Developments in the treatment of pediatric long bone shaft fractures.

Shaft fractures of the long bones are among the most common pediatric injuries. They are characterized by a limited potential for spontaneous correction because of the fractures--distance to the physis. Although conservative treatment has been practiced for many years with satisfying results, several aspects have led to an increase in the numbers of surgical procedures including changes of living, sports habits, economics, and patients-request to treatment. We reviewed the literature to describe developments of treatment concepts of pediatric shaft fractures. In shaft fractures of the upper ex

Vanishing bone disease (Gorham's disease) - a rare occurrence of unknown etiology.

A 20-year-old male patient presented with painful swelling around left elbow joint. Radiographic examination revealed osteolytic lesion with pathological fracture of lower end of humerus and upper radius. Upper end of ulna was completely absent along with bony erosion. Histopathology of the bony tissue revealed hemangioma-like lesion composed of vascular channels lined by benign endothelium replacing bone. The diagnosis of Gorham's massive osteolysis was made. Gorham's disease is a benign self-limiting condition affecting any age, may involve any part of the skeleton and is characterized by re

Quantitative measurement of radial head fracture location.

BACKGROUND: The most common location of a displaced fracture of part of the radial head is often described as the anterior lateral aspect of the radial head with the forearm in neutral position, based on observation rather than precise measurements. The purpose of our study was to measure the exact location of fractures involving part of the radial head using quantitative 3-dimensional computed tomography (CT). MATERIALS AND METHODS: We measured the fracture lines with respect to the biceps tuberosity in 24 patients with a displaced articular fracture of part of the radial head (Mason type 2)

Is distal ulna resection influential on outcomes of distal radius malunion corrective osteotomies?

INTRODUCTION: The mechanical repercussions of distal radius malunion on the distal radio-ulnar (DRU) joint are common and inconsistently corrected by radius osteotomy alone. Ulnar resection has thus become a palliative solution. HYPOTHESES: Does ulna resection influence the outcomes of distal radius malunion corrective osteotomies? What preoperative factors warrant preserving the distal radio-ulnar joint? PATIENTS AND METHODS: Twenty-one corrective osteotomies of the radius were retrospectively reviewed. Ulna resection was performed in cases of cartilage damage, joint incongruence, or persis

Dietary calcium and serum 25-hydroxyvitamin D status in relation to BMD among U.S. adults.

A higher calcium intake is still the primary recommendation for the prevention of osteoporosis, whereas vitamin D deficiency is often not addressed. To study the relative importance of dietary calcium intake and serum 25-hydroxyvitamin D [25(OH)D] status in regard to hip BMD, 4958 community-dwelling women and 5003 men >/=20 yr of age from the U.S. NHANES III population-based survey were studied. Calcium supplement users and individuals with a prior radius or hip fracture were excluded. We calculated standardized means for BMD by quartiles of sex-specific calcium intake for three 25(OH)D catego

An intramedullary nail with multifunctional interlocking for all types of fracture in both femurs.

OBJECTIVE: A femoral compound interlocking intramedullary nail (FCIIN) was designed to treat all types of fractures between the trochanter and epicondyle of both femurs. It could substitute for femoral interlocking intramedullary nails (FIIN) at five points. METHODS: According to the morphological characteristics of the femoral medullary canal, the nail is designed to accommodate a 1250 mm radius of radian and a 135° neck-shaft angle. Three interlocking holes of 6.5 mm diameter are located at the proximal end of the FCIIN, making crossing of the screws possible. The hole is designed to be ver

Long-term follow-up of combined fractures of the proximal radius and ulna during childhood.

Forty-five children with combined fractures of the proximal end of the radius and ulna were treated in our orthopedic department over a period of 16 years (1984-1999). The age of the patients ranged from 5 to 12 years (mean 8 years) with a peak of 7-8 years. The mechanism of injury was a fall on the outstretched hand with the elbow extended and with a valgus force being applied to the elbow. In 35 patients, closed reduction or immobilization in plaster, or both, were performed. In the other 10 patients, a surgical treatment was followed either for fractures of the radius (4) or for both the ra

Current concepts in the management of distal radius fractures.

The treatment of fractures at the distal end of the radius continues to challenge orthopaedic and upper extremity surgeons. As our understanding of the injury mechanism and local anatomy continues to improve, so too have our surgical techniques in helping patients regain functional use of the injured extremity. The purpose of this manuscript is to review the treatment methods available for distal radius fracture management.

[Do injuries of the upper extremity in geriatric patients end up in helplessness? A prospective study for the outcome of distal radius and proximal humerus fractures in individuals over 65].

In a prospective study 104 patients >or=65 years with distal radius fractures (DRF; n=52) and proximal forearm fractures (PHF; n=52) were followed up for a period of 4 months after injury. As an inception- cohort study, influence on treatment pattern was not part of the examination. A total of 53% of the DRF and 74% of the PHF patients underwent surgery. There were no significant changes in the ability of daily living management (IADL) with either fracture form. Functional outcome was better in PHF than DRF patients. PHF patients showed a high incidence in "fear of falling" throughout the whol

The effect of radial shortening on wrist joint mechanics in cadaver specimens with inherent differences in ulnar variance.

PURPOSE: To assess load changes in the wrist and forearm resulting from surgically-induced radial shortening in wrists with inherent differences in ulnar variance. METHODS: Eleven fresh cadaver upper extremities, 4 with ulnar-plus variance of 2 mm or greater and 7 with ulnar-minus variance of 2 mm or greater were used. The radius and ulna of each specimen were instrumented with load cells, a 15-mm segment of the radius was resected and replaced with aluminum blocks of various sizes, and the specimens were loaded with 143 N (32 lb) at 1-mm differences of radial length. The load distribution be

Peripheral nerve compression neuropathy after fractures of the distal radius.

Sixty patients with unilateral distal radius fractures were managed conservatively. Clinical assessment included objective and subjective evaluations of the outcome of treatment. Radiographic evaluation included fracture classification using the AO/ASIF system and measurement of volar tilt, radial inclination and radial height shortening at the end of treatment. Clinical signs of carpal tunnel syndrome were confirmed electrodiagnostically in 12 patients (20%) during the recovery period. Each patient had electrodiagnostic studies of both upper limbs performed to confirm the diagnosis. The mean

Palmar tilt changes due to distal radius fractures and radiocarpal instability: a biomechanical study.

OBJECTIVE: To investigate the impact of dorsally angulated malunited distal radius fractures on the stability of the radiocarpal joint. METHODS: Eight fresh upper limb specimens from 4 human cadavers were obtained, in which radial osteotomy was performed to prepare models of Frykman I distal radius fracture. Pressure-sensitive film was applied to study the changes in the contact area distribution and the pressure load across the radiocarpal joint in response to palmar tilt variation, and the lateral radiograph of the wrist was taken at the end of pressure loading. RESULTS: When the dorsal ti

A biomechanical analysis of fixation of intra-articular distal radial fractures with calcium-phosphate bone cement.

BACKGROUND: Calcium phosphate cement has been used to treat unstable fractures of the distal end of the radius with the intent of avoiding the stiffness and morbidity associated with prolonged immobilization in a cast or external fixation. The purpose of this study was to compare the stability of the fracture fragments after fixation with augmented calcium phosphate cement with that after alternative methods of percutaneous fracture treatment. METHODS: Both an osteotomy and osteoclasis were used to create a model of an intra-articular fracture of the distal part of the radius (AO type C2) wit

Brachial artery injury in closed posterior elbow dislocation case report.

The authors describe a case with a closed posterior elbow dislocation associated with a distal radial fracture and complete transsection of the brachial artery. The patient had a pulseless distal upper extremity and immediate gross swelling of the elbow and forearm. As closed reduction was not possible, open reduction had to be performed through an anteromedial approach to the elbow. End-to-end suture of the brachial artery was successful. After fasciotomy and internal fixation of the distal radial fracture, the elbow was stabilized with an external fixator spanning the elbow joint. After two
