CONTEXT: Adults with type 1 diabetes (T1D) have elevated fracture risk, but the effects of T1D on bone accrual during adolescence remain unclear.
OBJECTIVE: To assess group differences in bone microarchitecture and strength between adolescents with type 1 diabetes and controls during sex-specific stages of pubertal bone accrual.
METHODS: In this cross-sectional study at a tertiary medical center, forty youth with T1D and 40 matched controls were studied during early-pubertal (Tanner 2-3, girls) and mid-pubertal (Tanner 3-4, boys) stages of bone accrual. Assessments included dual-energy X-ray

BACKGROUND: Titanium elastic nailing (TEN) is widely used for the treatment of unstable pediatric forearm fractures. Although subcutaneous nail tips remain the standard technique, superficial (exposed) nail tip placement has been described as an alternative strategy to facilitate implant removal. However, evidence regarding complication profiles associated with different TEN tip management techniques remains limited, particularly with respect to age-related differences. This study addresses three critical gaps: (1) comparative complication rates between superficial and subcutaneous TEN placeme

BACKGROUND: End-stage renal disease (ESRD) requiring hemodialysis (HD) disproportionately affects older adults, who face increased risks of falls and fractures due to age-related changes, renal osteodystrophy, comorbidities and dialysis-related factors. Although bone abnormalities and fall risk have often been examined separately in patients with ESRD, few studies have evaluated vertebral compression fractures (VCF) in older HD patients while concurrently assessing skeletal, functional and geriatric risk factors. The role of Comprehensive Geriatric Assessment (CGA) in identifying multidimensio

BACKGROUND: Nonunion and malunion of upper extremity fractures are uncommon but challenging complications, particularly when associated with infection, deformity, or multiple previous operations. Although internal fixation remains the standard treatment, external fixation based on Ilizarov principles offers advantages in complex situations including infection control, deformity correction, and gradual bone reconstruction. This study evaluates the indications, surgical technique, and outcomes of external fixation as a definitive treatment for nonunion and malunion of the upper extremity.

UNLABELLED: Type 2 diabetes (T2D) and glucocorticoid treatment both increase risk of bone fractures. We found that glucocorticoid use increases fracture risk similarly regardless of T2D status. Findings were consistent over time and across subgroups. Care should be taken when prescribing glucocorticoids to individuals with T2D already at increased fracture risk.
BACKGROUND: Type 2 diabetes (T2D) is associated with increased risk of bone fractures, which may be caused by suppressed bone formation. Similarly, glucocorticoid (GC) treatment leads to suppressed bone formation, in addition to incre

PURPOSE OF REVIEW: Distal radius fractures are common fragility fractures in women, but exercise-related evidence has focused mainly on the hip and spine. This review and meta-analysis examined whether exercise improves upper-limb bone outcomes and whether exercise modality and intensity influence the response.
RECENT FINDINGS: Twenty-six controlled trials involving 1,880 women were included. The pooled estimate favored exercise with a standardized mean difference (SMD) of 1.05; with a 95% confidence interval (CI) from 0.68 to 1.43; p < 0.001), representing a large positive average effect. Th

Stress fractures of the forearm are uncommon, and to our knowledge, sequential involvement of both the ulna and radius has not been well characterized. Neurofibromatosis type 1 is associated with increased bone fragility and delayed fracture healing due to impaired osteoblastic differentiation, reduced mineralization capacity, and increased osteoclastic activity. We report the case of a woman in her early 60s with neurofibromatosis type 1 and lower-extremity weakness who relied on upper-extremity weight-bearing ambulation. She initially developed a nondisplaced transverse stress fracture of th

Background: Photon-counting detector CT (PCCT) combines high-resolution anatomical and spectral imaging by directly counting individual x-ray photons and discriminating their energies. Its role in surgical planning in complex multi-trauma with concurrent pulmonary embolism remains undefined. Case Presentation: A 58-year-old previously healthy woman was entrapped by a cement mixer, sustaining traumatic scalp avulsion (20 × 20 cm) with exposed skull, depressed right parietal fractures, subdural and epidural hematoma, right 3rd-9th rib fractures, mid-shaft radius and ulna fractures with tendon an

Tetanus is a serious infectious disease characterized by muscle rigidity and autonomic storm. In severe cases, long-term deep sedation is required, resulting in a high risk of post-intensive care syndrome (PICS). However, criteria for determining the timing of early rehabilitation intervention have not yet been established. We report the case of a woman in her 70s who developed tetanus following a wood-splinter injury to her right forearm and subsequently suffered bilateral humeral fractures due to muscle rigidity and convulsions. She was admitted to the intensive care unit (ICU) with an Ablet

Emerging evidence highlights the potential of bioactive compounds, particularly polyphenols, as adjunctive therapeutic agents in the treatment of pancreatic cancer (PC), one of the most aggressive malignancies. This review focuses on epigallocatechin gallate (EGCG) and resveratrol due to their extensively documented anticancer activity, favorable safety profiles, and their unique ability to modulate multiple signaling pathways relevant to pancreatic tumorigenesis. Among polyphenols, these two have shown superior anti-cancer activity, epigenetic regulatory effects, and synergy with standard che

PURPOSE: The absence of evidence based general guidelines for radiographic follow-ups of pediatric diaphyseal forearm fractures treated with ESIN results in an arbitrary array of X-ray examinations. In most pediatric traumatology departments, an X-ray check is carried out 4 weeks after ESIN osteosynthesis of forearm shaft fractures to detect incipient consolidation of the fracture. However, the elevated sensitivity to ionizing radiation requires special precautions in the pediatric populations. To prevent the indiscriminate use of this diagnostic tool it is necessary to assess the consequence

Pediatric forearm fractures of the radius and ulna are common. Previous literature suggests that conservatively managed mid-shaft radius and ulna forearm fractures have a remanipulation rate of 10-70%. The purpose of this study is to compare the re-displacement rate of closed displaced mid-shaft both-bone forearm fractures (BBFF) in children treated with closed reduction and casting with a loop and sling applied proximal to the fracture site (Rang method) compared with a standard sling. A retrospective review was performed of 42 patients under the age of 14 with BBFF treated over a 4-year peri

Osteogenesis imperfecta (OI) is a rare inherited connective tissue disorder. It is characterized by short stature, fragility and decreased bone mass, which leads to multiple and recurrent fractures after low-energy trauma, which generates susceptibility to long bone deformity and vertebral compression. There are several types of OI, with types I to IV, in which the COL1A1 and COL1A2 genes are affected, being the most frequent. In recent years, the discovery of new forms of OI has led to research into the pathways critical aspects of bone metabolism, with new genes involved being identified. Th

Shaft fractures in children and adolescents differ from those of adulthood in terms of the fracture pattern and treatment. With respect to the procedure, humeral shaft fractures must be differentiated from forearm shaft fractures. Almost no axial deviation can be tolerated on the forearm shaft, even at a young age as this could lead to functional restrictions in the rotation of the forearm. For these reasons, closed reduction and elastic stable intramedullary nailing (ESIN) nowadays represents the gold standard for dislocated or unstable fractures. A special type is the greenstick fracture of
