Acute bilateral the thumb ulnar collateral ligament rupture: A case report.

INTRODUCTION AND IMPORTANCE: Ulnar collateral ligament (UCL) injuries of the thumb's metacarpophalangeal (MCP) joint are commonly seen in athletes and result from forced abduction or hyperextension of the thumb. CASE PRESENTATION: This case involved a 29-year-old female who presented with simultaneous pain and instability in both thumbs after a traumatic incident. Bilateral ulnar collateral ligament (UCL) rupture was confirmed radiographically. Open repairs were performed using the pull-out technique and cast spica, followed by postoperative rehabilitation. CLINICAL DISCUSSION: The patient d

The Association of Metacarpal Head Morphology with Risk of Ulnar Collateral Ligament Rupture.

PURPOSE: A flat metacarpal head of the thumb is thought to predispose to ulnar collateral ligament ruptures because of limited range of motion. In this study, we quantitatively compared the roundness of the thumb metacarpal head in individuals with an ulnar collateral ligament rupture to the roundness in patients without any thumb pathology using conventional thumb metacarpophalangeal joint anteroposterior radiographs. METHODS: Patient data were extracted from a single institutional system, including six urban hospitals in the United States. Eighty-two patients with a thumb ulnar collateral l

Traumatic Triceps and Medial Ulnar Collateral Ligament Rupture in an Adolescent Mogul Skier: A Case Report.

Triceps tendon ruptures are uncommon injuries that account for less than 1% of all upper extremity tendon injuries. Medial ulnar collateral ligament injury (mUCL), while common in overhead athletes as a result of valgus forces during the throwing mechanics, has scarcely been reported in non-overhead, throwing individuals. Traumatic assault to the elbow may result in the rupture of the triceps tendon with concomitant mUCL injury. As such an injury pattern typically presents in middle-aged males, weightlifters, or American football players from eccentric overloading of the elbow. We present an a

Distal Triceps Rupture with an Injury of the Medial Ulnar Collateral Ligament: a Result of a Fall Onto an Outstretched Arm - a Case Series.

Traumatic triceps tendon ruptures are rare and known to result in substantial disability unless appropriate surgical treatment is performed. A traumatic rupture can occur due to a fall onto the outstretched hand. Tearing of the triceps tendon results in a valgus load onto the elbow, which can lead to injuries to the radial head/neck and/or the ulnar collateral ligament. Hence, attention must be paid to associated pathologies after diagnosis of rupture to the distal triceps tendon. Our surgical procedure in these cases includes diagnostic arthroscopy to detect concomitant injuries. In the follo

Surgical stabilization of traumatic elbow joint luxation and proximal ulnar fracture in a silvery langur (Trachypithecus cristatus).

CASE DESCRIPTION: A 3-year-old 5-kg sexually intact female silvery langur housed in a single-species group at a zoological institution was presented because of acute trauma to the left forelimb. CLINICAL FINDINGS: Radiography of the left forelimb revealed a type II Monteggia fracture (proximal ulnar fracture with cranial displacement and caudal luxation of the radial head). During surgery, disruption of the annular ligament and rupture of the lateral collateral ligament were noted. TREATMENT AND OUTCOME: The langur underwent open reduction and internal fixation of the ulnar fracture and plac

Transosseous Suture Fixation of Avulsion Fracture of the Ulnar Collateral Ligament of the Thumb Metacarpophalangeal Joint.

Acute traumatic avulsion or rupture of the ulnar collateral ligament (UCL) of the thumb metacarpophalangeal (TMP) joint is a frequent sports-related injury. If not diagnosed and treated early, it may lead to chronic instability, pain, and loss of pinch and grip strength and possibly osteoarthritis. UCL insufficiency may be treated by various techniques, such as bone anchors, transosseous sutures with or without a pull-out button, tendon grafts, or with TMP joint fusion in neglected cases with arthrosis. A simple technique of internal fixation of an avulsion fracture of the distal attachment of

Accuracy of Ultrasound Compared to Magnetic Resonance Imaging in the Diagnosis of Thumb Ulnar Collateral Ligament Injuries: A Prospective Case Series.

This prospective case series compared the accuracy of ultrasound (US) to magnetic resonance imaging (MRI) in differentiating complete displaced (CD) thumb ulnar collateral ligament (UCL) tears from nondisplaced injuries in 10 patients with suspected traumatic thumb UCL injuries. Ultrasound identified 100% (2 of 2) of MRI-documented CD tears, both of which were further confirmed during surgical repair. Ultrasound identified the absence of CD tears in the remaining 8 patients. Although MRI is the reference standard imaging modality for characterizing thumb UCL injuries, ultrasound should be cons

Elbow Common Flexor Tendon Repair Technique.

Medial epicondylitis, also known as "golfer's elbow," is a common orthopaedic condition that typically results from overuse of the flexor pronator mass. Repetitive eccentric loading of the muscles responsible for wrist flexion and forearm pronation leads to microtrauma and subsequent degeneration of the flexor pronator tendon. Patients with medial epicondylitis typically present in the fourth to sixth decade of life and have an insidious onset of medial elbow pain. Occasionally, medial epicondylitis may result from an acute traumatic event, such as an acute avulsion of the common flexor tendon

Complete Rupture of the Triceps Tendon and Ulnar Collateral Ligament of the Elbow in a 13-Year-Old Football Player: A Case Report.

INTRODUCTION: With the increasing number of children and adolescents participating in sports, pathologies once reserved for high-level athletes are now emerging in this younger population. Distal triceps tendon tears represent an injury infrequently seen even among older, skeletally mature athletes. We report a case of distal triceps tendon tear with concomitant ulnar collateral ligament (UCL) injury in a skeletally-immature football player. CASE REPORT: This is a rare case of traumatic triceps tendon tear with UCL injury in a 13-year-old male football player during a fall and hyperextension

Primary Total Hip Replacement for a Femoral Neck Fracture in a Below-Knee Amputee.

INTRODUCTION: Though isolated traumatic extensor pollicislongus rupture and ulnar collateral ligament rupture of the metacarpophalangeal joint of the thumb are well known injuries, it is rare to have simultaneous bilateral thumb injuries from a single traumatic event. In this article, we report simultaneous bilateral traumatic thumb injury with extensor pollicislongus tendon rupture on one side and ulnar collateral ligament tear on the other side. To the best of our knowledge, such injury has never been reported in the English literature. CASE REPORT: A 48-year-old healthy gentleman sustained

Avulsion fracture of the supinator crest as an indication for a sustained posterolateral (sub)luxation of the elbow. A case report, anatomical evaluation and review of the literature.

The treatment of elbow injuries can be challenging because of the complexity of both anatomy and pathology. We present a rare traumatic avulsion fracture of the supinator crest of the ulna in a 37-year-old patient. Conservative treatment in a long arm cast for four weeks led to satisfactory results. Reproduction of the fracture on a cadaveric elbow clarified that the avulsed fragment holds the insertion of the lateral ulnar collateral ligament (LUCL). The mechanism of trauma that causes this fracture is a posterolateral (sub) luxation of the elbow, which usually causes the LUCL to rupture, but

Elbow Trauma in the Athlete.

In caring for athletes, the physician must be able to accurately diagnose and appropriately treat all forms of elbow injuries. Traumatic injuries to the elbow are common in the athlete. The late cocking phase of throwing produces tremendous valgus stress on the elbow that can lead to medial epicondyle avulsion fractures in adolescents or rupture of the medial ulnar collateral ligament in skeletally mature overhead throwers, such as baseball pitchers and javelin throwers. Common traumatic elbow injuries suffered by athletes, surgical techniques for operative repair of these injuries, as well as

Closed Sagittal Band Injury of the Metacarpophalangeal Joint.

Although it is an uncommon injury, traumatic rupture of the sagittal band often results in subluxation or dislocation of the extensor digitorum communis tendon. The radial sagittal band prevents ulnar subluxation of the extensor tendon at the metacarpophalangeal joint. Injury may result from a direct blow to the hand or from relatively low-energy mechanisms. Symptoms range from metacarpophalangeal joint pain and edema to dislocation of the extensor tendon. Associated injuries include collateral ligament sprains, capsular injury, and osteochondral fractures. Many acute injuries can be managed n

Therapeutic options for acute and chronic elbow instability.

INTRODUCTION: Acute elbow instability usually develops after injuries involving the bony or ligamentous stabilizers of the joint. It occurs frequently after dislocation and/or fracture-dislocation, but isolated valgus or varus overloading can also lead to ligament ruptures with subsequent instability. Chronic instability can result from incompletely healed acute injuries or from recurring microtrauma, for example after repetitive strain from participating in certain sports. CONCLUSION: Stable conditions of the joint are essential for early functional post-traumatic or postoperative treatment

Results of terrible triads in the elbow: the advantage of primary restoration of medial structure.

BACKGROUND: The purpose of this study was to report the clinical and radiological outcomes and surgical strategy for terrible triad injury of the elbow. We hypothesized that the outcomes of this type of injury would be satisfactory if the medial structure was routinely restored at the same time as the repair of the lateral structure. METHODS: We retrospectively reviewed the results of this treatment performed in 13 elbows with terrible triads. Our surgical protocol included fixation or replacement of the radial head and repair of the ruptured lateral ulnar collateral ligament through the late

Surgical management of traumatic elbow luxation in two cats using circumferential suture prostheses.

Two Domestic Shorthaired cats were admitted after sustaining a traumatic lateral elbow luxation. Non-surgical management with reduction and external coaptation was unsuccessful at maintaining reduction. At surgery, the lateral collateral ligament (LCL) was intact but grossly elongated in one case, and ruptured in the other. Trans-condylar, trans-radial and trans-ulnar tunnels were drilled from lateral to medial and a suture prosthesis was used in order to augment a suture imbrication of the LCL in the fist cat. The prosthesis was used as a stand-alone procedure in the second case. No postopera
