The Pediatric Open Supracondylar Fracture: Associated Injuries and Surgical Management.

BACKGROUND: While supracondylar (SC) fractures are relatively common in children, the incidence of open injuries is believed to be only 1%. Two prior studies on open SC fractures in children reported an increased incidence of vascular injuries. The purposes of our study were to clarify the incidence, associated conditions, and current treatment for open SC fractures. METHODS: The Pennsylvania Trauma Outcome Study database was queried. Subjects age 25 to 156 months old admitted to trauma centers between January 2000 and December 2015 with a SC fracture were included. Controls were those with c

Fracture profiles of a 4-year cohort of 266,324 first incident upper extremity fractures from population health data in Ontario.

BACKGROUND: Understanding the profiles of different upper extremity fractures, particularly those presenting as a 1st incident can inform prevention and management strategies. The purpose of this population-level study was to describe first incident fractures of the upper extremity in terms of fracture characteristics and demographics. METHODS: Cases with a first adult upper extremity (UE) fracture from the years 2013 to 2017 were extracted from administrative data in Ontario. Fracture locations (ICD-10 codes) and associated characteristics (open/closed, associated hospitalization within 1-da

Risk Factors of Elbow Stiffness After Open Reduction and Internal Fixation of the Terrible Triad of the Elbow Joint.

OBJECTIVE: To analyze the risk factors of elbow stiffness following open reduction and internal fixation of the terrible triad of the elbow joint. METHODS: A retrospective study was conducted of 100 patients with the terrible triad of the elbow joint, who had been treated at our hospital from January 2015 to December 2018. All patients were treated with a loop plate to repair the ulnar coronoid process. According to the severity of the injury, the radial head was either fixed or replaced, and the lateral collateral ligament was repaired with an anchor. According to the range of motion of the

Reconstruction of the Weightbearing Area of the Foot with Vascularized Chimeric Osteocutaneous Fibular Flap: A Case Report.

Damage to the weightbearing surface of the foot is a challenge for the reconstructive surgeon. The aim is to reconstruct the skeletal tripod and soft tissue, allowing the patient to walk normally. We report the case of a patient admitted with an acute right foot open fracture of the second, third, fourth, and fifth metatarsal bones. After debridement of all nonvital tissues, the patient required reconstruction of the metatarsal heads (third, fourth, and fifth) plus soft tissue coverage. We then performed a reconstruction with a free osteocutaneous fibular flap, insetting the bone perpendicular

Long Segment, Mixed Sensorimotor Nerve Reconstruction with Allograft: A Case Report of High Radial Nerve Injury.

CASE: A 35-year-old man sustained a Grade I open right mid-diaphyseal humerus fracture and high radial nerve transection from a motor vehicle accident. Acute humeral plate osteosynthesis and radial nerve reconstruction using an intercalary 4-cm processed nerve allograft (PNA) was performed. Five years postoperatively, elbow extension, forearm supination, and wrist extension were 5/5 strength and independent digital extension was 5-/5. Radial nerve sensation recovered to 90% of the uninjured side. CONCLUSIONS: Use of PNA resulted in outcomes comparable with or superior to autograft reconstruct

Risk factors for developing acute compartment syndrome in the pediatric population: a systematic review and meta-analysis.

PURPOSE: Acute compartment syndrome (ACS) is often difficult to diagnose in pediatric patients due to their erratic symptomatology. Therefore, it is of paramount importance to identify at-risk patients to facilitate a prompt diagnosis. This study aims to identify risk factors for the development of ACS in the pediatric population. METHODS: We included studies comprised of pediatric patients with traumatic ACS. We excluded studies evaluating compartment syndrome secondary to exertion, vascular insult, abdominal processes, burns, and snake bites. Heterogeneity was addressed by subgroup analysis

Total Scapholunate Extrusion Into the Forearm in a Transscaphoid, Transcapitate Perilunate Fracture Dislocation: A Case Report.

CASE: A 50-year-old woman involved in a motorcycle accident sustained a transscaphoid transcapitate perilunate fracture dislocation with complete extrusion of the proximal scaphoid pole and lunate as a unit into the forearm. The patient underwent urgent open reduction and internal fixation (ORIF) with screw fixation of the capitate and Kirschner wire fixation of the scaphoid, midcarpal joint, and lunotriquetral intervals. At 1-year postoperatively, radiographs confirm healing of capitate and scaphoid fractures, with no signs of avascular necrosis of the lunate. CONCLUSION: Despite a complex f

Complex open elbow fracture Gustilo-Anderson type IIIB treated with the primary elbow arthroplasty: A case report.

Total elbow arthroplasty as a treatment option for open elbow fracture is relatively rare described. We reported a 39 years old polytrauma patient with complex open elbow fracture (Gustilo-Anderson type IIIB). The patient presented with large soft tissues defect on dorsal part of the left elbow, ulnar palsy due to the irreparable loss of the ulnar nerve, distal triceps loss due to the complete loss of the olecranon, loss of both humeral condyles with collateral ligaments and complex elbow instability. Only few similar cases have been published. Reconstructive surgery included repetitive radica

Multiple Unilateral Upper Limb Fractures in the Pediatric Setting - A Case Report.

INTRODUCTION: Lateral condyle fractures and both bone forearm fractures account for 10-15% and 13-40% of all elbow fractures in children, respectively. About 5% of forearm fractures in children have associated supracondylar fractures; hence, any forearm fractures warrant a careful examination of the elbow, and any radiographs taken should visualize the elbow joint above and wrist joint below for other injuries. We report a case of multiple upper limb fracture in a child, comprising lateral condyle and both bone forearm fractures. CASE REPORT: A 5-year-old boy was admitted through the emergenc

Thorough Debridement Facilitated by Timely Planning of One-Bone Forearm Surgery: A Report of 2 Cases.

CASES: Two patients with complex open forearm fractures underwent initial treatment with radioulnar fusion (arthrodesis of the radius to the ulna)-one patient sustained elbow and forearm injuries in a car accident and the other had an elbow injury caused by conveyor belt entanglement. CONCLUSIONS: Differentiating viable tissue from nonviable tissue is important in the management of potentially contaminated wounds. Arthrodesis of the radius to the ulna is generally considered a salvage option for complicated forearm instability. The decision to perform arthrodesis of the radius and ulna in the

Vascularised fibular graft in the management of non-union of fracture shaft of radius: a less ventured entity.

INTRODUCTION: Non-union of the radius and ulna is a major complication of forearm fractures, accounting upto 10% of all forearm fractures. Multiple modalities are available for the treatment of non-union. Vascular grafts are a less sought-after surgical choice owing to the need of expertise and skills of surgeons. We discuss a case of gap non-union of fracture shaft radius treated with vascular fibula graft. CASE REPORT: We describe a case of 45yr old lady with closed fracture of both bones of left forearm. She underwent open reduction and internal fixation with 3.5 small DCP (6 hole) two days

Necrotizing fasciitis with mediastinitis following snakebite envenomation.

Snakebite envenomation is a life-threatening injury and a neglected public health issue in Africa. We report the case of a child that presented 6 days following a forearm snakebite with compartment syndrome and necrotizing fasciitis of the upper limb extending to the neck and chest who developed mediastinitis. She underwent multiple surgical debridements and the mediastinitis was managed non-surgically with antibiotics and postural drainage leading to recovery. The wounds were later grafted and the child discharged. Extension of necrotizing fasciitis from the limbs to the chest wall with devel

Damage control orthopedics applied in an 8-year-old child with life-threatening multiple injuries: A CARE-compliant case report.

RATIONALE: Damage control is a staged surgical approach to manage polytraumatized patients. The damage control approach comprises three steps. First, bleeding is controlled and fractures are stabilized temporarily; second, vital parameters are stabilized and the child is rewarmed in the intensive care unit; and third, the child is reoperated for definitive repair of injuries. We aimed to describe the feasibility of the damage control orthopedic approach in a child. PATIENT CONCERNS: An 8-year-old girl fell from the balcony of the 5th floor onto concrete pavement and was admitted to our accide

An Extensive Volar Forearm Laceration - The Spaghetti Wrist: A Systematic Review.

PURPOSE: 'Spaghetti wrist' is an extensive laceration that involves multiple structures in the volar wrist, including tendons, nerves and arteries. This injury is frequently encountered in trauma units, but despite its complex nature, management is often handled by junior surgeons.Furthermore, the guidance on how to approach these injuries is limited, with a relatively poor evidence base on management and outcomes. METHODS: In this article, we perform a systematic review of the literature on the management and outcomes of the spaghetti wrist injury. Patient demographics, definitions of spaghe

Epidemiology of Pediatric Musculoskeletal Injuries and Their Pattern in a Tertiary Care Center of North India.

BACKGROUND: Trauma is emerging as an epidemic and a leading cause of morbidity and mortality in children. Children <15 years of age comprise about 32.8% or about 1/3rd of the total Indian population. In India, up to one fourth of hospital admissions and approximately 15% of deaths in children are due to injury. This study presents the epidemiology, various causes and pattern of musculoskeletal injuries in pediatric population of North India. MATERIALS AND METHODS: This is an observational, prospective hospital-based study conducted in a tertiary care center of North India for 6 months from Ju

Open reduction and internal fixation for Mason type III radial head fractures: Is it different from that for Mason type II fractures?

PURPOSE: The objective of this study was to investigate whether the outcomes of Mason type III radial head fractures (RHFs) treated by open reduction and internal fixation (ORIF) were comparable to those of Mason type II RHFs treated by ORIF. METHODS: A total of 87 surgically treated RHF patients were reviewed. Their fractures were Mason type II in 39 patients (all treated by ORIF) and Mason type III in 48 patients (40 treated by ORIF, 7 by radial head arthroplasty, and 1 by resection). Although ORIF was preferred for Mason type III RHFs in our series, an arthroplasty was performed when the f

Free Flap Transfer with Arteriovenous Loop Establishment for Upper Limb Salvage in a Crush Injury.

Constructing arteriovenous (AV) loops saves a crushed limb when healthy recipient vessels are unavailable at the site of injury by supporting a transferred free flap. Literature about using AV loops in the upper limb defect is scarce. We present a case with a devastating upper limb crush injury that was successfully managed with fibula flap transfer preceded by AV loop construction. A 38-year-old woman was admitted to our hospital with severe crushing trauma to her right forearm and hand. There was significant skin loss with injuries to blood vessels and tendons. She underwent multiple staged

Basic principles of fracture treatment in children.

This review aims to summarize the basic treatment principles of fractures according to their types and general management principles of special conditions including physeal fractures, multiple fractures, open fractures, and pathologic fractures in children. Definition of the fracture is needed for better understanding the injury mechanism, planning a proper treatment strategy, and estimating the prognosis. As the healing process is less complicated, remodeling capacity is higher and non-union is rare, the fractures in children are commonly treated by non-surgical methods. Surgical treatment is
