A Rare Adolescent T-Y Distal Humerus Fracture with Capitellar Shear Component Managed Using a Triangular Cannulated Screw Construct and Herbert Screw Fixation: A Rare Case Report.

INTRODUCTION: T-condylar (T-Y) distal humerus fractures are rare intra-articular injuries in adolescents (<2% of pediatric elbow fractures). The coexistence of a capitellar coronal shear fragment is exceptionally uncommon and can be easily missed on radiographs. We report successful management of this rare injury using computed tomography (CT)-guided planning and stable screw fixation. CASE REPORT: A 12-year-old boy presented after a fall with a displaced T-condylar distal humerus fracture. CT demonstrated an associated capitellar coronal shear fragment. Open reduction through a posterior par

Ipsilateral concomitant fractures of the distal radius and radial head: a radial Joint-Coupled Pattern (JCP).

BACKGROUND: Radius bipolar fractures are characterized by an ipsilateral combination of fractures in both the distal and proximal radius, which occur much less commonly than their separate counterparts. We propose the term Joint-Coupled Pattern (JCP) to describe this distinct injury, defined as a single bone fractured at both proximal and distal ends, thereby affecting two adjacent joints. CASE PRESENTATION: A 48-year-old female presented with right wrist pain following a motorcycle accident. Initial X-rays revealed a distal radius fracture (2R3B2, volar displacement). Open reduction internal

Tennis Serve Biomechanics, Joint Load Mechanics and Overuse Injuries: A Narrative Review.

BACKGROUND: The tennis serve is a fundamental stroke in a player's performance during matches. But it may be also eminently traumatic as it causes very high joint mechanical loading which can lead to injury regardless of the player's experience, sex or age. This narrative review aims to synthesize current scientific knowledge on the key factors contributing to changes in joint mechanical loadings during the tennis serve and its potential association with overuse shoulder, elbow, wrist and lower back injuries. MAIN BODY: The databases PubMed, Google Scholar, ScienceDirect and SPORTDiscus were

Parallel vs. Orthogonal dual plating for distal humerus fractures: a systematic review and pooled analysis of functional outcomes and union times.

OBJECTIVE: The primary purpose of this study was to determine whether open reduction and internal fixation (ORIF) utilizing parallel or orthogonal dual plating resulted in improved function, as measured by the Mayo Elbow Performance Score (MEPS). Additionally, this study investigated whether union time was influenced by the method of fixation. METHODS: A systematic review and pooled analysis were conducted utilizing five databases to search for articles examining orthogonal and parallel dual plating methods for the treatment of distal humerus fractures. Statistical analysis was conducted util

Lateral Condyle Fractures: Kirschner Wire or Screw Fixation?

Lateral condyle fractures of the distal humerus are the second most common elbow fracture among children. These fractures present unique considerations because of their intra-articular nature and potential for growth disturbances. Management of lateral condyle fractures depends on the amount of fracture fragment displacement, joint congruency, and presence or lack thereof of rotational deformity. Patients with nondisplaced and minimally displaced fractures can be treated nonsurgically, whereas those with displaced fractures (>2 mm) warrant surgical intervention either by closed reduction and p

What factors influence outcomes in olecranon fractures in older adults? A cohort study of operative and nonoperative management.

BACKGROUND: Olecranon fractures occur more commonly in older individuals. For patients aged ≥70 years, operative management is often considered standard of care, although recent evidence has supported nonoperative treatment in frail and/or elderly patients. With evolving treatment indications, more granular evidence is beneficial to guide patient-specific decision-making. The aim of this investigation was to explore patient and treatment factors associated with outcomes after displaced, closed olecranon fractures in older individuals. METHODS: This retrospective cohort study with prospective

Increasing complexity and plateauing outcomes in AO type C distal humerus fractures: a 50-year institutional case series.

BACKGROUND: Distal humerus fractures in adults are rare but complex injuries, particularly AO type C3 fractures, often involving comminution, instability, and osteoporotic bone. Demographic trends indicate rising incidence, especially among elderly patients, with increasing functional demands challenging modern osteosynthesis. Open reduction and internal fixation (ORIF) with bicolumnar plating remains the preferred reconstructive approach, although complications remain substantial. This study aimed to evaluate trends in fracture complexity, patient demographics, and functional outcomes over fi

Pediatric Medial Epicondyle Fractures: To Fix or Not To Fix?

Pediatric medial epicondyle fractures are common elbow injuries that represent an avulsion fracture of the apophysis. These fractures are commonly associated with elbow dislocation, and as such, acknowledging the soft-tissue injury and resultant potential elbow instability is important. Treatment decision making is controversial because few absolute indications for surgery exist, and no true displacement cutoffs have been settled despite plentiful research. Although fracture incarceration in the joint, open fractures, ulnar nerve entrapment, and frank elbow instability are indications for surg

Effectiveness of intramedullary nailing vs. locked plating (open reduction and internal fixation) in adult displaced proximal humerus fractures: a systematic review and meta-analysis.

BACKGROUND: Proximal humerus fractures (PHFs) are becoming more common, especially among older adults, and often require surgical management. Two widely used surgical techniques are intramedullary nailing (IMN) and locking plate fixation (open reduction and internal fixation), yet there is ongoing debate regarding their comparative effectiveness. This study aims to systematically evaluate and compare functional, clinical, and complication-related outcomes of IMN and plating for adult patients with displaced PHFs. METHODS: A systematic review and meta-analysis was conducted in accordance with

The Efficacy of Hook Plates Around the Elbow: A Stable Fixation.

Hook plates are low-profile implants commonly used for olecranon fractures and osteotomies, offering stable fixation especially in complex or osteoporotic fractures. This study evaluates the clinical and functional outcomes of hook plate fixation around the elbow. We retrospectively analyzed 20 patients (mean age 52.7 years) treated between 2016 and 2023 for olecranon or distal humerus fractures using hook plates alone or combined with other implants. Inclusion criteria were patients aged 16 years or older with at least 12 months of follow-up. Exclusion criteria included ASA ≥ IV, open fractur

Return-to-Work and Clinical Outcomes After Anterior Latissimus Dorsi and Teres Major Tendon Transfer for Irreparable Subscapularis Tears.

BACKGROUND: Irreparable subscapularis tears represent a significant clinical challenge, especially in active patients aiming to preserve shoulder function. Anterior latissimus dorsi and teres major (LDTM) tendon transfer has been proposed to restore internal rotation and anterior shoulder stability; however, data on return-to-work (RTW) outcomes remain limited. PURPOSE/HYPOTHESIS: The study aimed to evaluate clinical outcomes and RTW rates after anterior LDTM transfer in patients with irreparable subscapularis tears, with the hypothesis that the procedure would provide favorable clinical resu

Complete Recovery of Iatrogenic Radial Nerve Palsy in a Child: A Case Report.

Supracondylar humerus fractures are among the most common paediatric injuries. Although neurologic complications are often seen in these fractures, iatrogenic nerve injuries following closed reduction and percutaneous pinning remain relatively uncommon. Radial nerve palsy is particularly rare in this context, and most cases represent neuropraxic injuries that recover spontaneously. This report describes a rare case of iatrogenic high radial nerve palsy following closed reduction and K-wire fixation of a comminuted supracondylar humerus fracture in a child, with complete functional recovery und

Outcomes of intra-articular distal humerus open reduction and internal fixation based on severity of articular comminution.

BACKGROUND: The advent of modern, precontoured locking plates has improved treatment of distal humerus fractures (DHFs) by providing an anatomic fit and a reliable locking mechanism with interdigitating screws. However, severe articular comminution may predispose surgical failure despite anatomic reduction and a biomechanically strong construct. METHODS: A retrospective review was performed to identify patients who underwent distal humerus open reduction internal fixation (ORIF) between 2015 and 2020. Fractures were classified using the AO/OTA classification and further categorized by anatomi

Bouldering-related trauma: Injury patterns and operative burden over 10 years at a UK major trauma centre.

INTRODUCTION: Bouldering is a rapidly growing discipline within climbing, characterised by short ascents performed without ropes, typically at heights of up to several metres above protective crash mats. While overall injury burden among climbers has been described in the literature, detailed analyses of severe orthopaedic trauma presenting to major trauma centres remain limited. As participation continues to increase, particularly within indoor climbing facilities, a clearer understanding of injury patterns associated with bouldering is needed. This study aimed to characterise the anatomical

Medial Epicondyle Fractures Treated with Diverse Fixation Techniques: A Case Series.

INTRODUCTION: Medial epicondyle fractures of the humerus, account for ~12-20% of pediatric elbow fractures and are often associated with elbow dislocations. Management is debated, especially for moderately displaced injuries. Conventionally, displaced fractures were sometimes treated non-operatively, yielding acceptable function but high rates of fibrous non-union. Recent evidence shows surgical fixation provides ~9-fold higher union rates without significant differences in long-term pain or nerve function. We present five patients treated with different fixation techniques to highlight manage

Suture Bridge Fixation for Ulnar Sublime Tubercle Avulsion Fractures in High School Baseball Players: A Report of Two Cases.

Ulnar sublime tubercle avulsion fractures caused by repetitive valgus stress are rare injuries predominantly observed in adolescent overhead throwing athletes. Although conservative management is typically the first-line treatment, surgical intervention may be required in cases of nonunion, displaced fragments, or persistent medial elbow pain. We report two high school baseball pitchers with symptomatic ulnar sublime tubercle avulsion fractures that were unresponsive to conservative treatment and were surgically treated using a suture bridge fixation technique. In both cases, the small, avulse
