Partial Tear of the Common Flexor Tendon and Medial Ulnar Collateral Ligament in a Six-Year-Old After a Minor Traumatic Injury: A Case Report.

Acute traumatic medial ulnar collateral ligament (MUCL) injury, associated with a high-grade sprain of the common flexor tendon and a low-grade sprain of the annular ligament, is an uncommon occurrence in the non-athletic pediatric demographic. At present, there is a lack of high-quality, evidence-based research that details comparable injuries. Furthermore, to our knowledge, there are no established guidelines that provide direction for the optimal treatment of this specific injury. We report a case involving a six-year-old boy who suffered an acute traumatic MUCL injury to his right elbow. T

Pitch-Tracking Risk Factors and Warning Signs for Ulnar Collateral Ligament Injuries in Major League Baseball Pitchers.

BACKGROUND: Ulnar collateral ligament (UCL) injuries remain highly prevalent among baseball pitchers across all competitive levels, and there is a need for improved prevention and early detection. The increasing availability of advanced pitch-tracking data allows for granular evaluation of pitching changes that may precede UCL injury, offering a potential tool to identify risk factors and warning signs for injury. PURPOSE: To identify baseline pitch-tracking risk factors present in the preinjury season and to detect temporal warning signs of UCL injuries across sequential time windows, includ

Masquelet technique in adolescent distal humerus fractures with bone loss.

Adolescent distal humerus fractures usually result from high-energy trauma; however, open fractures with associated bone loss are quite rare. Although the Masquelet technique has demonstrated proven success in the reconstruction of lower extremity bone defects, its use in upper extremity periarticular defects, especially in adolescents, has been very rarely reported. This study aims to present the clinical and radiological outcomes of the Masquelet technique in adolescent distal humerus fractures with bone loss. A retrospective review was conducted of patients aged 12-16 years who underwent th

Pitch Counts and Injury Incidence in Major League Baseball: Responses to Pitch Clocks and Defender Rule Changes.

BACKGROUND: The implementation of the Major League Baseball (MLB) pitch clock in 2023 aimed to shorten game durations and enhance pace of play. However, concerns have been raised regarding its potential effect on pitcher health, particularly related to musculoskeletal and elbow-specific injuries. PURPOSE: To determine if recent MLB pitch clock rule changes have affected total pitch counts, time to first injury, total injury time, pitches per inning, total batters faced, or total musculoskeletal injuries for the 200 highest-volume pitchers across 2 full seasons postimplementation (2023 and 202

Do Pitching Restriction Policies Reduce Shoulder and Elbow Injuries in High School Baseball Players?

BACKGROUND: Shoulder and elbow injuries (SEIs) remain prevalent among high school (HS) baseball players. Pitching restriction (PR) policies were mandated by the National Federation of State High School Associations (NFHS) in 2016 as an injury prevention strategy to set workload limits and reduce overuse injuries. The effect of PR policies on HS baseball SEIs remains unknown. HYPOTHESES: (1) SEI rates in HS baseball players would be lower in the 3 seasons after PR policy implementation when compared with the 3 seasons before PR policy implementation and (2) specific components of PR policies w

Predicting complications following proximal humerus fracture repair using the 6-item modified frailty index.

BACKGROUND: Frailty is a well-established predictor of poor outcomes after orthopedic surgery. The 6-item modified Frailty Index (mFI) 6, which includes hypoalbuminemia, may provide enhanced risk stratification compared to the widely used 5-item mFI-5. The predictive value of the mFI-6 vs. the mFI-5 for short-term complications after surgical management of proximal humerus fractures (PHFs) remains unclear. METHODS: A retrospective analysis of the American College of Surgeons National Surgical Quality Improvement Program database was conducted to identify patients who underwent open reduction

Age-Related Medial Elbow Laxity in Youth Baseball Players: Ultrasonographic Evaluation.

BACKGROUND: Medial elbow joint instability is a critical concern among young baseball players and is associated with throwing-related injuries. Greater medial elbow joint space (MEJS) gapping has been proposed as a key indicator of valgus instability. However, age-specific differences remain unclear, particularly in elementary school-age pitchers. HYPOTHESIS/PURPOSE: This study aimed to clarify age-related differences in MEJS gapping by comparing joint space changes between resting and gravity stress conditions among young baseball players. It was hypothesized that 12-year-old players would s

Capitellum Fractures in the Pediatric Population: A Case Report.

Capitellum fractures are rare injuries in the pediatric population. Their diagnosis is particularly challenging due to the predominantly cartilaginous nature of the pediatric elbow, which often limits the sensitivity of conventional radiographs. Consequently, these fractures are frequently overlooked at initial presentation. We present the case of a nine-year-old boy who sustained indirect trauma to the left elbow after a fall from standing height. On clinical examination, the patient complained of pain, mainly in the lateral aspect of the elbow, with refusal to actively mobilize the joint and

Pediatric Floating Elbow: A Case Report of Combined Supracondylar Humerus and Distal Both Bone Forearm Fractures in an 11-Year-Old Boy.

The term "floating elbow" describes a rare injury pattern in children involving ipsilateral fractures of the humerus and forearm bones. These high-energy injuries are frequently associated with neurovascular compromise and carry a risk of compartment syndrome. We report an 11-year-old boy who sustained a fall from height, resulting in a left supracondylar humerus fracture (Gartland type III) and ipsilateral distal radius and ulna fractures. Closed reduction and percutaneous K-wire fixation were performed for the supracondylar fracture, and open reduction with K-wire fixation was done for the d

An unusual pediatric capitellar-trochlear shear fracture: a case report and narrative review.

INTRODUCTION: Capitellum fracture (CF) of the distal humerus is an unusual and rare pediatric fracture. CF is not only confined to the capitellum but can extend into the trochlea. Treatment options vary from casting to surgical fixation based on displacement and fracture characteristics. CASE PRESENTATION: A 13-year-5-month-old boy who presented after a fall on his left elbow was found to have a displaced coronal shear CF extending into the trochlea, type 4 McKee fracture/type 2A Dubberley. An open reduction and internal fixation (ORIF) was performed through a lateral approach using headless

Social determinants of health and outcomes in proximal humerus fractures based on surgery type.

BACKGROUND: Proximal humerus fractures (PHFs) account for 5% to 6% of all adult fractures. The optimal surgical management for PHFs remains under debate, with 2 of the most common operations being open reduction and internal fixation (ORIF) and reverse total shoulder arthroplasty (rTSA). Social determinants of health (SDOHs) have gained particular attention in many medical fields because of their relationship to health outcomes, with the Social Vulnerability Index (SVI) as an example of an adopted measure of geographic disadvantage. The purpose of this study was to investigate the associations

Terrible Triad Injury of the Elbow Associated With an Olecranon Fracture: A Case Report With Three-Year Follow-Up.

Terrible triad injuries of the elbow consist of elbow dislocation associated with fractures of the radial head and coronoid process and are characterized by significant instability. Variants that include an associated olecranon fracture are rare, and data regarding long-term functional outcomes and patient-reported outcome measures remain limited. We report the case of a 74-year-old female patient who sustained a terrible triad injury of the elbow associated with an ipsilateral olecranon fracture following a low-energy fall. This injury pattern has occasionally been referred to as a "terrible
