Contralateral median nerve autograft.

Interfascicular nerve autografting is a useful method to repair nerves with gaps, mainly if the removal of a nerve does not imply a defect to the donor area. Final results of peripheral nerve repair definitely depend on the size of the nerve gap, the level of the injury and the time elapsed from the time of injury until the nerve is repaired.

Palmar uniportal extrabursal endoscopic carpal tunnel release.

A new technique of endoscopic carpal tunnel release using a 1.5-cm longitudinal palmar incision was used in 280 cases. The incision allows identification of the superficial palmar arch as well as the median nerve and its branches. A new knife/sleeve device that attaches to a standard 4-mm endoscope was created to simplify the procedure. The flexor retinaculum is endoscopically divided proximally into the distal forearm; the "interthenar fascia" (fascia superficial to transverse carpal ligament) can be preserved. Early postoperative results include a mean overall return to work and full activit

Large-scale reorganization at multiple levels of the somatosensory pathway follows therapeutic amputation of the hand in monkeys.

Reorganization of somatosensory cortex after peripheral nerve damage typically has been attributed to cortical plasticity. Here we provide evidence that much of the large-scale cortical reorganization that occurs after a major loss of peripheral inputs reflects the sprouting or expansion of afferents from the remaining forelimb into deprived territories of the spinal cord and brainstem. We examined sensory afferent terminations in the spinal cord and brainstem, and determined the somatotopic organization of cortical area 3b in three adult monkeys with previous hand or forearm amputation, as ve

[Transposition of flexor pollicis brevis muscle for reconstruction of opposition function of the thumb: anatomical study and clinical application].

Anatomical study was performed on 20 cadaveric hands and the results showed that M. flexor pollicis brevis overlap the M. abductor pollicis brevis for about half of its width at their origins and the overlapping reduced to about 1/3 of the width at the muscle belly level. The M. flexor pollicis brevis chiefly inserts on the palmar aspect of the base of proximal phalanges. The M. abductor pollicis brevis primarily inserts to the radial side of MP joints, the M. flexor pollicis brevis is entirely innervated by the deep branch of the ulnar nerve. In order to increase the angle bettween longitudin

The effect of wearing a flexible wrist splint on carpal tunnel pressure during repetitive hand activity.

We investigated how repetitive hand activity normally affects carpal tunnel pressure and whether a flexible wrist splint can influence this effect. Nineteen healthy subjects were evaluated under four test conditions: at rest with and without a wrist splint (baseline) and while performing a repetitive task with and without a wrist splint. The task involved loading and unloading 1 lb. cans from a box at a rate of 20 cans per minute for period of 5 minutes. Carpal tunnel pressure and wrist angles were continuously monitored by means of a fluid-filled catheter inserted into the carpal canal and a

Nerve repair: correlation of restitution of functional sensibility with specific cognitive capacities.

To test the hypothesis that cognitive capacity is correlated with the outcome of functional sensibility after nerve repair, 19 patients were evaluated 2 to 5 years after median or ulnar nerve repair at the distal forearm level. The sensory evaluation included tests for functional sensibility as well as assessments addressing perception thresholds for touch/pressure and vibration. Psychometric tests for cognitive capacity were also carried out. Multiple regression analysis, correcting for the effect of age and the ability to perceive touch/vibration, was used to investigate the relationship bet

The results of secondary microsurgical repair of ulnar nerve injury.

110 patients with injuries to the ulnar nerve were assessed, on average, 12.7 (3-20) years after secondary repair. 89 were male and 21 female, with a mean age of 27.9 years. Most of the nerve lesions were sharp (63) or blunt (41) injuries. Division was total in 76 cases and most were at the forearm level. The average delay from the injury to the operation was 10.1 (1-48) months. Secondary repair was performed in 34 cases and fascicular grafting in 76 cases. The mean graft length was 5.4 (1-30) cm. Four factors of motor and sensory function were assessed and the quantitative evaluation was comp

Anatomic basis of the physiopathology of the epicondylalgias: a study of the deep branch of the radial nerve.

Compression of the deep branch of the radial n. during its passage through Fröhse's arcade in the supinator m. is one of the classical explanations advanced for epicondylalgia. The object of this study was to define the anatomy of the deep branch of the radial n. from its origin up to the origin of the branches to the lateral epicondylar mm. 34 upper limbs were dissected and three segments of the deep branch of the radial n. were distinguished: segment I, from its origin to its entry into the supinator m.; segment II, corresponding to its passage through the supinator m.; and segment III, exte

Posterior interosseous nerve: an anatomic study of potential nerve grafts.

Anatomic dissection of the posterior interosseous nerve in 26 cadaver forearms was performed to assess variability in location, diameter, and branching patterns of the nerve. The purpose of our study was to determine the anatomic reliability of the posterior interosseous nerve at the distal forearm level as a potential donor nerve graft for segmental digital nerve loss. The nerve was located in a separate fascicle sheath in the deep radial aspect at the fourth dorsal compartment in all specimens. It ranged from 1 to 5 mm in diameter with one to five fascicles present (15 of 26 specimens had a

Tubed radial fascial flap and reconstruction of the flexor apparatus in the forearm.

A reversed radial forearm fascial flap has been used as a vascularized tube through which tendon grafts may glide and in which nerve grafts may rest. It may have applications in the treatment of the severely damaged flexor system at wrist level. A further advantage of this technique is that it does not [corrected] sacrifice the radial artery in an already injured hand.

[Complete restoration of supination and flexion strength after surgical treatment of distal biceps tendon rupture by the Boyd-Andersson method].

Two patients with rupture of the distal biceps brachii tendon were treated using the Boyd-Andersson two-incision technique. The aim of the method is to restore the anatomy and the important function of the biceps muscle as the strongest supinator of the forearm. In the present investigation, both patients returned to their preinjury activities at 8 and 12 weeks, respectively. At follow-up 20 and 101 months postoperatively they had returned to normal levels of elbow flexion and supination strength measured with a strain-gauge. One patient suffered from a partial radial nerve palsy, which, howev

The superficial branch of the radial nerve: an anatomic study with surgical implications.

Twenty fresh cadaver extremities were dissected to delineate and quantify the course of the superficial branch of the radial nerve. This branch bifurcated from the radial nerve at the level of the lateral humeral epicondyle in eight specimens, and in all specimens the bifurcation was no more than 2.1 cm from the lateral epicondyle. It continued distally, deep to the brachioradialis and became subcutaneous a mean of 9.0 cm proximal to the radial styloid, traversing between the tendons of the brachioradialis and extensor carpi radialis longus. The superficial branch of the radial nerve branched

Intraforaminal repair of plexus spinal nerves by a posterior approach: an experimental study.

Many spinal nerve roots injured due to stretch or other types of lesions are not reparable. Some spinal nerves might be repaired if they could be exposed in their intraforaminal course. A posterior subscapular approach for a more lateral exposure of the brachial plexus was combined with a facetectomy to expose intraforaminal nerves in a series of Macaca rhesus monkeys. This approach exposed a 6- to 10-mm segment of spinal nerve not approachable by a more classic anterior operation. Sural grafts were placed from the dural exit of the spinal nerves to the cord level of the plexus. Nine surviving

[Reducing tension in sutures of peripheral nerves].

A method is presented to reinforce normal suturing techniques in median and ulnar nerve repairs at the level of the forearm or wrist. If the distance between the stumps does not exceed 20 to 30 mm the coaptation can be protected from tension by two ribbons of polydioxanone (PDS) which are sutured to the epineurium bridging the site of the suture of the nerve. PDS ribbons proved to give remarkable support to experimental nerve repairs using epineural, perineural or Tsuge suture techniques. In further experiments with tibial nerves of rats the PDS ribbon did not interfere with the nerve function

Motor reflex responses elicited by cutaneous stimulation in the regenerating nerve of man: axon reflex or ephaptic response?

In 57 of 60 nerves (29 median and 31 ulnar) sutured at the wrist, forearm and arm, we recorded motor responses in thenar or hypothenar muscles by electrical stimulation of the corresponding fingers. Recordings were made at different times during the process of regeneration, ranging from 3 months up to 11 years. The responses showed a constant shape and latency to every stimulation (simple or repetitive). The latency was shorter the more distal the level of injury and the greater the elapsed time from the reinnervation. The point of "reflexion" of the responses is at or very near the line of ne

[Cross-forearm replantation in bilateral amputation].

We report the case of a 27-year-old patient who sustained a traumatic amputation of his 4 limbs. For the right upper limb, the site of amputation was at the level of the upper third of the forearm. For the left upper limb, the site of amputation was at the level of the Cower third of the forearm. In both cases, the loss of soft tissue was so extensive that hand replantation was impossible in the initial position. In such an unusual case, replantation of the right hand to the left side was attempted in this young patient. Osteosynthesis of the radius to the ulna in supination was performed, and

The authors observed 25 children with combined injuries of flexor tendons and vascular nervous fascicles on the level of the lower third part of the forearm and the carpal canal. During the initial operation the flexors and the nerves were restored and the nerves were ligated in all patients. Precision equipment was employed when putting in the epineural stitches. A complex of electrophysiological non-invasive and clinical methods of investigation of regional hemodynamics and the neuromuscular apparatus of the hand were employed during this study. Reconstruction of the ulnar collateral ligament in athletes.

Reconstruction of the ulnar collateral ligament using a free tendon graft was performed on sixteen athletes. All participated in sports that involved throwing (mostly professional baseball), and all had valgus instability of the elbow. After reconstruction and rehabilitation, ten of the sixteen patients returned to their previous level of participation in sports, one returned to a lower level of participation, and five retired from professional athletics. Despite precautions, there was a high incidence of complications related to the ulnar nerve. Two patients had postoperative ulnar neuropathy

Clinical and neurophysiologic characteristics of the pronator syndrome.

Nine patients were clinically diagnosed as having a pronator syndrome, i.e., high median nerve compression. The main symptom was pain at the proximal volar aspect of the forearm increasing for several hours after exercise. All patients showed local tenderness over the median nerve 4-5 cm distal to the elbow and pain on active forearm pronation against resistance. Two patients had been previously operated upon for carpal tunnel syndrome. Preoperative routine neurographic-electromyographic studies were normal. In the differential diagnosis, the exclusion of carpal tunnel syndrome and anterior in
