Redefining Orthopaedic Expectations of Pediatric Fracture Reductions Performed in Hybrid Emergency Departments.

BACKGROUND: Many pediatric emergency department (ED) visits do not occur at a standalone pediatric hospital. Although anticipated outcomes after pediatric fracture reductions are published by tertiary children's centers, limited data originate from the more common hybrid (pediatric and adult) ED setting, where general emergency medicine (EM) clinicians perform reductions. This study aims to clarify success rates after pediatric fracture reductions were performed within a regional health network's emergency medicine system, whereafter subsequent in-network pediatric orthopaedic surgeon follow-u

Upper extremity injury patterns unique to utility terrain off-road vehicles.

OBJECTIVES: Utility terrain vehicles (UTVs), equipped with roll cages and safety harnesses, are increasingly popular recreational vehicles. Unlike all-terrain vehicles (ATVs) which lack these features and have well-documented injury patterns, UTV injury patterns are not represented well in literature. This study aims to characterize these distinct injury patterns and their impact. METHODS: A retrospective chart review from a Level 1 trauma center was conducted, including all UTV- and ATV-related injuries between 2018 and 2022. Statistical analyses utilized Chi-square, t-tests, and Mann-Whitne

Comparative Study of Functional and Radiological Outcomes of Antegrade Versus Retrograde Titanium Elastic Nailing for Pediatric Ulnar Diaphyseal Fractures.

Background Pediatric forearm fractures account for approximately 40% of childhood skeletal injuries, with ulnar fractures being particularly prevalent. Titanium Elastic Nailing (TEN) offers an effective minimally invasive option for these injuries; however, the optimal nail entry point - antegrade (olecranon) versus retrograde (distal ulna) - remains debated. This study compared functional and radiological outcomes, complication profiles, and implant-related morbidity between the two approaches. Methods A retrospective comparative study was conducted from May 2023 to May 2025 at the R.L. Jalap

Pediatric Both-Bone Forearm Fracture with Flexor Digitorum Profundus Entrapment Causing Pseudo-Volkmann Contracture: A Case Report.

CASE: A 10-year-old boy presented with mechanical flexion contractures of the little, ring, and middle fingers after a both-bone forearm fracture surgery. Despite initial rehabilitation, he was unable to extend the fingers actively or passively in a neutral or extended wrist position, although extension was possible with wrist flexion, suggesting tendon entrapment. Surgical exploration revealed flexor digitorum profundus (FDP) entrapment at the radial fracture site. Surgical release provided immediate resolution of finger contractures. CONCLUSION: This case demonstrates that FDP entrapment ca

The use of external fixation in the treatment of nonunion and malunion of the upper extremity.

BACKGROUND: Nonunion and malunion of upper extremity fractures are uncommon but challenging complications, particularly when associated with infection, deformity, or multiple previous operations. Although internal fixation remains the standard treatment, external fixation based on Ilizarov principles offers advantages in complex situations including infection control, deformity correction, and gradual bone reconstruction. This study evaluates the indications, surgical technique, and outcomes of external fixation as a definitive treatment for nonunion and malunion of the upper extremity. METHO

Systemic glucocorticoid use increases fracture risk similarly in individuals with and without type 2 diabetes: a Danish nationwide cohort study.

UNLABELLED: Type 2 diabetes (T2D) and glucocorticoid treatment both increase risk of bone fractures. We found that glucocorticoid use increases fracture risk similarly regardless of T2D status. Findings were consistent over time and across subgroups. Care should be taken when prescribing glucocorticoids to individuals with T2D already at increased fracture risk. BACKGROUND: Type 2 diabetes (T2D) is associated with increased risk of bone fractures, which may be caused by suppressed bone formation. Similarly, glucocorticoid (GC) treatment leads to suppressed bone formation, in addition to incre

Rehabilitation Strategy Guided by Physiological Indicators in a Patient With Tetanus and Multiple Fractures: A Case Report.

Tetanus is a serious infectious disease characterized by muscle rigidity and autonomic storm. In severe cases, long-term deep sedation is required, resulting in a high risk of post-intensive care syndrome (PICS). However, criteria for determining the timing of early rehabilitation intervention have not yet been established. We report the case of a woman in her 70s who developed tetanus following a wood-splinter injury to her right forearm and subsequently suffered bilateral humeral fractures due to muscle rigidity and convulsions. She was admitted to the intensive care unit (ICU) with an Ablet

Osteogenesis imperfecta type V: About a clinical case.

Osteogenesis imperfecta (OI) is a rare inherited connective tissue disorder. It is characterized by short stature, fragility and decreased bone mass, which leads to multiple and recurrent fractures after low-energy trauma, which generates susceptibility to long bone deformity and vertebral compression. There are several types of OI, with types I to IV, in which the COL1A1 and COL1A2 genes are affected, being the most frequent. In recent years, the discovery of new forms of OI has led to research into the pathways critical aspects of bone metabolism, with new genes involved being identified. Th

From injury to reconstruction, utilizing staged technique in a self-inflicted facial gunshot wound: a case report.

BACKGROUND: Facial gunshot injuries are rare and complex, with mortality and complication rates of up to 15% and 30%, respectively. Management of facial gunshot wounds has proven to be challenging for surgeons because the treatment requires multidisciplinary evaluation and planning. We present a rare case of a gunshot wound to the face that resulted in extensive deformities, requiring multiple reconstructive procedures. CASE PRESENTATION: A 19-year-old male of South Asian (Pakistani) origin presented to the emergency department with a 3-day-old gunshot wound to the floor of the mouth, causing

Biodegradable (PLGA) Implants in Pediatric Trauma: A Brief Review.

Background/Objectives: Biodegradable implants have emerged as a promising alternative to traditional metallic fixation devices in pediatric orthopedic surgery. Avoiding implant removal is especially advantageous in children, who would otherwise require a second operation with additional anesthetic and surgical risks. This study reviews the current use of poly(lactic-co-glycolic acid) (PLGA) implants in pediatric fracture fixation and evaluates how they address limitations associated with traditional hardware. Methods: A narrative review was conducted summarizing current evidence, clinical expe

A Rare Case of Ipsilateral Radial Head, Neck, and Shaft Fracture With Intact Ulna in a Young Adult.

Fractures involving multiple levels of the radius are rare and often associated with ulnar involvement or distal radius injury. We report a previously undocumented injury pattern: a comminuted ipsilateral fracture of the radial head, neck, and shaft, with an intact ulna, in a 28-year-old male following road traffic trauma. The classification system of the Arbeitsgemeinschaft für Osteosynthesefragen (AO), also known as the Working Group for Osteosynthesis Questions, does not include this type of fracture configuration. The patient underwent open reduction and internal fixation using a headless

Rare Run-Over Clinical Case. Evisceration of the Intestines through the Wrist.

BACKGROUND: Deaths resulting from car-pedestrian collisions are frequently encountered in forensic practice. Such accidents often cause extensive, multi-system injuries, many of which are concealed beneath minimally damaged soft tissues and may not be immediately visible externally. Common external injuries include extensive skin abrasions, subcutaneous hematomas, and lacerations. Internally, these incidents typically result in multiple bone fractures and severe organ ruptures. A less frequent but noteworthy injury is the evisceration of abdominal organs through ruptures in the diaphragm, rect

Managing complex hand, wrist, and forearm injuries with WALANT: A case-based technical report.

Wide-Awake Local Anaesthesia No Tourniquet (WALANT) is an effective and increasingly utilised method of anaesthesia for the surgical treatment of injuries to the hand, wrist, and forearm. Despite its proven safety, efficacy, and cost-effectiveness, the adoption of WALANT has been limited, particularly in Western healthcare systems where general and regional anaesthesia predominate, especially in complex injuries. We report the application of WALANT in a variety of hand surgical procedures for the treatment of complex injuries ranging from complex fractures to multiple soft tissue injuries in a
