Knotless suture anchor fixation of Hahn-Steinthal fractures of the capitellum: A technical note with preliminary clinical outcomes.

BACKGROUND: Hahn-Steinthal fractures are rare osteochondral injuries of the humeral capitellum that primarily affect young and active patients. Although several fixation techniques have been described, most rely on metallic implants or knot-tying constructs that may increase intra-articular bulk and the risk of cartilage damage. The use of knotless suture anchor fixation for this specific fracture pattern has not yet been specifically described. To our knowledge, this study represents the first report describing knotless suture anchor fixation specifically for Hahn-Steinthal fractures of the c

Use of a Bio-Inductive Collagen Implant in Distal Triceps Tendon Repair: A Case Report.

Distal triceps tendon injuries are uncommon but clinically significant, typically occurring after heavy eccentric loading and resulting in loss of active elbow extension. Complete ruptures are traditionally treated with surgical repair using transosseous tunnels or suture anchors; however, high retear rates have been reported following traditional repair techniques. Bio-inductive collagen implants have demonstrated efficacy in augmenting rotator cuff repair by promoting neovascularization and tissue regeneration; however, their use in distal triceps tendon injuries has not been previously repo

Do surgical technique and pin configuration influence clinical and radiological outcomes in pediatric supracondylar humerus fractures? a modified Flynn-based analysis.

BACKGROUND: Pediatric supracondylar humerus fractures are among the most common elbow injuries in childhood. Although K-wire fixation is widely accepted, the optimal surgical technique and pin configuration remain controversial. Additionally, the Modified Flynn criteria, which assess both functional and cosmetic outcomes, have been less frequently utilized in the literature. AIM: This study aimed to evaluate whether surgical technique and pin configuration influence clinical and radiological outcomes in pediatric supracondylar humerus fractures using Modified Flynn criteria. METHODS: This re

Bilateral Distal Myotendinous Biceps Ruptures: A Case Report.

CASE: A 41-year-old man presented with bilateral distal biceps tendon ruptures at the distal myotendinous junction, sustained in 2 separate incidents 15 months apart. This injury pattern is rare and differs from the more common avulsion from the radial tuberosity. Initial imaging was inconclusive, and the diagnosis was aided by physical examination. He elected to undergo open repair of the right side after persistent weakness with elbow flexion and supination. CONCLUSION: This case illustrates an uncommon location of distal biceps rupture and emphasizes the importance of careful clinical eval

Is surgery always necessary? Successful closed reduction of displaced capitellum fracture: A case report.

Distal humerus shear fractures are uncommon intra-articular injuries including capitellum fractures, most frequently resulting from a fall on an outstretched hand where the radial head pushes the capitellum while it engages in a full elbow extension and axial load. Displaced fractures, particularly those classified as Dubberley Type 2A capitellum fracture, are typically managed surgically with open reduction and internal fixation (ORIF) to restore articular congruity and allow early mobilization. Successful nonoperative treatment in such cases remains limited with high complication rates. A 35

Ulnar Collateral Ligament Hybrid Reconstruction With Palmaris Longus Autograft, Suture Brace Augmentation, and Ulnar Nerve Transposition.

BACKGROUND: Many overhead athletes, both amateur and professional, experience an increasing number of injuries to the ulnar collateral ligament (UCL) as a result of overuse or trauma. As a result of this increasing frequency, multiple techniques have been developed to reconstruct or repair the UCL with success and faster return to sports (RTS) for athletes who are ideal candidates for a repair. However, a large proportion of UCL tear patients cannot undergo repair due to chronic injury, leading to poor tissue quality, midsubstance tears, or older age, which predispose them to slower healing an

Management of proximal humerus fractures: a review of contemporary treatment options.

Proximal humerus fractures are frequent injuries, representing the third most common fracture in geriatric patients. Due to the variety of fracture patterns, patient profiles, and subspecialty training of treating physicians, management of these fractures continues to garner debate. Nonoperative treatment remains the mainstay for most fractures; however, operative treatment has increased over the last decade. The purpose of this article is to provide a contemporary review of the clinical evaluation, classification, and management of proximal humerus fractures.

Outcome of Retrograde Titanium Elastic Nailing System in Shaft of Humerus Fractures: A Case Series.

INTRODUCTION: Humeral shaft fractures account for approximately 1-3% of all fractures and present a unique surgical challenge due to the proximity of the radial nerve. Retrograde titanium elastic nailing system (TENS) has gained increasing acceptance as a minimally invasive, load-sharing implant for managing such fractures, particularly in adolescents and young adults. This case series evaluates the functional and radiological outcomes of retrograde TENS in humeral shaft fractures. MATERIALS AND METHODS: We present a series of 10 patients (age range 19-55 years; M:F = 7:3) treated with retrog

Osteochondral Autograft Options for Reconstruction of Intra-articular Distal Humerus and Proximal Ulna Injuries in Physiologically Young Patients.

PURPOSE: Surgical options are limited for distal humerus and proximal ulna injuries with articular bone loss in physiologically young patients. Osteochondral grafts are a potential treatment option between open reduction internal fixation and arthroplasty that does not impart lifelong activity restrictions and has longer longevity. Feasible graft harvest sites that match the surface geometry of the distal humerus and proximal ulna are unknown. This study assesses the surface geometry of potential grafts with known well-defined donor site morbidity for the reconstruction of distal humerus and p

Effect of prior rotator cuff repair technique and timing on clinical outcomes of reverse shoulder arthroplasty.

AIMS: Reverse shoulder arthroplasty (RSA) after prior rotator cuff repair (RCR) is commonly performed for recurrence of pain and arthritic progression; however, the influence of prior RCR timing and technique on outcomes of RSA has not been adequately explored. Our primary aim was to evaluate the relationship between the postoperative American Shoulder and Elbow Surgeons (ASES) score, whether prior RCR was performed open or arthroscopic, and the time interval between RSA and prior RCR. Secondary outcomes were forward elevation (FE) and external rotation (ER). METHODS: A prospectively collecte

Endosteal Button Fixation for Latissimus Dorsi Tendon Rupture in a Bodybuilding Athlete.

BACKGROUND: Bodybuilders are uniquely at risk of shoulder injuries given training intensity, frequency, and heavy overhead loads through a full arc of motion. Latissimus tears are rare injuries that have increased incidence in overhead and weightlifting athletes, following forceful hyperabduction/adduction during sport-specific activities. Humeral avulsions of the latissimus tendon are treated with endosteal button fixation. INDICATIONS: Endosteal button repair is indicated for acute or chronic latissimus tears up to 2 years after injury, in patients who are unable to return to their desired

Comparative Study of Functional and Radiological Outcomes of Antegrade Versus Retrograde Titanium Elastic Nailing for Pediatric Ulnar Diaphyseal Fractures.

Background Pediatric forearm fractures account for approximately 40% of childhood skeletal injuries, with ulnar fractures being particularly prevalent. Titanium Elastic Nailing (TEN) offers an effective minimally invasive option for these injuries; however, the optimal nail entry point - antegrade (olecranon) versus retrograde (distal ulna) - remains debated. This study compared functional and radiological outcomes, complication profiles, and implant-related morbidity between the two approaches. Methods A retrospective comparative study was conducted from May 2023 to May 2025 at the R.L. Jalap

Open repairs with biceps rerouting does not impact retears in large to massive rotator cuff tears compared to conventional open repair: a randomized clinical trial.

BACKGROUND: The treatment of large to massive rotator cuff tears leads to a high failure rate and several techniques have been proposed to improve these results. Rerouting the long head of the biceps tendon (LHBT) appears to be a biological reinforcement to increase tendon healing. Therefore, this study aimed to compare the clinical and radiological outcomes of open repair for large and massive rotator cuff tears reinforced with the LHBT to conventional open repair. METHODS: A prospective randomized study was conducted with patients diagnosed with large to massive rotator posterosuperior rota

The Persistent Preference for Traditional Bonesetting in Ethiopia: Risk Factors and Missed Opportunities.

INTRODUCTION: Many patients in Ethiopia prefer traditional bonesetting (TBS) over formal health services for musculoskeletal injuries, despite well-documented complications. We analyzed risk factors for initial and persistent TBS preference among patients who patronized TBS and subsequently presented to a tertiary hospital for definitive treatment. METHODS: Over 15 months, we prospectively enrolled patients with musculoskeletal injuries who presented to 8 Ethiopian tertiary hospitals after patronizing TBS for the same injury. Multivariable regression evaluated associations between TBS prefere

Safety and Effectiveness of Placental Connective Tissue Matrix Allograft Versus Corticosteroid Injection in Adhesive Capsulitis: A Double-Blinded Prospective Randomized Study.

BACKGROUND: Adhesive capsulitis causes significant pain and functional limitation, with limited effective treatment options. Current standard-of-care treatments, such as corticosteroid injections, may offer short-term symptom relief but are associated with limited long-term efficacy and potential side effects. Regenerative therapies using extracellular matrix-based products derived from placental tissue have shown promise in treating musculoskeletal injuries due to their anti-inflammatory and tissue-healing properties. Clinical data supporting their use in adhesive capsulitis remains limited.
