5% and the trunk in 11,5%. Two fifth of all leg injuries were on the left side, three fifth on the right side. The arms were hurt in 98% on the right and only in 2% on the left side. Causes of accidents were in 21% slipping on wet ground, in 17% related to the rackets, in 33% a too speedy start with damage to muscles and tendons, in 14% stepp

For the conservation of crushed extremities all achievements of up-to-date traumatology are to be used. The impairement of the seriously crushed extremity displays a certain similarity to the polytraumatism involving the whole organism. This is to be taken into consideration in the surgical treatment. Conserving the vitality of the extremity deferred treatment and reconstructive operations at a later date become possible. As absolute rule must be in view that all cases are to be examined with the greatest care and individually, as regards the surgical treatment.
