The Novel Use of a Custom Three-Dimensional-Printed Polycaprolactone-Tricalcium Phosphate Synthetic Bone Scaffold (Osteopore®) in the Management of a Critical-Sized Bone Defect for a Patient with an Open Intra-Articular Calcaneal Fracture - A Case Report.

INTRODUCTION: Intra-articular calcaneal fractures are generally treated with open reduction and internal fixation in order to restore calcaneal anatomy as well as subtalar and calcaneocuboid joint congruency. Bone loss is common due to the impaction of cancellous bone beneath the posterior facet as a result of axial loading. Critical-sized bone defects are commonly addressed with the Masquelet "induced membrane technique" incorporating autogenous or allogenous bone graft. Synthetic bone scaffolds are readily available and customizable through three-dimensional (3D) printing. This case report d

Single-Stage Press-Fit Osseointegration of the Radius and Ulna for Rehabilitation After Trans-Forearm Amputation.

BACKGROUND: Upper limb (UL) amputation is disabling. ULs are necessary for many domains of life1, and few effective motor and sensory replacements are accessible2. Approximately 41,000 people in the United States have UL amputation proximal to the fingers3, two-thirds of (all) traumatic amputations are UL4, and 80% of UL amputations are performed for trauma-related etiologies5. Socket prosthesis (SP) abandonment remains high because of the lack of sensation, limited prosthesis control, perceived weight, and difficulty comfortably wearing the SP6. Transcutaneous osseointegration7,8 surgically i

Using Next-Generation Sequencing to Understand Infection Prevention in Surgical Treatment of Upper Extremity Fractures-A Prospective Cohort Study.

INTRODUCTION: Postoperative fracture site infection can lead to notable patient morbidity, increase cost of care, and further contribute to healthcare disparities globally. Dogma suggests surgical blades as a vehicle for introducing bacteria into the surgical site; however, there is a paucity of literature to support this claim. This study uses advanced DNA sequencing to detect bacterial DNA on surgical blades used in upper extremity fracture surgeries. METHODS: This was a prospective study, conducted at a high-volume level 1 trauma center. All acute, closed upper extremity fractures requirin

Fibula flap in upper extremity segmental/critical size bone defects fixed with locking plates. Single-institution observational cohort.

INTRODUCTION: Management of bone defects in the upper extremity can vary depending on the size and location of the defect. Large defects may require complex reconstruction techniques. Vascularized bone grafts, mainly free vascularized fibula flap (FVFF), have many advantages in the treatment of bone or osteocutaneous defects. However, complications such as graft fracture are the most common, with a free fibula flap for bone defects in the upper extremity. This study aimed to describe the results and complications associated with the treatment of posttraumatic bone defects in the upper extremit

Interposition Arthroplasty in an Acute Setting to Treat Unexpected Denuded Distal Humerus Articular Cartilage After AO 13C2.2 Distal Humerus Fracture, Surgical Technique, and a Case Report.

Restoring elbow joint motion is paramount for upper extremity optimum function. In end-stage elbow disease and stiffness, total elbow arthroplasty is the recommended option for older patients; however, for younger, highly demanding patients, interposition arthroplasty (IPA) is the management option of choice. We report a case of an 16-year-old female patient who presented after she had an open-grade IIIA, type AO 13C2.2 distal humerus fracture, which was managed initially by debridement and a cross-elbow external fixation. The decision was made to manage the fracture by open reduction and inte

Rare triad of fractures in the upper limb. Simultaneous fractures of the distal radius, with the radial head and the ulna styloid.

We're reporting a 35-year-old gentleman that presented with a rare combination of ipsilateral fracture of both proximal and distal ends of the radius associated with ulna styloid fracture due to a fall with outstretched hands. Thorough clinical examination of 2 adjacent joints (elbow and wrist) is critical in identifying such an unusual or rare combination of injuries. Appropriate management for such cases depends on the characteristics of the injury, such as fracture patterns, job, hand dominance, and age of the patient. The treatment should aim to preserve the radial head to avoid the possib

Hand laterality recognition in distal radius and/or ulna fracture.

INTRODUCTION: Hand laterality, an important ability to determine the orientation of a limb is common to get affected after short term immobilization. Distal radius and/or ulna fracture is a commonly encountered fracture resulting from upper-limb trauma. Conservative treatment using closed reduction and plaster cast application to immobilize the joint remains choice of treatment over surgery in the treatment of these fractures. There is a paucity of literature reporting impairment in hand laterality after long term immobilization as commonly performed in patients with distal radius and/or ulna

Minimally invasive treatment of forearm double fracture in adult using Acumed forearm intramedullary nail: A case report.

BACKGROUND: Currently, open reduction internal fixation is the conventional surgical method for treatment of double ulna and radius fracture. However, open reduction is associated with a high risk of complications. This case of forearm double fracture involved a patient treated using an Acumed intramedullary nail. The patient experienced good follow-up outcomes. The Acumed forearm intramedullary nail enables early functional exercise and hastens healing of the fracture. Few studies have reported on the use of this approach for the treatment of fractures. CASE SUMMARY: A 23-year-old male patie

Elastic Intramedullary Nailing of Pediatric Both-Bone Forearm Fractures.

BACKGROUND: Elastic intramedullary nailing of both-bone pediatric forearm fractures is used to treat unstable fractures that fail conservative management with closed reduction and casting, open injuries, or injuries with neurovascular compromise. DESCRIPTION: Small incisions are used to enter the medullary canal of the distal end of the radius and proximal part of the ulna, avoiding injury to the adjacent physis. The elastic nail is advanced to the fracture site under fluoroscopic guidance. The fracture is reduced with traction and manipulation, and the nail is passed across the fracture site

Use of Compressive Osseointegration Endoprostheses for Massive Bone Loss From Tumor and Failed Arthroplasty: A Viable Option in the Upper Extremity.

BACKGROUND: Endoprostheses using principles of compressive osseointegration have shown good survivorship in several studies involving the lower extremity; however, no series to our knowledge have documented the use of this technology in the management of massive bone loss in the upper limb. QUESTIONS/PURPOSES: (1) What proportion of upper extremity implants using compressive osseointegration fixation principles achieved durable short-term fixation, and what were the modes of failure? (2) What surgical complications resulted from reconstruction using this technique? METHODS: A multiinstitutio

Comparing clinical and economic outcomes of biologic and conventional medications in postmenopausal women with osteoporosis.

RATIONALE, AIMS AND OBJECTIVES: Biologics are substantially more expensive than their conventional counterparts but it is unclear whether extra costs deliver better health outcomes. We compare clinical and economic outcomes between teriparatide (monthly costs $1120) and bisphosphonates (monthly costs $14) among postmenopausal women with osteoporosis. METHODS: From a 5% random sample of Medicare beneficiaries, we selected women newly diagnosed with osteoporosis between 1 January 2007 and 31 December 2011 and who initiated teriparatide or bisphosphonates after the diagnosis. We followed them up

Complications of hinged external fixation compared with cross-pinning of the elbow for acute and subacute instability.

BACKGROUND: Elbows that are unstable after injury or reconstructive surgery often are stabilized using external fixation or cross-pinning of the joint supplemented by cast immobilization. The superiority of one approach or the other remains a matter of debate. QUESTIONS/PURPOSES: We compared patients treated with external fixation or cross-pinning in terms of (1) adverse events, (2) Broberg and Morrey scores, and (3) ROM. METHODS: Between 1998 and 2010, 19 patients (19 elbows) had hinged external fixation and 10 patients (11 elbows) cross-pinning and casting for subacute or acute posttraumat

Developments in the treatment of pediatric long bone shaft fractures.

Shaft fractures of the long bones are among the most common pediatric injuries. They are characterized by a limited potential for spontaneous correction because of the fractures--distance to the physis. Although conservative treatment has been practiced for many years with satisfying results, several aspects have led to an increase in the numbers of surgical procedures including changes of living, sports habits, economics, and patients-request to treatment. We reviewed the literature to describe developments of treatment concepts of pediatric shaft fractures. In shaft fractures of the upper ex

Long-term results after ulnar collateral ligament reconstruction of the elbow in European athletes with interference screw technique and triceps fascia autograft.

BACKGROUND: In the last decade, there has been increasing interest in medial ulnar collateral ligament (MUCL) reconstruction techniques for MUCL insufficiency of the elbow. All case series are based on American and Asian Athletes and use primarily a palmaris longus tendon or gracilis tendon as an autograft in reconstructions. A new technique is the interference screw fixation. Evidence that supports the use of this technique is mainly from controlled laboratory studies. The purpose was to evaluate the interference screw technique for MUCL reconstructions in a European, clinical setting, with a

Vanishing bone disease (Gorham's disease) - a rare occurrence of unknown etiology.

A 20-year-old male patient presented with painful swelling around left elbow joint. Radiographic examination revealed osteolytic lesion with pathological fracture of lower end of humerus and upper radius. Upper end of ulna was completely absent along with bony erosion. Histopathology of the bony tissue revealed hemangioma-like lesion composed of vascular channels lined by benign endothelium replacing bone. The diagnosis of Gorham's massive osteolysis was made. Gorham's disease is a benign self-limiting condition affecting any age, may involve any part of the skeleton and is characterized by re

Is distal ulna resection influential on outcomes of distal radius malunion corrective osteotomies?

INTRODUCTION: The mechanical repercussions of distal radius malunion on the distal radio-ulnar (DRU) joint are common and inconsistently corrected by radius osteotomy alone. Ulnar resection has thus become a palliative solution. HYPOTHESES: Does ulna resection influence the outcomes of distal radius malunion corrective osteotomies? What preoperative factors warrant preserving the distal radio-ulnar joint? PATIENTS AND METHODS: Twenty-one corrective osteotomies of the radius were retrospectively reviewed. Ulna resection was performed in cases of cartilage damage, joint incongruence, or persis
