Possibility of Avoiding Anesthesia in the Reduction of Greenstick and Angulated Forearm and Distal-End Radius Fractures in Children: A Comparative Study.

Introduction Greenstick and angulated forearm bone fractures are the most common fractures in children and invariably require closed reduction under anesthesia. However, pediatric anesthesia is somewhat risky and not always available in developing countries like India. Therefore, this study aimed to evaluate the standard (quality) of closed reduction without anesthesia in children and to determine satisfaction among parents. Materials and methods The present study included 163 children with closed angulated fractures of the distal radius and fracture shafts of both forearm bones, who were trea

Delayed dislocation of the radial head associated with malunion of distal radial fracture: A case report.

BACKGROUND: Traumatic radial head dislocation (RHD) is a well-described injury in the pediatric population. It is usually associated with an injury to the ulna in Monteggia fracture-dislocation, although it can occur as an isolated injury. Traumatic RHD with ipsilateral radial shaft fracture has rarely been reported. Delayed RHD secondary to the malunion of an isolated radial shaft fracture is extremely rare. CASE SUMMARY: We report a 9-year-old boy with limited pronation of the right elbow. The patient was diagnosed with delayed RHD associated with the malunion of a distal radial fracture. S

Fracture of the two forearm bones and congenital radioulnar synostosis: A case report and review of literature.

Congenital proximal radioulnar synostosis limits prono-supination and is often well tolerated. Only one publication in the literature describes a fracture associated with this malformation. We report a case of radius and ulna shaft fractures in a 35-year-old patient with well-tolerated bilateral proximal congenital proximal radioulnar synostosis.

The use of a laminar spreader for the reduction of extra-articular distal radius fractures: A technical trick.

Extra-articular distal radius fractures are often accompanied with shortening, loss of radial height, and radial displacement of the articular segment relative to the shaft of the radius, all seen in the coronal plane. Reduction can be somewhat challenging when reliance on traction and ligamentotaxis fails, especially in subacute or osteoporotic fractures. In this technical report, we describe a technique where application of a laminar spreader between the radius and the ulna in the metaphyseal region can easily reduce the fracture and help attain anatomic alignment in the coronal plane. An ac

Efficacious Enactment of Ilizarov for Mangled Forearm: A Case Report on Our Resolution for the Revitalization of a "Nearly Lost Limb".

INTRODUCTION: Mangled is the evocative term for a grievance caused by crushing, resulting in a limb confiscation. Motor vehicle crashes and industrial/farm accidents are the leading causes of such occurrences. The verdict to reconstruct or to amputate still commonly lies with the surgical skills and experience of the treating surgeon. CASE REPORT: We present a case of 33-year-old male military personnel involved in a road traffic accident who presented to our center with a mangled forearm with segmental fracture radius and fracture shaft of ulna with vascular injury. He was immediately taken

Adolescent Distal Ulna Physeal Fracture with Extensor Retinaculum Interposition.

Background  Isolated distal ulna fractures are rare injuries and are commonly associated with distal radius fractures. Though most of them can be managed conservatively, few of them require open reduction due to the interposition of various structures. Case Description  In this case report, we report two cases of irreducible isolated distal ulna fractures in adolescents due to the interposition of the extensor retinaculum with its underlying tendons requiring open reduction to achieve bony union and distal radioulnar joint stability. These distal ulna fractures can be best reduced by open redu

Refracture Following Operative Treatment of Pediatric Both Bone Forearm Fractures.

INTRODUCTION: Both bone forearm fractures are common injuries in children. Most can be treated with reduction and casting. Those that fail nonoperative management can be treated with a plate or intramedullary fixation; however, refracture remains a problem. The goal of this study is to evaluate the refracture rate in both bone forearm fractures based on the mode of fixation. METHODS: Institutional board review approval was obtained. A retrospective chart review from 2010 to 2021 at a single tertiary care institution was conducted for all operative patients <18 years who sustained a both bone

Vascularized fibular flap and custom-made synthesis in post-traumatic ulnar diaphyseal pseudarthrosis: a case report.

BACKGROUND: Although isolated fractures of the ulnar shaft are considered common and relatively benign injuries, numerous complications can arise especially in the context of suboptimal care pathways. For pediatric patients, however, there is no single indication of the surgical approach. In the context of the management of these complications, it is known that the vascularized fibular graft has numerous advantages and indications in the treatment of recurrent pseudarthrosis. However, in revision surgery the frequent occurrence of anatomical subversions requires the use of fixation means adapt

Evaluation of Functional and Radiological Outcomes of Humeral Shaft Fracture Treated With Locking Compression Plate.

Background The best surgical procedure for humeral shaft fractures is still plate and screw fixation. Researchers have shown that plate fixation lessens the occurrence of malunion and nonunion. This study aims to describe the functional and radiological outcomes of a humerus shaft fracture treated with a locking compression plate (LCP) using the visual analog scale (VAS) and disabilities of the arm, shoulder, and hand (DASH) scoring systems. Method From December 2020 to July 2022, 25 patients with humerus shaft fractures were enrolled in the prospective observational study at RL Jalappa Hospit

Pronation-Type Galeazzi-Equivalent Fracture: A Case Report.

Fractures of the forearm are common among children and adolescents. Radial shaft fracture with dislocation of the distal radioulnar joint (DRUJ), called Galeazzi fracture, is unusual in pediatrics. The Galeazzi-equivalent fracture is a variant of the classic Galeazzi fracture that occurs in children and adolescents. It is a radius fracture associated with a distal ulnar displaced physeal injury without dislocation of the DRUJ. Our patient was a male, aged 15 years, who visited our emergency department after falling off a scooter onto his left hand. Left wrist X-rays showed a displaced Galeazzi

Combined Supracondylar Humerus Fracture, Monteggia Injury, and Radial/Ulnar Shaft Fractures in a 3-Year-Old Child.

Ipsilateral pediatric elbow and forearm injuries are uncommon, particularly those comprising a supracondylar humerus fracture, radiocapitellar joint dislocation, proximal ulna fracture, and distal forearm fracture. We present the case of a boy who was 3 years, 6 months old and sustained this constellation of injuries. He underwent urgent treatment with closed reduction and percutaneous fixation of the humerus and flexible intramedullary nail fixation of the ulna. He presented with an ulnar sensory and motor neuropraxia that resolved, and he had full radiographic union and upper extremity range

Forearm Fractures: Diagnosis and Contemporary Treatment Strategies.

Diaphyseal forearm fractures represent a spectrum of injury patterns that include isolated radial shaft fractures, isolated ulnar shaft fractures, both-bone forearm fractures, Galeazzi fracture-dislocations, and Monteggia fracture-dislocations. Although less common than distal radius fractures, diaphyseal forearm fractures are still a frequently encountered clinical entity seen by orthopaedic surgeons, both as isolated injuries and in the patient with polytrauma. It is important to review the epidemiology of diaphyseal forearm fractures and the classification systems used to describe them, as

Flexible Nailing of Pediatric Both Bone Forearm Fracture.

This is the case of a 12-year-old boy presented with a displaced radial and ulnar shaft fracture. Historically, these fractures were managed conservatively with closed reduction and cast immobilization; however, there has been an increasing trend toward operative fixation. Malalignment is an indication for operative fixation of pediatric both bone forearm fractures. Flexible nailing was performed in this case because of the small incisions, minimal tissue disruption, shorter operative time, and ease of implant removal. Five years postoperatively, the patient's fracture was healed. Patient pres

"You're O.K. Anesthesia": Closed Reduction of Displaced Pediatric Forearm and Wrist Fractures in the Office Without Anesthesia.

BACKGROUND: Angulated wrist and forearm fractures are among the most common pediatric fractures, and they often require closed reduction with anesthesia. There are several issues associated with pediatric anesthesia including the low but non-zero risk of significant complications, increased physician and staff time and effort, and increased cost. Despite these issues, there have not been any studies to prove that using anesthesia results in better outcomes or higher caregiver satisfaction in comparison to performing closed reductions without anesthesia. The purpose of this study was to evaluat

[Operative Approaches to the distal radioulnar Joint and distal Ulna].

At the distal radioulnar joint (DRUJ) and the ulnocarpal joint several anatomical structures are in a limited space. Therefore, the operative approach has to consider the pathology to be treated. The DRUJ may be approached from the dorsal or the palmar side. Procedures to treat the arthrotically destroyed DRUJ involving bone resection of the ulnar head and destabilizing ligamentous injuries of the triangular fibrocartilage complex (TFCC) are best approached from the dorsal side. This approach allows excellent visualization of the ulnar head and the ulnar and radial attachment of the TFCC. The

Monteggia variant with posterior elbow dislocation and radial shaft fracture: A case report.

INTRODUCTION: The most commonly used Bado classification encompasses the large majority of Monteggia fractures, however, certain patterns cannot be classified with this system. Monteggia injuries and Monteggia-Variants are uncommon but serious injuries that can have dire consequences if not identified and treated appropriately. This paper aims to review relevant literature, discuss current classification systems, and present a case of an atypical fracture pattern. PRESENTATION OF CASE: We present a case of a unique Monteggia variant in an adult that to our knowledge has not been discussed in

Management of ipsilateral trifocal fracture of the radius-a case report.

INTRODUCTION: Diaphyseal forearm fractures are commonly seen in clinical practice. However, segmental forearm fractures are relatively rare. We hereby present a case of surgical management of trifocal fracture of radius (mid-shaft of radius, distal end of radius and neck of radius fracture) with ipsilateral shaft ulna fracture. CASE REPORT: A middle aged patient with ulnar shaft fracture and trifocal radius fracture was managed surgically with open reduction and internal fixation of all four fractures. The ulna was addressed first, to achieve alignment, followed by distal radius and shaft rad

Dynamic palmar dislocation of the ulnar head at the distal radioulnar joint (DRUJ) after radius shaft malunion.

INTRODUCTION: Palmar instability of the distal radioulnar joint (DRUJ) is a rare condition, which is, in contrast to the dorsal dislocation, scarcely represented in the literature. This palmar instability can result from a dorsally angulated malunion of the radial shaft after forearm fracture in childhood. Treating such a condition is controversial in the literature and was described in small case series. This study represents the largest case series in the literature that dealt with this condition, alongside a review of the key papers in the English literature. MATERIALS AND METHODS: This is
