Evaluation of sonographic ulnar nerve cross-sectional area cutoff values at and around the medial epicondyle for diagnosing ulnar neuropathy at the elbow in wheelchair users.

BACKGROUND: Cubital tunnel syndrome (CuTS) is a frequent complication in wheelchair users, but optimal sonographic cross-sectional area (CSA) cutoff values and their diagnostic performances remain unclear. Electrodiagnosis studies for confirmation can be uncomfortable and may require another visit. AIM: To evaluate the diagnostic performance of the sonographic ulnar nerve CSA (CSA-U) cutoff value of 10 mm2, and to identify alternative CSA-U cutoffs and assess their diagnostic accuracy at and around the medial epicondyle. DESIGN: Single-centre, prospective, observational, cross-sectional stud

Ultrasound Applications in Adaptive Sports Medicine.

The use of ultrasound in adaptive sports has not been well studied. There is more literature for use in wheelchair athletes who suffer overuse upper extremity injuries, including shoulder and biceps tendon injuries, lateral epicondylosis, ulnar neuropathy at the elbow, and carpal tunnel syndrome. A few key lower extremity injury phenotypes have been studied with ultrasound in this population, including HO and skin complications. There are also unique applications of ultrasound, including boosting, respiratory muscle training, and evaluation of deep tissue injuries. Further ultrasound studies f

Association of Ultrasound and Electrodiagnostic Studies in Patients Evaluated for Ulnar Neuropathy.

PURPOSE: Our goal was to determine the association between the severity of electrodiagnostic (EDX) studies with the cross-sectional area (CSA) of the ulnar nerve at the cubital tunnel using diagnostic ultrasound. Based on our clinical experience, we hypothesized there would not be a positive correlation between the severity of EDX and ulnar nerve CSA. METHODS: This was a retrospective analysis of patients 18 years or older evaluated from May 1, 2020, to June 31, 2021, referred for an upper limb EDX and neuromuscular ultrasound to evaluate for an upper limb neuropathy. History, physical examin

Endoscopic Versus Open In situ Decompression of Cubital Tunnel Syndrome: A Systematic Review and Meta-analysis of 686 Cases.

INTRODUCTION: Cubital tunnel syndrome (CuTS) is the second most common compressive neuropathy of the upper extremity. Several surgical approaches are available for a definitive treatment. This systematic review and meta-analysis aimed to comprehensively assess and compare the efficacy and safety of endoscopic in situ decompression (EISD) and open in situ decompression for CuTS. METHODS: A systematic search was conducted using online databases (PubMed/MEDLINE, Embase, and Cochrane) on the 14th of April 2024. All human RCTs and observational studies in English were included in this review. RES

[LACERTUS SYNDROME].

Lacertus syndrome is a relatively rare condition that occurs because of median nerve compression at the elbow. Despite its low prevalence, it is important for healthcare professionals to be aware of this condition and its potential symptoms to provide timely and accurate diagnosis and treatment. This review provides a comprehensive overview of lacertus syndrome, including its anatomy and physiology, symptoms, and challenges of diagnosis. In particular, the article highlights the potential for misdiagnosis as carpal tunnel syndrome or other conditions that affect the median nerve and provides g

Treatment for ulnar neuropathy at the elbow.

BACKGROUND: Ulnar neuropathy at the elbow (UNE) is the second most common entrapment neuropathy after carpal tunnel syndrome. Treatment may be conservative or surgical, but optimal management remains controversial. This is an update of a review first published in 2011 and previously updated in 2012 and 2016. OBJECTIVES: To determine the effectiveness and safety of conservative and surgical treatment for ulnar neuropathy at the elbow (UNE). We intended to test whether: - surgical treatment is effective in reducing symptoms and signs and in increasing nerve function; - conservative treatment is

Order of surgical intervention in double-crush syndrome.

BACKGROUND: Double-crush syndrome involves concurrent nerve compression at two points, typically in the cervical spine and peripherally in the upper extremity, such as the ulnar nerve at the elbow or the median nerve at the wrist. Surgical management of this condition, including which decompression to perform first, remains controversial. We aim to explore the effect of surgical technique, timing, and relative order on outcomes in patients with combined cervical and peripheral upper extremity nerve compression. METHODS: This retrospective study included adult patients treated at a Level 1 trau

A prospective study of predictors of return to work after surgery for ulnar nerve entrapment.

Ulnar nerve entrapment at the elbow (UNE) is the second most common compression neuropathy in the upper limb. Immediate return-to-work (RTW) after surgery is seldom possible due to restrictions and recovery demands. We aimed to explore RTW after UNE surgery and investigate factors contributing to variations. A prospective study with patients undergoing primary UNE surgery was conducted. Treatment decisions were based on a novel five-grade clinical scale. In total, 65 patients (25 women and 40 men) with a mean age of 51 ± 16 years were included. Simple decompression was the predominant procedur

Compression of the Ulnar Nerve by the Arcade of Struthers: Look and You Shall Find.

BACKGROUND: The arcade of Struthers was first proposed by Kane et al in 1973. Clinical investigations of this structure have been limited to small case series, focusing on the arcade as an isolated cause of compressive ulnar neuropathy. The purpose of our study was to investigate the incidence of this structure in patients undergoing ulnar nerve transposition. METHODS: A retrospective chart review of prospectively maintained data in a single surgeon's practice was performed. Records of patients undergoing surgery for compressive ulnar neuropathy at the cubital tunnel were evaluated for docume

Electrodiagnostic severity does not predict short- to midterm outcomes of cubital tunnel release surgery.

HYPOTHESIS: This study aimed to explore the prognostic value of electrodiagnostic studies (EDS) to clarify their utility in clinical practice prior to cubital tunnel release surgery and to identify patient factors associated with patient-reported functional improvement after surgery. Our hypothesis was that patients with severe preoperative findings on EDS will tend to experience less functional improvement after surgery given the extent of ulnar nerve compressive injury. METHODS: Patients with cubital tunnel syndrome and preoperative electrodiagnostic data treated from 2012 to 2022 with cubi

The anatomical variations of the cubital tunnel in a South African body donor sample.

PURPOSE: The ulnar nerve (UN) courses through the cubital tunnel, which is a potential site of entrapment. Anatomical variations of the cubital tunnel may contribute towards cubital tunnel syndrome (CuTS), however, these are not well described. The aim was to compare the range of variations and dimensions of the cubital tunnel and the UN between sexes and sides of the body. METHODS: Sixty elbows from 30 embalmed bodies (17 males and 13 females) were dissected. The prevalence of the cubital tunnel retinaculum (CuTR) or anconeus epitrochlearis (AE) forming the roof of the tunnel was determined.

Volumetric assessment of ulnar nerves in cubital tunnel syndrome with 3D modeling of the MRI and its relationship with electrodiagnostic findings.

BACKGROUND: Thickened nerve cross-sectional areas (CSA) have been investigated in compressive neuropathy, but the longitudinal extent of nerve swelling has yet to be evaluated. We did a volumetric assessment of the ulnar nerve in cubital tunnel syndrome (CuTS) with three-dimensional (3D) magnetic resonance imaging (MRI) modeling and investigated this relationship with clinical and electrodiagnostic parameters. METHODS: We compared 40 CuTS patient elbow MRIs to 46 patient elbow MRIs with lateral elbow epicondylitis as controls. The ulnar nerve was modeled with Mimics software and was assessed

Cubital tunnel release with Wide Awake Local Anaesthesia No Tourniquet (WALANT) technique in an outpatient setting is safe and effective.

Ulnar nerve release is often performed under general anaesthesia. Wide Awake Local Anaesthesia No Tourniquet (WALANT) is a new anaesthetic method increasingly used by hand surgeons in an outpatient setting. It has advantages such as the possibility to shift surgical interventions out of the regular surgical theatre settings into an outpatient clinical setting, no risk of complications or side effects resulting from regional and general anesthesia and decreased costs. The use of WALANT has not been investigated extensively in elbow surgery. This study aims to evaluate clinical outcomes after ul

Subcutaneous Anterior Transposition With Modified Procedures to Prevent Recurrence Associated With the Transposed Ulnar Nerve.

Subcutaneous anterior transposition of the ulnar nerve is a common surgical treatment for cubital tunnel syndrome. However, there are surgical failures associated with the new compressive sites at the edge of flexor carpi ulnaris (FCU) and resubluxation posterior to the medial epicondyle of the transposed nerve. To reduce the muscle volume at the edge of FCU, we approach the ulnar nerve by dividing the muscle belly of the FCU humeral heads. This procedure can reduce repeated traction forces on the transposed nerve at the edge of the FCU. To keep the transposed ulnar nerve anteriorly, we use a

Cubital tunnel syndrome: anatomy, pathology, and imaging.

Cubital tunnel syndrome (CuTS) is the second most common peripheral neuropathy in the upper limb. It occurs due to ulnar nerve compression within the fibro-osseous cubital tunnel at the elbow joint. Although CuTS is typically diagnosed clinically and with electrodiagnostic studies, the importance of imaging in evaluating the condition is growing. Knowing the typical imaging findings of ulnar nerve entrapment is necessary for precise diagnosis and proper treatment. In this article, we focus on the clinical features, workup and complex imaging of the "anatomic" cubital tunnel and relevant pathol
