Developments in the treatment of pediatric long bone shaft fractures.

Shaft fractures of the long bones are among the most common pediatric injuries. They are characterized by a limited potential for spontaneous correction because of the fractures--distance to the physis. Although conservative treatment has been practiced for many years with satisfying results, several aspects have led to an increase in the numbers of surgical procedures including changes of living, sports habits, economics, and patients-request to treatment. We reviewed the literature to describe developments of treatment concepts of pediatric shaft fractures. In shaft fractures of the upper ex

Monteggia equivalent fracture associated with Salter I fracture of the radial head.

Monteggia fracture is an infrequent lesion, which associates ulna fracture and radial head dislocation. Equivalent Monteggia can occur by associated lesions such as olecranon fracture or radial neck or head fracture. We report an unusual case of Monteggia equivalent lesion associating a fracture of the proximal third of the ulnar shaft and a growth plate fracture Salter I of proximal-radial physis and divergent displacement due to a bottle-opener effect of the radial head over the capitellum during trauma. Surgical care consisted of intramedullary pinning of the radial head and fixation by a p

Delayed union in pediatric forearm fractures.

BACKGROUND: The aim of this study was to assess the predictive factors of delayed union in pediatric forearm fractures. Few previous reports have approached this complication, but contrary to these studies we excluded cases with known pathogenic factors and included in the sample cases treated with conservative methods. METHODS: This is a case-control study of all pediatric shaft forearm fractures presenting delayed union in a minimum follow-up period of 12 months, from 2003 through 2009, in a hospital covering a health area. Exclusion criteria were fracture dislocations, infection, suboptima

The distal interosseous membrane: current concepts in wrist anatomy and biomechanics.

The distal interosseous membrane (DIOM) of the forearm acts as a secondary stabilizer of the distal radioulnar joint (DRUJ) when the dorsal and palmar radioulnar ligaments of the triangular fibrocartilage complex are cut. Recent anatomical studies revealed that thickness of the DIOM varies widely among specimens and the distal oblique bundle (DOB) exists within the DIOM in 40% of specimens. The DOB originates from the distal one-sixth of the ulnar shaft and runs distally to insert on the inferior rim of the sigmoid notch of the radius. The mean thickness of the DIOM without a DOB was 0.4 mm, w

Volar dislocation of the distal ulna in supination caused by apex volar malunion of the radial shaft: a report of 2 cases.

INTRODUCTION: Fractures of the distal third of the radius in children are common and most heal without incident. However, distal radial shaft malunion with apex volar angulation may lead to volar dislocation of the distal ulna with forearm supination, although it has been rarely reported. The aim of this study was to document 2 such cases and to make recommendations regarding the management of these patients. METHODS: We report the cases of 2 boys, ages 6 and 8 years, who sustained radial shaft fractures that healed with apex volar angulation and who developed intractable volar dislocation of

Nonoperative treatment of both-bone forearm shaft fractures in children: predictors of early radiographic failure.

BACKGROUND: Forearm shaft fractures are the third most common fracture in children. Although closed reduction and casting is the preferred treatment; outcomes remain variable. The purpose of this study was to identify factors associated with failure of nonoperative treatment for pediatric complete forearm shaft fractures and to explore the time frame in which failure is likely. METHODS: Male patients less than 18 years and female patients less than 17 years of age, who were treated for a complete both-bone forearm shaft fracture between January 2005 and January 2008, were included. Unilateral, trifocal, diaphyseal fracture of the radius with ipsilateral mid-shaft ulna fracture in an adult: a case report.

INTRODUCTION: To the best of our knowledge, a trifocal, diaphyseal fracture of the radius associated with ipsilateral mid-shaft fracture of the ulna in an adult has not been reported in the literature to date. The AO classification system does not include such a fracture configuration. CASE PRESENTATION: We report a case of trifocal, diaphyseal fracture of the radius with a mid-diaphyseal fracture of the ulna in a 53-year-old Caucasian, British, right-hand dominant woman involved in a head-on collision with another vehicle. The management of this rare fracture configuration is described and a

The middle radioulnar joint and triarticular forearm complex.

The forearm is composed of the radial and ulnar shafts, which are linked by the interosseous membrane and intercalated between the elbow and wrist. The radius and ulna are connected by three joints, the proximal, middle, and distal radioulnar joints. The forearm ensures pronation/supination and longitudinal load transfer. The biomechanical and clinical relevance of the proximal and distal radioulnar joints is well established. In contrast, the middle radioulnar joint was considered relatively unimportant until studies published in the last decade showed that it fulfils crucial biomechanical fu

Radius morphology and its effects on rotation with contoured and noncontoured plating of the proximal radius.

BACKGROUND: The radius has a sagittal bow and a coronal bow. Fractures are often treated with volar anterior plating. However, the sagittal bow is often overlooked when plating. This study looks at radial morphology and the effect of plating the proximal radius with straight plates and then contoured plates bowed in the sagittal plane. We report our findings and their effect on forearm rotation. MATERIALS AND METHODS: Morphology was investigated in 14 radii. Attention was paid to the proximal shaft of the radius and its sagittal bow; from this, 6-, 7-, and 8-hole plates were contoured to fit

Surgical exposures of the radius and ulna.

The forearm contains many muscles, nerves, and vascular structures that change position on forearm rotation. Exposure of the radial shaft is best achieved with the Henry (volar) or Thompson (dorsal) approach. The volar flexor carpi radialis approaches are used increasingly for exposure of the distal radius. Although the dorsal approach is a safe utilitarian option with many applications, its use for managing fracture of the distal radius has waned. Potential complications associated with radial exposure include injury to the superficial branch of the radial nerve, the lateral antebrachial cuta

Complex elbow dislocation associated with radial and ulnar diaphyseal fractures: a rare combination.

We illustrate a rare complex dislocation of the elbow involving a posterior ulno-humeral dislocation associated with open diaphyseal fracture of the ulna, radial shaft fracture, Type 1 coronoid fracture and neuropraxia of the deep branch of the radial nerve. The isolated ulno-humeral dislocation without radio-capitellar involvement, and ulnar diaphyseal fracture, makes this "reverse Monteggia" type of injury pattern very unique. This patient was managed with an initial reduction of his ulno-humeral joint and stabilization of his radius and ulna fractures. He underwent a delayed medial collater

Surgical interventions for diaphyseal fractures of the radius and ulna in children.

BACKGROUND: Diaphyseal forearm fractures in children are a common injury usually resulting from a fall. The treatment options include non-surgical intervention (manipulation and application of cast) and surgical options such as internal fixation with intramedullary nails or with plate and screws. OBJECTIVES: To assess the effects (benefits and harms) of a) surgical versus non-surgical interventions, and b) different surgical interventions for the fixation of diaphyseal fractures of the forearm bones in children. SEARCH METHODS: We searched the Cochrane Bone, Joint and Muscle Trauma Group Spe

[Fracture of the diaphyseal radius during Cyr wheel practice - an uncommon injury of wheel gymnastics].

The cyr wheel is a modified gymnastic wheel with only one ring that can lead to extreme forces on the gymnast. We report on a distal radius shaft fracture (AO 22 A 2.1) and a fracture of the styloid process of the ulna that occurred after holding on to a slipping Cyr wheel and exposition to high pressure on the lower arm. The fracture was fixed by screws and a plate.
