UNLABELLED: An 89-year-old man presented with right elbow pain after a low-energy fall. At age 2 years, he sustained an untreated distal humeral fracture dislocation, which resulted in progressive remodeling and formation of a functional pseudoarticulation. Despite permanent deformity, he maintained stable functional motion for decades and performed heavy manual labor. Imaging revealed a displaced distal humeral fracture through the chronically remodeled elbow. Open reduction and internal fixation with a variable-angle locking plate was performed, while the pseudoarticulation was intentionally

INTRODUCTION: High-velocity gunshot-related humeral fractures are complex injuries often associated with a high rate of nonunion, infection, and poor functional outcome. In low- and middle-income countries these injuries are further complicated by delayed presentation and implant unavailability. Staged management with spanning external fixation followed by internal fixation with intramedullary nail or plate has been well described in the literature; however, the use of a circular frame as a definitive treatment has been very seldom mentioned.
MATERIALS AND METHODS: Six high-velocity gunshot-r

BACKGROUND: Post-operative elbow stiffness is a common and debilitating complication following open reduction and internal fixation for displaced radial head fractures. The relative contributions of post-operative immobilization and intrinsic soft tissue damage remain debated. The purpose of this pilot study was to generate preliminary data on this relationship to inform the design of future, definitive research.
METHODS: This study retrospectively reviewed 45 patients who underwent open reduction and internal fixation for displaced radial head fractures. Pre-operative magnetic resonance imag

INTRODUCTION: Proximal humerus fractures account for approximately 4-6% of all fractures and are among the most common fractures in the elderly. Surgical treatment is increasingly applied for displaced and unstable fracture patterns. The two most commonly used techniques are open reduction and internal fixation (ORIF) via a deltopectoral approach and minimally invasive plate osteosynthesis (MIPO) using a deltoid-split or anterolateral approach. ORIF allows direct fracture visualization and anatomical reduction but is associated with more extensive surgical exposure. MIPO aims to preserve soft

BACKGROUND: As achieving stable fixation in unstable far-lateral distal clavicle fractures is challenging, the optimal surgical treatment remains controversial. The aim of this study was to compare the clinical and radiological results of coracoclavicular fixation (CCF) versus anatomical locking plate fixation (ALPF) in patients with Cho type IIC fractures.
METHODS: Thirty-eight patients who underwent surgical treatment for Cho type IIC fracture were included. Open CCF using a suture button device was performed in 25 patients and ALPF was performed in 13 patients. The mean follow-up period wa

Post-traumatic elbow stiffness is a common complication limiting range of motion and function. Open surgical release is invasive and may not improve patient outcomes. Manipulation under anesthesia (MUA) is a less invasive way to break adhesions and restore motion, though high-level comparative evidence is lacking. This study evaluated whether early MUA plus rehabilitation improves elbow motion, pain, and function compared to rehabilitation alone in patients with postoperative elbow stiffness. This retrospective propensity score-matched cohort study included 240 post-ORIF elbow patients (120 re

Background/Objectives: Falls and upper limb injuries (ULI) are prevalent in older adults, yet whether ULI independently predisposes to subsequent falls remains poorly characterized. This nationwide cohort study evaluated the association between ULI and future fall risk using Taiwan's National Health Insurance Research Database (2011-2019, follow-up through 2020). Methods: Adults aged ≥ 50 years with newly diagnosed ULI-defined as fractures (clavicle, scapula, humerus, radius, ulna, hand), sprains, strains, or open wounds of the shoulder, arm, elbow, forearm, wrist, or hand-were propensity scor

PURPOSES: Open elbow arthrolysis (OEA) is a common method for treating post-traumatic elbow stiffness, which may lead to severe bleeding when resecting heterotopic ossification (HO) and fibrous scar tissue. The purpose of this study is to compare the efficacy and safety among three different ways of using tranexamic acid (TXA) in reducing total blood loss and drainage volume in OEA.
PATIENTS AND METHODS: This was a pilot, double-blinded, randomized controlled trial involving a total of 60 patients of post-traumatic elbow stiffness. Patients in the IV group (n = 20) received TXA intravenously,

Pediatric musculoskeletal trauma differs significantly from adult injuries due to the dynamic and evolving nature of the growing skeleton. The presence of open growth plates (physes), secondary ossification centers, and a more elastic bone structure contributes to unique injury patterns and imaging findings in children. Proper interpretation of pediatric imaging requires an in-depth understanding of age-specific anatomy, biomechanics, and normal developmental variants. This review provides a comprehensive overview of pediatric bone physiology and fracture types, including the Salter-Harris cla

BACKGROUND: Proximal humeral fractures (PHFs) are common in the elderly population, and the use of reverse total shoulder arthroplasty (rTSA) as treatment has risen substantially in recent years. As opposed to statistical significance, clinical value has increasingly been used to evaluate outcomes; thus, this study aims to define patient acceptable symptom state (PASS) thresholds, Minimal Clinically Important Difference (MCID), and substantial clinical benefit (SCB) values for American Shoulder and Elbow Surgeons Standardized Shoulder Assessment Form (ASES) score, Constant-Murley score (CMS),

Olecranon nonunion is an uncommon complication of proximal ulna fractures that can cause pain, deformity, and functional limitation. It usually arises from inadequate fixation, comminution, or impaired healing biology. Timely surgical intervention is often required to restore elbow function. A 34-year-old female, right-hand dominant homemaker presented with persistent deformity, weakness in extension, and difficulty performing overhead activities 4 months after a conservatively managed right elbow injury. Clinical examination revealed a palpable olecranon defect, and radiographs confirmed an a

INTRODUCTION: Coronal shear fractures of the distal humerus are rare, comprising <1% of elbow fractures. McKee Type IV fractures involve both the capitellum and lateral trochlea. These injuries are uncommon and may result in stiffness and functional impairment if diagnosis is delayed or treated conservatively.
CASE REPORT: A 15-year-old boy presented 6 months after an untreated McKee Type IV coronal shear fracture of the distal humerus with pain, stiffness, and restricted elbow motion. Radiographs and computed tomography demonstrated a malunited shear fragment with a "double-arc" sign. Osteot

The upper limb experiences postoperative stiffness as a frequent disabling complication that restricts range of motion, extends recovery time for work and sports activities and diminishes patient contentment. The condition develops from an intensified wound-healing response which primarily involves inflammation and fibrosis. The clinical presentation of joint stiffness manifests as frozen shoulder (adhesive capsulitis) with capsular thickening and rotator-interval/coracohumeral ligament contracture but elbow contracture includes both capsular fibrosis and heterotopic ossification (HO). The pre

Background Tennis is a physically demanding sport characterized by explosive movements, repeated trunk rotations, and asymmetric loading of the dominant upper limb. Overuse injuries are common, with dominant-arm elbow pain (DEP) and low back pain (LBP) representing frequent musculoskeletal complaints among tennis players. Biomechanical differences between the one-handed backhand (1HB) and the two-handed backhand (2HB) may influence the distribution of mechanical loads on the upper limb and spine; however, their relationship with clinically relevant symptoms remains unclear. This cross-sectiona

BACKGROUND: Clavicle fractures are common injuries. As displaced or multifragmentary midshaft patterns carry a substantial risk of malunion and nonunion with nonoperative treatment, they have contributed to the increasing use of surgical fixation. Modern operative techniques include plate fixation, intramedullary devices, and hybrid strategies to address comminution and improve fragment stability. Supplementary interfragmentary screws and cerclage constructs have been proposed to enhance fragment control and optimize load transfer in multifragmentary fracture patterns. This study aimed to comp

This review details an open reduction internal fixation technique of a comminuted, intraarticular Y-type bicolumnar distal humerus fracture using supplemental minifragment plating in addition to dual locking plates. Owing to significant articular comminution, an olecranon osteotomy with an anconeus pedicle flap was used to facilitate exposure to achieve an accurate articular reduction to restore elbow function.

INTRODUCTION: Anterior shoulder instability is a common condition, frequently seen in young and active individuals. For patients with critical glenoid bone defects (≥15%), Bankart repair has limited effectiveness and bony augmentation surgery is often required. However, traditional bone grafts carry the risk of graft resorption, while 3D-printed personalised implants aimed at anatomical reconstruction face limitations in accessibility, cost and production time.
METHODS AND ANALYSIS: In this multicentre, randomised controlled trial, anterior shoulder instability patients with critical glenoid
