A 91-year-old female sustained injuries to her left forearm while walking across a crosswalk. X-rays showed left radial shaft and ulna shaft fractures, and the injury was a type IIIB open fracture. On the day of admission, irrigation and debridement of the open wound, and temporary fixation of the radius and ulna using an external fixator and a Kirschner wire were peformed. Six days after the surgery, we used negative pressure wound therapy (NPWT) using the V.A.C.ATS system for the open wound. Thirteen days after the first surgery, definitive fixation was performed by using locking compression

Subcapital ulnar fractures in association with distal radius fractures in elderly patients increase instability and pose a treatment challenge. Fixation of the ulnar fracture with traditional implants is difficult due to the subcutaneous location, comminution, and osteoporosis. We describe an intrafocal pin plate that provides fixation by a locking plate on the distal ulna and intramedullary fixation within the shaft. The low profile and percutaneous technique make this device a useful alternative for treatment of subcapital ulna fractures in the elderly.

BACKGROUND: Pediatric supracondylar fracture and forearm bone fracture is common in isolation but combined supracondylar fracture with ipsilateral forearm bone fracture, known as floating elbow is not common injury. The incidence of this association varies between 3% and 13%. Since the injury is rare and only limited literatures are available, choosing best management options for floating elbow is challenging.
METHOD: In retrospective review of 759 consecutive supracondylar fracture managed in between July 2005 to June 2011, children with combined supracondylar fracture with forearm bone inju

Irreducible isolated dislocation of the radial head is a rare injury. In this study, we describe a patient with irreducible dislocation of the radial head associated with an undisplaced fracture of the olecranon. A 6-year-old girl fell down while walking and suffered injury to the posterior aspect of the proximal ulnar shaft with the right elbow in a slightly flexed position. Plain radiographs of the elbow revealed an anterior-medial dislocation of the radial head and an undisplaced fracture of the olecranon. However, the attempted closed reduction was not successful. An open reduction was the

PURPOSE: To clarify if dorsal plate osteosynthesis of the forearm shaft is associated with impingement of the interosseous membrane (IOM) and if rotational kinematics of the forearm are influenced.
BACKGROUND: Forearm fractures are treated by separate incisions. Osteosynthesis via one approach in proximal forearm fractures was associated with higher rates of synostosis. It has been claimed that this is due to IOM impingement.
MATERIALS AND METHODS: 16 forearms of fresh frozen cadavers were dissected until only radius, ulna and the IOM remained. To preserve physiological forearm rotation, the

The authors report a case of a patient with complex radial head and ulnar shaft fractures with an associated tear of the interosseous membrane, which failed to heal despite open reduction internal fixation of the ulna, a bipolar radial head replacement, and cross pinning of the distal radioulnar joint. This case demonstrates that failure of the interosseous membrane can occur even in a properly replaced bipolar radial head with adequate immobilization of the forearm and pinning of the distal radioulnar joint.

Shape memory alloys (SMAs) and in particular Ni-Ti alloys are commonly used in bioengineering applications as they join important qualities as resistance to corrosion, biocompatibility, fatigue resistance, MR compatibility, kink resistance with two unique thermo-mechanical behaviors: the shape memory effect and the pseudoelastic effect. They allow Ni-Ti devices to undergo large mechanically induced deformations and then to recover the original shape by thermal loading or simply by mechanical unloading. Diaphyseal fractures of the radius and ulna present specific problems not encountered in the

Malunions of forearm fractures in adults can present with limitation of forearm rotation, or as distal radioulnar joint instability with functional impairment. This contrasts with paediatric patients in whom malunions of similar severities are often better tolerated. We did a retrospective review of six adult patients after corrective forearm osteotomy for symptomatic malunited forearm shaft fractures. The corrective operations were done between 7 and 168 months after initial injury, using oblique or wedge osteotomies. Median follow-up was 22.5 months. The patients recovered well, with statist

BACKGROUND: The most frequent fractures in pediatric patients are those of the mid or distal shaft of the radius and ulna. Most of them can be treated with closed reduction and a brachio-palmar cast, but sometimes redisplacement or angulation occur within the first few weeks. Xray measurements to foresee these complications have been designed; this paper focuses on their certainty and usefulness.
METHODS: Twenty-four patients ages 2-14 years of age were included, all of them with closed fractures of the mid or distal shaft of the radius and ulna treated with reduction maneuvers under anesthes

A randomized, double-blind, sham-controlled, feasibility and dosing study was undertaken to determine if a common pulsing electromagnetic field (PEMF) treatment could moderate the substantial osteopenia that occurs after forearm disuse. Ninety-nine subjects were randomized into four groups after a distal radius fracture, or carpal surgery requiring immobilization in a cast. Active or identical sham PEMF transducers were worn on the distal forearm for 1, 2, or 4 h/day for 8 weeks starting after cast removal ("baseline") when bone density continues to decline. Bone mineral density (BMD) and bone

INTRODUCTION: Treatment of diaphyseal forearm fractures by open reduction and internal plate fixation is a well-accepted strategy. In a variety of fracture localizations, the use of bridging plate fixation with locking compression plates (LCP) has been shown to improve biomechanical and biological characteristics. Only very limited clinical data are available on bridging plate fixation using LCPs for the treatment of diaphyseal forearm fractures. The aims of this study were to assess both clinical outcomes of LCP fracture treatments, and the implant-specific advantages and disadvantages.

BACKGROUND: The treatment of forearm fracture-nonunions continues to represent a therapeutic challenge, and reported outcomes are moderate at best. Limiting aspects of this particular anatomic location include the relation between restoration of shaft length with the anatomy and long-term functional outcome of adjacent joints, as well as the risk of elbow and wrist stiffness related to prolonged immobilization. The present study was designed to assess the outcome of autologous bone grafting with compression plating and early functional rehabilitation in patients with forearm fracture non-union

Congruency of the distal radioulnar joint was assessed by computed tomography after gradual lengthening of the radius in patients with considerable ulnar positive variance of mean 12 (range 10-17) mm and chronic dislocation of the distal radioulnar joint. Six patients of mean age 25 years were treated by radial osteotomy and subsequent gradual lengthening using either a single-rod or a half-ring external fixator, which was applied for a mean of 81 days. The causes of deformity were distal radial epiphyseal injury in four, malunion of a radius shaft fracture in one and Madelung deformity in one

BACKGROUND: Non-surgical treatment of forearm shaft fractures in adults is associated with a high incidence of non-union. Operator errors during surgery also often result in bone union complications. We attempted to identify the errors made during the treatment or other factors that might influence the development of forearm shaft union complications.
MATERIALS AND METHODS: We have analysed the causes of 67 cases of non-union in 53 patients (17 women and 36 men), aged 18 to 85 years. Thirty-five patients were treated by open repositioning and fixation with plates and screws, 11 patients were

BACKGROUND: Forearm fractures are common in the paediatric age group. Closed reduction and casting are the primary means of treatment in over 90% of these fractures. Resultant deformities are usually a product of indirect trauma involving angular loading combined with rotational deformity and fragment displacement.
MATERIALS AND METHODS: Retrospectively, 48 patients aged between 4 to 12 years with forearm fractures, were treated conservatively with closed reduction and a cast during a 2-year period. Functional outcomes were measured in terms of pronation and supination.
RESULTS: All fracture

BACKGROUND: There are no reported series that specifically deal with repair of infected nonunions of the diaphysis of the forearm bones. We sought to determine whether a standardized treatment protocol we have utilized for 15 patients from 1989 to 2005 results in a high union rate, resolution of infection, and a good functional outcome.
METHODS: The study cohort included nine male and six female patients who presented to a University hospital setting with an infected nonunion of the diaphysis of the radius or ulna. Every patient had a minimum of 2-year follow-up. The average patient age was 4

Osteopetrosis (OP) is a rare heterogeneous group of inherited skeletal dysplasias characterised by osteoclast dysfunction, impaired bone resorption and poor bone remodelling. Three groups can be categorised on the basis of clinical findings. These include neurological symptoms, haematological abnormalities and renal tubular acidosis in the first group. Increased bone density, osteomyelitis and frequent fractures are the clinical findings in the second group, and the third group have normal life expectancy but may develop cranial nerve compression and osteomyelitis. Fractures in patients with O

The elastic stable intramedullary nailing (ESIN) is the current treatment of choice for unstable forearm shaft fractures in children. There is no large study on paediatric nonunion of forearm shaft fractures in children after intramedullary nailing. There are only sporadic reports on nonunions after ESIN in children. The aim of this study was to define predisposing factors of nonunions in paediatric forearm fractures. All children who had been treated for forearm fractures by ESIN in our hospital from 1990 to 2006, and all children treated elsewhere surgically and being followed up at our inst

BACKGROUND: A large percentage of autogenous and homogenous grafts are resorbed and replaced by the recipient's own tissue. It is generally believed that autogenous grafts are superior to homogenous ones. The aim of our study was to compare the effectiveness of autografts and allografts in achieving the healing of a non-union of forearm bones.
MATERIAL AND METHODS: Between 1976 and 2005, 56 patients with 68 non-unions of forearm bone shafts were operated on. The study group consisted of 45 male and 11 female patients aged 8 to 85. The patients were operated on using methods requiring autogeno

Elbow dislocation associated with both ipsilateral radial and ulnar shaft fractures is a rare pattern of injury, although it is common for elbow dislocation and forearm fractures to occur separately. We report a case of an 80-year-old woman who had a posterolateral elbow dislocation and ipsilateral radial and ulnar shaft fractures and underwent closed reduction and plate fixation. She had an excellent outcome after 22 months of follow-up.
