Proposed screening tool for the detection of cumulative trauma disorders of the upper extremity.

The incidence of cumulative trauma disorder (CTD) has become noteworthy in plants that rely on automated production. This rise was evident in an Ohio poultry company. The purpose of this study was to develop a screening tool to detect employees who might be at risk of sustaining a CTD injury and to identify trends of CTD at specific workstations. Thirty-five employees at three workstations completed a questionnaire and underwent a physical examination. Results identified 24 employees with a potential for CTD, and these employees were recommended for remediation. Job modifications and supportiv

Injuries in Professional Rodeo: An Update.

In brief: A medical team kept records for four years on all injuries that occurred during an annual state championship rodeo. Fifty-five (19.7%) of the 278 competitors and rodeo clowns who participated sustained a total of 61 injuries. Bull riding was the most dangerous event, accounting for 20 injuries to 18 competitors; saddle bronc riding and bareback riding followed with 13 injuries each. Barrel racing was the safest, with no injuries. The most common injuries were contusions, sprains, and strains, and the elbow was injured more often than any other part of the body. Protective equipment a

Lateral and medial epicondylitis of the elbow.

Tennis elbow is a common condition, with the extensor carpi radialis brevis attachment being the usual site of pain. Conservative care including decreased activity, ice, nonsteroidal anti-inflammatory medications, and muscle strengthening will help most people. The small percentage of cases that require surgery usually benefit from debridement of the damaged portion of the extensor carpi radialis brevis attachment. The postoperative course must include muscle strengthening and a gradual return to activity.

Ulnar nerve involvement in carpal tunnel syndrome.

This is a retrospective study of the involvement of the ulnar nerve in patients with electrodiagnostic evidence of carpal tunnel syndrome as defined by median sensory nerve action potential (SNAP) greater than 3.6msec at the wrist and/or motor distal latency in excess of 4.3msec. The study included 248 patients, 63 (25%) with unilateral and 185 (75%) with bilateral carpal tunnel syndrome. The electrodiagnostic criteria for ulnar nerve involvement was a SNAP peak latency greater than 3.7msec and/or motor distal latency in excess of 4.2msec. One hundred fourteen patients (46%) had delayed ulnar

The contribution of median-ulnar communications in diagnosis of mild carpal tunnel syndrome.

In six patients with characteristic symptoms of carpal tunnel syndrome, electrophysiologic studies were normal, except for an initial positive deflection of the thenar muscle action potential with median nerve stimulation at the elbow but not at the wrist. This phenomenon is only seen with coexistent carpal tunnel syndrome and median to ulnar nerve communication and suggests carpal tunnel syndrome is present in these patients. As normal electrophysiologic studies occur in carpal tunnel syndrome (up to 8%) and median to ulnar communications are common (15%-31%), this observed finding is of prac

Sports medicine for the internist.

Because of increasing participation in leisure sports and exercise rehabilitation programs, many patients are developing nonsurgical, soft tissue overuse syndromes. Many of these musculoskeletal problems arise because of underlying biomechanical difficulties, training errors, or improper use of equipment. Epidemiologic observations reveal that these individuals frequently visit a general medical practitioner before seeking the advice of an orthopedic specialist. Prompt diagnosis simply requires an appreciation of anatomy and an analytic understanding of sports biomechanics. Although compulsive

Entrapment neuropathies of the upper extremity.

Entrapment neuropathies occur in a number of different but predictable locations in the upper extremity. These neuropathies can produce a spectrum of sensory and motor deficits in the hand and upper extremity. A careful clinical examination in conjunction with electromyography and nerve conduction velocity studies can usually delineate peripheral nerve dysfunction. Prognosis depends on the degree of nerve injury, which is related to both the cause and the duration of the entrapment.

Fibrolipomatous hamartoma of nerve. A clinicopathologic analysis of 26 cases.

Nineteen cases of fibrolipomatous hamartoma of nerve without macrodactyly and seven cases with macrodactyly are discussed. Twenty-five involved the hand, wrist, palm, and finger, and one case involved the foot. Nineteen patients had isolated fibrofatty enlargement of nerve, while seven had macrodactyly in addition to the peripheral nerve changes. Involved nerves included the median nerve, ulnar nerve, an unidentified nerve near the elbow, and a nerve on the dorsum of the foot. Four of nine patients with neurologic symptoms of pain or paresthesias had physical findings compatible with compressi

Carpal tunnel syndrome in paraplegic patients.

Median nerve compression at the wrist (Carpal Tunnel Syndrome) is commonly associated with local trauma around the flexor retinaculum. Repeated manual activity also exacerbates the disease severity. We undertook a prospective study of the incidence of Carpal Tunnel Syndrome (CTS) in 47 paraplegic patients who have used their hands extensively for daily activity. Since surgical decompression generally provides excellent relief of symptoms, early detection of CTS will be particularly important in these patients. Of the 47 patients studied, 19 had clinical CTS (40%). A total of 91 hands (nerves)

Orthopedic problems in athletes.

Physicians and patients must recognize the demands of the sport involved, as well as the individual's ability to meet those demands. Careful analysis of the sport and examination of the patient to assess recovery and chances of reinjury are mandatory, if he is to enjoy sports after injury. Physicians should become familiar with padding, shoes, warm-up programs, preparticipation examinations, and protective equipment and devices, all of which are helpful in preventing sports injuries. We have touched only briefly on the epidemic problem of quality of life trauma that is part of our society. Bec

Clinical and neurophysiologic characteristics of the pronator syndrome.

Nine patients were clinically diagnosed as having a pronator syndrome, i.e., high median nerve compression. The main symptom was pain at the proximal volar aspect of the forearm increasing for several hours after exercise. All patients showed local tenderness over the median nerve 4-5 cm distal to the elbow and pain on active forearm pronation against resistance. Two patients had been previously operated upon for carpal tunnel syndrome. Preoperative routine neurographic-electromyographic studies were normal. In the differential diagnosis, the exclusion of carpal tunnel syndrome and anterior in

Amyloid arthropathy in patients undergoing periodical haemodialysis for chronic renal failure: a new complication.

Seven patients (five male and two female) with chronic renal failure (CRF) treated by periodical haemodialysis presented with swelling and effusion of more than three months' duration in knees (four bilateral), shoulders (two, one of them bilateral), elbow (one), and ankle (one). Four had a carpal tunnel syndrome both clinically and electromyographically (three bilateral). All patients had hyperparathyroidism secondary to their CRF, which was not due to amyloidosis in any of them. The dialysis duration period varied from five to 14 years, with an average of 8.6 years. [Thoracic outlet syndrome and its significance for surgery of the hand (on the etiology and pathogenesis of epicondylitis, tendovaginitis, median nerve compression and trophic disorders of the hand)].

At present, the term "thoracic outlet syndrome" (TOS) is used as a term to include all factors compressing the nerves and vessels situated in the outlet of the thorax and the costo-clavicular area. It is marked by neurological and vascular disorders; their manifestation can be either spontaneous or posttraumatic. In case of surgical treatment the transaxillary approach (according to Roos) proved to be the best, and is indicated in therapy-resistant TOS, in certain cases of arterial complications and in outlet obstructions of the subclavian vein. Entrapment of the median nerve after dislocation of the elbow.

An unusual case of posterior dislocation of the elbow, complicated by entrapment of the median nerve, is reported. Entrapment occurred in the reduction of an associated medial humeral epicondyle fracture. Because the nerve was not in the joint, differentiating the entrapment from a nerve traction injury was difficult. Careful diagnosis and delayed specific surgery can secure an excellent outcome.
