[Instability of fractures of the lower end of the radius: apropos of a series of 167 cases].

One-hundred-sixty-seven consecutive cases of distal radius fracture have been treated in our department from October 1985 until November 1987. All cases were rated according to anatomical criteria defined on the first radiogram. These criteria were the presence of a dorsal tilt greater than 20 degrees, dorsal comminution, intra-articular radiocarpal involvement, an associated fracture of the ulna and comminution of the distal radius. Age greater than 60 years was also considered as a factor in determining severity. With regard to fractures treated conservatively (from which comminuted fracture

Fracture of the upper end of the ulna associated with dislocation of the head of the radius in children.

Dislocation of the head of the radius associated with fracture of the upper end of the ulna is a common elbow injury in children. The literature on diagnosis, however, is relatively limited. The injury can be misdiagnosed and consequently mismanaged. In the present series of 46 children, the age range was from 2.5 to 12 years. The dislocation of the radial head was lateral in 50% of the cases, posterolateral in 28.2%, anterolateral in 17.4%, and anterior in 4.3%. Thorough clinical and radiological examinations are essential. Simple bending of the upper end of the ulna may help to avoid missing

[Intra-focal pinning of fractures of the distal end of the radius 10 years later].

Ten years after its introduction, the technique of intra-focal pinning of fractures of the lower extremity of the radius is now widely used. In view of the large number of operators, it is important to define a number of technical points: insertion of the pins without damaging the nerves or tendons, the number and type of pins, conduct of the operation, clinical and radiological criteria of success, post-operative course and removal of the pins. The author also reviews his experience concerning the frequency and the type of possible complications together with methods designed to avoid them an

[Pseudarthrosis of the lower end of the radius due to interposition of the long extensor of the thumb].

The authors describe one case of a severely displaced Smith's fracture which was originally immobilised by external fixation. Three month's later the fracture had not united and active extension of the thumb was absent. At operation, the extensor pollicis longus tendon was found to be dislocated into the fracture site. The tendon was repaired and the non-union was treated by plating. A review of the literature discovered only one other similar case.

Bone mineral content, cortical thickness and fracture rate in osteoporotic women after withdrawal of treatment with nandrolone decanoate, 1-alpha hydroxyvitamin D3, or intermittent calcium infusions.

Twenty-six post-menopausal osteoporosis patients were studied prospectively over a 2-yr period after the cessation of 2 yr of treatment with either nandrolone decanoate (anabolic steroid), 1 alpha hydroxyvitamin D3 or intermittent calcium infusions, the total observation period being 4 yr. Serial evaluations of bone mineral content in the radius, cortical thickness of the metacarpals and spinal roentgenograms were obtained. Nine patients who had been treated with 50 mg nandrolone decanoate every three weeks did not lose bone from the radius or the metacarpals during the 2 yrs following withdra

[Horizontal cubito-radial pinning in fractures of the distal radius including a postero-internal fragment].

The authors describe 68 articular fractures of the lower end of the radius in which a postero-medial accessory fragment was displaced. They have made a study of the anatomy and fibrous connections of the fragment and conclude that this fragment is best maintained reduced by a transverse pin passed from the ulna towards the radius. They attach great importance in such fractures to injury to the triangular radio-ulnar ligament and to inferior radio-ulnar dislocation.

The Kapandji-Sauvé operation. Its techniques and indications in non rheumatoid diseases.

The Kapandji-Sauvé operation consists in the arthrodesis of the distal radioulnar joint surmounted with a segmentary resection of the lower ulna. This technique may be used not only in rheumatoid dislocations of the distal radioulnar joint instead of the resection of the distal end of the ulna (Moore-Darrach) but also in traumatic diseases such as dislocations, sprains, chronical instabilities of this joint and stiffness secondary to Colles fractures. Two techniques are described, following the original one proposed in 1936. The first one (Technique I) indicated to chronical instabilities seco

[The osteoporosis disease in menopausal women. Physiopathology and therapeutic impact].

Trabecular osteoporosis of spongy bone expresses itself clinically by spontaneous vertebral compression and fractures of the lower end of the radius. Its prevention relies on calcium therapy, physical exercise, vitamin D, and, in women, oestrogens. The most effective curative treatment relies on fluorine, at a dose of 40-60 mg sodium fluoride, in conjunction with calcium equivalent to 1 g element (not the calcium salt). Small doses of vitamin D are used. The treatment is without effect on cortical osteoporosis and thus cannot prevent fractures of the neck of the femur.

Ulnar impingement syndrome.

We report the ulnar impingement syndrome, which is caused by a shortened ulna impinging on the distal radius and causing a painful, disabling pseudarthrosis. Of the 11 cases reported, 10 were due to excision of the distal ulna after injury to the wrist; the other was a result of a growth arrest after a fracture of the distal ulna in a child. The symptoms are a painful, clicking wrist and a weak grip; clinical examination reveals a narrow wrist with pain on compression of the radius and ulna and on forced supination. Radiographs in the majority of cases show scalloping of the distal radius corr

[Osteosynthesis with bracket-plate in the treatment of fractures of the lower end of radius with posterior displacement in adults].

For the last 3 years all fractures of the lower end of the radius with posterior displacement in adults have been treated routinely by osteosynthesis with a bracket-plate. Long-term results have been sufficiently good for this type of treatment to be proposed as standard for these fractures, particularly as it is simple to perform and rapidly completed, with reduced risks in elderly patients. The frequency and severity of sequelae of a disabling nature encountered up to the present have demonstrated the lack of a satisfactory solution for therapy of these fractures. Results were good or very g

[Plate osteosynthesis of unstable fractures of the wrist with anterior displacement].

A shaped plate was used for internal fixation of 23 fractures of the anterior margin of the lower end of the radius and of 5 Smith's fractures. Exposure was improved by opening the carpal tunnel. The fractures were accurately reduced and temporarily maintained by K wires before application of the plate and screws. Satisfactory results were obtained.

The importance of the posteromedial fragment and its specific pinning in fractures of the distal radius.

In fractures of the lower end of the radius, the presence of a posteromedial fragment should be recognized if present. This fragment with two articular surfaces, if not reduced, will damage the radiocarpal and inferior radioulnar articulations. The diagnosis needs to be made on 3/4 X-ray views, as the fracture is often mistaken for a Colles' fracture. The author advises reduction and fixation by a method of double pinning. By comparing two sets of statistics, the overall results are shown to be improved without more complications.

Nerve complications in closed fractures of the lower end of the radius.

Nineteen cases of nerve complications following closed fractures of the lower end of the radius (17 lesions of the median nerve, 4 of which were associated with lesions of the ulnar nerve, and 2 isolated lesions of the ulnar) were observed. A therapeutic approach may be suggested for lesions of the median nerve and/or the ulnar nerve: immediate lesions: immediate reduction, gently if possible and avoiding the position of volar flexion and ulnar deviation. Early secondary lesions associated with immobilization in flexion and ulnar deviation: modify the position and complementary pinning when ne

Congenital pseudarthrosis of the ulna due to neurofibromatosis.

Two cases of congenital pseudarthrosis of the ulna due to neurofibromatosis are reported. Similar radiographic changes in the ulna were found, with distortion of the capitulum and part of the trochlea and bowing of the radius. In one patient the head of the radius was dislocated and in the other it dislocated readily during pronation. A possible mechanism to explain the findings was suggested. On the basis of these and a review of the literature it is recommended that the main aim of treatment should be maintenance of the normal relative lengths of the radius and ulna by early excision of the

Results from resection of the distal end of the ulna after fractures of the lower end of the radius.

Pain, impaired mobility and weakness in the wrist are common complications after fractures of the lower end of the radius. When these symptoms persist, resection of the distal end of the ulna has been the surgical treatment of choice. 24 patients who had undergone this procedure were reviewed. 50% stated they were not improved by the operation. Of 11 patients with degenerative changes in the distal radio-ulnar joint on preoperative X-ray, 8 stated they were helped by the operation, while of 13 patients without any signs of arthrosis in the distal radio-ulnar joint, only 4 experienced relief of

[Arthrography in lesions of triangular fibrocartilage of the wrist (author's transl)].

Arthrography of the wrist is a safe method to demonstrate lesions of the triangular fibrocartilage. Indications are posttraumatic pain and restriction of movement of the wrist. Lesions of the triangular fibrocartilage are caused by a distal fracture of the radius with shortening, sudden drop on the overextended hand and work with rock drills. The extent of injury is quite different: small fissures and splits, detachment of the discus from the lower end of the ulna, fragmentation and destruction of the fibrocartilage. Problems of therapy, however, are greater than problems of diagnosis: actuall
