Paediatric forearm refractures with flexible nails in-situ: risk factors and management.

To identify risk factors for forearm shaft refractures in paediatric patients treated with flexible nails in situ and explore effective management strategies. From January 2022 to April 2024, paediatric patients with diaphyseal radius and ulna fractures treated with flexible nails were retrospectively reviewed. Patients without refractures were assigned to group A, and those with refractures and flexible nails in situ to group B. Demographics, injury mechanisms, operative details, complications, and time to radiographic union were analyzed. Fifty-six paediatric patients with 105 forearm fractu

Conservative treatment for refracture of the forearm with elastic stable intramedullary nails in situ in an adolescent: A case report.

Elastic stable intramedullary nailing (ESIN) for forearm shaft fractures in children and adolescents is generally associated with favorable outcomes. However, refracture with ESINs in situ is a rare complication. We report a rare case of refracture with implants in situ in a 14-year-old male patient. Six months after the initial surgery, the patient sustained a reinjury while playing soccer. Radiographs confirmed forearm shaft refracture of both the radius and ulna, and both intramedullary nails were bent at the fracture sites but remained unbroken. We performed emergent closed manual reductio

Limb salvage and functional recovery following accidental shotgun injury in a young child: A case report.

BACKGROUND: Paediatric firearm injuries are rare in civilian settings but pose significant challenges due to high-energy trauma, small anatomical compartments, and limited physiological reserve. Shotgun wounds, in particular, produce complex skeletal and soft tissue destruction requiring staged multidisciplinary care. CASE PRESENTATION: We report a 3-year-old girl with an accidental close range shotgun injury to the right forearm, presenting with grossly contaminated wounds, soft tissue loss, and comminuted fractures of the shaft of radius and ulna. Emergent debridement and elbow-spanning ext

Treatment and outcomes of fractures in children: a registry-based cohort study of 10,144 fractures.

BACKGROUND AND PURPOSE: There is no universal consensus on evaluating the quality of pediatric fracture care. While U.S. News ranks hospitals based on surgical timing and procedural benchmarks, additional metrics such as complication rates, reoperations, and patient-reported outcomes could provide a more comprehensive assessment. We aimed to evaluate pediatric fracture care outcomes at a Level I trauma center, emphasizing standardization and transparency in benchmarking. METHODS: This retrospective study reviewed all fractures diagnosed at HUS New Children's Hospital, Helsinki from 2018 to 20

A Case of Nonanatomic Alignment: Acute Radial Head Dislocation Following Radial Shaft Osteosynthesis.

Radial head dislocation after both bone forearm fractures (BBFF) fixation is rare. The management of BBFF involves open reduction and internal fixation (ORIF) to restore anatomy and function. Anatomic reduction of the radius and ulna is required to restore the unique geometric relationship of the forearm and allow normal rotation. Nonanatomic reduction of the radius can result in postoperative radial head dislocation, disrupting forearm function and stability.

Moderate accuracy of the AO/OTA Forearm Shaft Fractures Classification as used in the Swedish Fracture Register.

BACKGROUND: This study evaluated the accuracy of classification of forearm shaft fractures (FSFs) in adult patients registered in the Swedish Fracture Register (SFR). A secondary aim was to examine intra- and inter-rater reliability for the modified 2007 AO/OTA classification within the SFR. MATERIAL AND METHODS: FSFs were classified into three types (A-C) based on fracture pattern and further grouped (1-3) according to involvement of the ulna, radius, or both. Of 130 randomly selected radiograph sets, 122 were available and suitable for validation. Three orthopaedic consultant trauma surgeon

Distal radius morphometry of volar curvature along with scaphoid and lunate facet inclinations and ulnar variance in the Anatolian population.

This study evaluates the morphometric characteristics of the volar distal radius, specifically volar curvature, scaphoid and lunate facet inclinations, and ulnar variance, in the Anatolian population. By identifying these population specific morphological variations, the study seeks to support improved anatomical compatibility of volar implants and enhance surgical planning for distal radius fracture fixation. Morphometric measurements were analyzed retrospectively on three-dimensional computed tomography images of 103 intact distal radii. Standardized sagittal sections were created through th

Mechanical comparison of different plate-and-screw configurations for repairing forearm diaphysis fractures: a systematic review.

PURPOSE: This is the first systematic review of all studies that adjusted plate and/or screw variables to determine the most mechanically stable implant configuration for forearm fracture repair. METHODS: PubMed and Scopus archives were searched using the phrase “biomechanics” AND “fracture” AND “plate” AND (“forearm” OR “radius” OR “ulna”). Eligibility criteria were applied: (i) studies that mechanically analyzed implant performance by modulating plate and/or screw variables, such as plate geometry, plate material, screw number, etc.; (ii) forearm diaphysis “shaft” fractures of the radius and

Percutaneous versus subcutaneous titanium elastic nail tips: complication profiles by age group in pediatric forearm shaft fractures.

BACKGROUND: Titanium elastic nailing (TEN) is widely used for the treatment of unstable pediatric forearm fractures. Although subcutaneous nail tips remain the standard technique, superficial (exposed) nail tip placement has been described as an alternative strategy to facilitate implant removal. However, evidence regarding complication profiles associated with different TEN tip management techniques remains limited, particularly with respect to age-related differences. This study addresses three critical gaps: (1) comparative complication rates between superficial and subcutaneous TEN placeme

Sequential stress fractures of the ulna and radius in a patient with neurofibromatosis type 1: A case report.

Stress fractures of the forearm are uncommon, and to our knowledge, sequential involvement of both the ulna and radius has not been well characterized. Neurofibromatosis type 1 is associated with increased bone fragility and delayed fracture healing due to impaired osteoblastic differentiation, reduced mineralization capacity, and increased osteoclastic activity. We report the case of a woman in her early 60s with neurofibromatosis type 1 and lower-extremity weakness who relied on upper-extremity weight-bearing ambulation. She initially developed a nondisplaced transverse stress fracture of th

Photon-counting CT angiography in a multi-trauma patient with bilateral pulmonary embolism: combined vascular mapping and quantitative iodine-density assessment prior to scalp reconstruction: case report.

Background: Photon-counting detector CT (PCCT) combines high-resolution anatomical and spectral imaging by directly counting individual x-ray photons and discriminating their energies. Its role in surgical planning in complex multi-trauma with concurrent pulmonary embolism remains undefined. Case Presentation: A 58-year-old previously healthy woman was entrapped by a cement mixer, sustaining traumatic scalp avulsion (20 × 20 cm) with exposed skull, depressed right parietal fractures, subdural and epidural hematoma, right 3rd-9th rib fractures, mid-shaft radius and ulna fractures with tendon an

Clinical consequence of the 4-week X-ray control after ESIN osteosynthesis of forearm fractures in children.

PURPOSE: The absence of evidence based general guidelines for radiographic follow-ups of pediatric diaphyseal forearm fractures treated with ESIN results in an arbitrary array of X-ray examinations. In most pediatric traumatology departments, an X-ray check is carried out 4 weeks after ESIN osteosynthesis of forearm shaft fractures to detect incipient consolidation of the fracture. However, the elevated sensitivity to ionizing radiation requires special precautions in the pediatric populations. To prevent the indiscriminate use of this diagnostic tool it is necessary to assess the consequence

Outcomes of displaced mid-shaft radius/ulna fractures in children treated with the Rang method: a pilot study.

Pediatric forearm fractures of the radius and ulna are common. Previous literature suggests that conservatively managed mid-shaft radius and ulna forearm fractures have a remanipulation rate of 10-70%. The purpose of this study is to compare the re-displacement rate of closed displaced mid-shaft both-bone forearm fractures (BBFF) in children treated with closed reduction and casting with a loop and sling applied proximal to the fracture site (Rang method) compared with a standard sling. A retrospective review was performed of 42 patients under the age of 14 with BBFF treated over a 4-year peri

[Shaft fractures in children and adolescents: age-appropriate treatment strategies].

Shaft fractures in children and adolescents differ from those of adulthood in terms of the fracture pattern and treatment. With respect to the procedure, humeral shaft fractures must be differentiated from forearm shaft fractures. Almost no axial deviation can be tolerated on the forearm shaft, even at a young age as this could lead to functional restrictions in the rotation of the forearm. For these reasons, closed reduction and elastic stable intramedullary nailing (ESIN) nowadays represents the gold standard for dislocated or unstable fractures. A special type is the greenstick fracture of

Functional Outcome of Radius and Ulnar Shaft Fractures Treated With Intramedullary Kirschner's Wire Fixation in Children.

INTRODUCTION: Fractures are a common occurrence in childhood, with approximately one-third of boys and girls sustaining at least one fracture before the age of 17. Both-bone forearm fractures, particularly those involving the radius and ulna, are more common in the non-dominant hand and in boys and usually involve the distal portions of both bones. If not properly treated, these injuries can have a significant impact on limb function. OBJECTIVE: This study aimed to determine the functional outcomes of radius and ulnar shaft fractures in children treated with intramedullary Kirschner's wire fi

To Splint or Not to Splint: Outcomes of Forearm Fracture Treatment With Elastic Intramedullary Nailing.

INTRODUCTION: When surgical treatment is indicated, closed reduction with elastic stable intramedullary nailing (ESIN) is a widely accepted technique for managing both-bone forearm fractures in children. However, the optimal postoperative protocol-particularly the necessity and duration of immobilization-remains a matter of debate. OBJECTIVE: This study aims to compare complication rates in patients with both-bone forearm fractures treated with ESIN, with or without postoperative splint immobilization. METHODS: This retrospective study included 137 patients with displaced and unstable both-b

The Legitimacy for the Use of Splint Immobilization After Elastic Intramedullary Nailing of Both-bone Forearm Shaft Fractures in Children.

BACKGROUND: Pediatric diaphyseal both-bone forearm fractures are increasingly treated with elastic stable intramedullary nailing (ESIN) using titanium nails. However, clear guidelines on the type and duration of splint immobilization are lacking. This study aims to assess the necessity of splint immobilization after ESIN for diaphyseal forearm fractures in children. METHODS: A prospective study was conducted on 38 patients with isolated radial and ulnar shaft fractures from 2018 to 2020. Patients were divided into two groups: 14 with splint immobilization for a mean of 3.93 weeks and 24 with

Elastic stable intramedullary nailing for pediatric forearm fractures: A review article.

Fractures of the shaft of the radius and ulna are common injuries in children and adolescents. Undisplaced fractures and fractures that are stable after closed reduction, can be treated with cast immobilization and an interosseous mold. Healing is reliable in children and remodeling would help to correct any residual malunion at the fracture site. However, in older children and adolescents, where the healing and remodeling potential is limited, and for unstable fractures, surgical intervention may be necessary. Intramedullary nailing using elastic nails, also known as elastic stable intramedul
