Relative frequency of nerve conduction abnormalities at carpal tunnel and cubital tunnel in France and the United States: importance of silent neuropathies and role of ulnar neuropathy after unsuccessful carpal tunnel syndrome release.

The two most frequently diagnosed nerve compression syndromes of the upper extremities are carpal tunnel syndrome and cubital tunnel syndrome. In order to determine the relative frequencies of nerve conduction abnormalities at the carpal tunnel and cubital tunnel in France and the United States, we evaluated all patients (nFR = 882 and nUS = 818) who had nerve conduction studies of the upper extremities over a six-yearFR or three-yearUS interval. In both France and the United States, the risk of electrophysiological abnormalities was 2 to 1 for the wrist relative to the elbow. Abnormal median

[Diagnosis, etiopathogenesis and therapy of carpal tunnel syndrome].

This is a retrospective study analyzing the operation records of 637 patients cured for carpal tunnel syndrome. No statistical difference could be demonstrated between the mean age of female compared to male. However a highly significant statistical difference between age distribution in between sex could be demonstrated (p = 0.0015). 61.3% of the female (male: 47.6%) are affected by the disease between 40 and 65 years and only 16.4% (male 27.6%) later on. Under 40 years of age both sex are affected in the same proportion. This confirms that hormonal factors related to sex play a major role in

Electrodiagnosis of retrograde changes in carpal tunnel syndrome.

We measured evoked mixed nerve action potentials (EMNAP) in the forearm segment of 108 hands as an indicator of retrograde degeneration in the carpal tunnel syndrome (CTS). This was made by recording at the elbow and stimulating the median nerve 4 cm proximal to the wrist crease. The amplitude of this EMNAP decreased in proportion to the prolongation of the motor terminal latency (MTL) or the decrease in the amplitude of the compound muscle action potential (CMAP) from the abductor pollicis brevis (APB) muscle. This suggests that retrograde degeneration progresses as the severity of CTS increa

Soft-tissue injuries related to use of the computer keyboard. A clinical study of 53 severely injured persons.

We studied 53 disabled keyboard operators who complained of pain in the forearms, elbows, wrists, shoulders, and hands. Passive wrist flexion and dorsiflexion impairment to less than 70 degrees due to myofascial shortening associated with an increase in forearm muscle pain on palpation was a useful clinical indicator of injury. Isometric muscle testing was useful in detecting injury to specific muscles. Ligamentous hypermobility of finger joints (72%) and harmful inefficient keyboard styles (intrinsic ergonomic factors) were noted, videotaped, and analyzed. Changes in the workstation (extrinsi

[The risk of vibrations to the hand-arm system and cumulative trauma disorders in shoe manufacturing: a clinical case report].

The article reports the case of a woman working in a shoe factory who had been using vibrating tools for 5 years. Because of paresthesia in her left hand, in 1989 she underwent various diagnostic trials that revealed an initial carpal tunnel syndrome of the left wrist and a homolateral epicondylitis. Measurement of the vibrations transmitted to the hand from the vibrating tool confirmed the probability of a high risk of exposure for the hand-arm system. Such adverse effects have been described by British authors as "cumulative trauma disorders" and were confirmed in this study. The importance

[An analysis of overstrain injuries in rock climbing].

Between spring and autumn 1990 a study was performed with the goal of recording and classifying overstrain injuries due to rock-climbing and to define their causes. Of the 332 climbers participating in the study, 114 (34.4%) had suffered from at least one overstrain injury. The degree of climbing skill proved to be the main risk factor; with increasing climbing skills of the observed persons the percentage of injuries increased very substantially. The degree of climbing skill also was the only significant difference between injured and non-injured persons--injured persons had a climbing skill

The value of electrophysiological examination of the flexor carpi ulnaris muscle in the diagnosis of cubital tunnel syndrome.

In the electrodiagnosis of cubital tunnel syndrome, the abductor digiti minimi (ADM) muscle is frequently used to evoke compound muscle action potentials (CMAP) in order to measure the motor nerve conduction velocity (MNCV) in the ulnar nerve. Slowing of the MNCV across the elbow is a major criteria in the electrodiagnosis of cubital tunnel syndrome. However, in advanced cases, slowing of the MNCV is also seen in the forearm segment, thus, localization of the nerve palsy is sometimes difficult. We evoked CMAP from the flexor carpi ulnaris (FCU) muscle to assess the value of electrophysiologica

Benefits and disadvantages of joint hypermobility among musicians.

BACKGROUND: Joint hypermobility is considered to be both an advantage and a disadvantage. However, the degree of hypermobility in members of particular occupations requiring intense physical activity and the nature of the association between symptoms referable to specific joints and their hypermobility are unknown. METHODS: We interviewed 660 musicians (300 women and 360 men) about work-related symptoms such as joint pain and swelling and examined them for joint hypermobility according to a standard protocol. We then determined the relation between the mobility of their fingers, thumbs, elbow

Anterior subcutaneous transfer of the ulnar nerve in the athlete.

To evaluate the effectiveness of subcutaneous subfascial anterior transfer of the ulnar nerve in the surgical treatment of cubital tunnel syndrome in athletes, we retrospectively reviewed athletes undergoing subcutaneous anterior transfer of the ulnar nerve at the elbow. Criteria for inclusion in the study included active participation in athletic activity, confirmed cubital tunnel syndrome, failure to respond to conservative treatment, and having an anterior subcutaneous subfascial transfer as the only procedure performed. Twenty athletes underwent a total of 21 procedures. Results were evalu

Arthrolysis for post-traumatic stiffness of the elbow.

Twelve of 14 patients who underwent arthrolysis of the elbow for post-traumatic stiffness were re-examined after a mean of 62 months. Pain and subjective stiffness were improved. The mean function of the elbow improved from 73 degrees to 112 degrees. Those operated on within a year of their injury improved twice as much as those who had been operated on after a longer period. Arthrolysis may be very rewarding when conservative treatment fails, especially when it is carried out within a year of the injury.

Common elbow problems in the athlete.

Because of the popularity of sports participation, sports physical therapists must recognize in the athlete the many clinical conditions that occur about the elbow. The purpose of this paper is to present the most common elbow problems that an athlete may encounter and to provide information to facilitate recognition of elbow pathology. This information is essential before initiating treatment. An attempt is made to include sprains, strains, neuropathies, dislocations, fractures, contusions, vascular insults, and skin problems in the distal humerus, elbow, and proximal forearm of both the imma

[Ultrasound imaging of the elbow joint. Loose joint bodies and osteochondrosis dissecans].

In 6 patients with loose bodies due to osteochondritis dissecans, trauma or chondromatosis at the elbow, an additional ultrasonic examination was performed. In all cases loose bodies as well as the osteochondral lesion at the radial head and trochlea humeri were seen. Ultrasonic examination of the elbow is recommended in addition to the mandatory x-ray examination. However, sonography enables reduction of x-ray exposure because additional x-ray tomography can be avoided.

Peripheral nerve injuries in athletes. Treatment and prevention.

Peripheral nerve lesions are uncommon but serious injuries which may delay or preclude an athlete's safe return to sports. Early, accurate anatomical diagnosis is essential. Nerve lesions may be due to acute injury (e.g. from a direct blow) or chronic injury secondary to repetitive microtrauma (entrapment). Accurate diagnosis is based upon physical examination and a knowledge of the relative anatomy. Palpation, neurological testing and provocative manoeuvres are mainstays of physical diagnosis. Diagnostic suspicion can be confirmed by electrophysiological testing, including electromyography an

Impaired sensory conduction in a mixed nerve induced by high frequency stimulation of the muscle fibres. An experimental study in cats.

To see if symptoms such as sensory disturbance or numbness worsen during exercise as is thought to occur in entrapment neuropathy, we studied the interaction between sensory and muscle nerve fibres in a mixed nerve in cats. Stimulation electrodes were placed on both the deep (muscle) and superficial (sensory) branch of the radial nerve at the elbow. High frequency stimulation at 100 Hz was applied to one branch for five minutes and the nerve action potential of the other branch was recorded at the axilla where the radial nerve is mixed. The stimulation decreased the amplitude of the nerve acti

Musculoskeletal adaptations and injuries due to overtraining.

Overtraining places a demand on the musculoskeletal system that may lead to damage to the musculoskeletal system, as well as to clinical, functional, and biomechanical adaptations that may be detrimental to sport performance. The types of injuries identified range from overt, which are obvious injuries that will usually prevent athletic performance for some period of time, to the subclinical, which decrease performance, but may be seldom recognized. These injuries apparently may be avoided or lessened in severity by a combination of several methods. A thorough preparticipation evaluation is im

Overuse injuries of the olecranon in young gymnasts.

We report overuse injuries in 14 elbows of ten élite young gymnasts. In 12 elbows of eight patients aged 11 to 15 years there was a spectrum of radiological abnormalities including widening of the olecranon physis and fragmentation of the epiphysis. The radiographs were compared with those of normal elbows matched for age and sex. The radiograph appearances were very similar to those of the Osgood-Schlatter lesion of the tibial tuberosity. Two older boys, aged 18 and 19, had stress fractures through the olecranon growth plate. Conservative management was successful in all except one case of st

A survey of golf injuries in amateur golfers.

A survey of 461 amateur golfers was undertaken to assess golf injuries. There were 193 respondents of whom 57% reported injuries. Wrist, back, muscle, elbow and knee problems were the most likely ailments to compromise a player's game. Overuse and poor technique were the main aetiological factors. Attention to those factors in addition to maintenance of physique would reduce the incidence of these problems.
