Sensory recovery after forearm median and ulnar nerve grafting.

BACKGROUND: Median and ulnar nerve injuries are common, whether isolated or combined injury of both nerve. A nerve graft, if performed in a tensionless manner, has been shown to generally have better results than an end-to-end approximation performed under tension. OBJECTIVE: The aim of this study is to analyze the long-term results of sensory recovery after secondary reconstruction median and ulnar nerve by autograft in patients who were treated on Clinic for Plastic and Reconstructive Surgery in the period from January 1st 1993 to December 31st 2005. We analyzed the influence of the patient

Prevalence of carpal tunnel syndrome in pregnant women.

Carpal tunnel syndrome (CTS) is a frequent complication of pregnancy, with a prevalence reported as high as 62%. The most typical symptoms are numbness and tingling in the thumb, index finger, middle finger, and radial half of the ring finger. Other common manifestations include burning dysesthetic wrist pain, as well as the loss of grip strength and dexterity. Proximal radiation along the volar forearm, medial arm, and shoulder, while not as common, is not unusual. Symptoms are often worse at night and can be exacerbated by forceful activity and extreme wrist positions. It can be diagnosed to

Long-term results of major upper extremity replantations.

OBJECTIVES: The aim of this study was to evaluate long-term clinical and functional results of major upper extremity replantations. METHODS: We retrospectively evaluated 26 male patients (mean age 27 years; range 3 to 69 years) who underwent major upper extremity replantations and had a mean follow-up of 11.3 years (range 5 to 19 years). The levels of the replantations were transmetacarpal (n=6), wrist (n=4), forearm (n=5), elbow (n=4), and arm (n=7). Amputations were of clean-cut, crush, and avulsion types in seven, eight, and 11 patients, respectively. Secondary operations were performed in

Median Nerve Stimulation in a Patient with Complex Regional Pain Syndrome Type II.

A 54-year-old man experienced injury to the second finger of his left hand due to damage from a paintball gun shot 8 years prior, and the metacarpo-phalangeal joint was amputated. He gradually developed mechanical allodynia and burning pain, and there were trophic changes of the thenar muscle and he reported coldness on his left hand and forearm. A neuroma was found on the left second common digital nerve and was removed, but his symptoms continued despite various conservative treatments including a morphine infusion pump on his left arm. We therefore attempted median nerve stimulation to trea

Clinical evaluation of a resorbable wrap-around implant as an alternative to nerve repair: a prospective, assessor-blinded, randomised clinical study of sensory, motor and functional recovery after peripheral nerve repair.

Peripheral nerve injures are common and often result in impaired functional recovery. The majority of injuries involve the arm and/or the hand. The traditional treatment for peripheral nerve injuries is repair by using microsurgical techniques, either by primary nerve suture or nerve graft, but research to find more successful methods that could improve recovery is ongoing. Tubulisation has been investigated by several authors and is suggested as an alternative to microsurgical techniques. The resorbable poly[(R)-3-hydroxybutyrate] (PHB) is one of the materials that has been previously tested

Cognitive capacity: no association with recovery of sensibility by Semmes Weinstein test score after peripheral nerve injury of the forearm.

In the recovery process of sensibility after repair of a peripheral nerve injury of the forearm, not only age but also surgical repair techniques are of importance. If regenerating axons are misdirected, reorganisation or other adaptic processes are needed at the level of the somatosensory brain cortex. These processes are thought to be dependent on the patient's cognitive capacity. We conducted a prospective multicentre study to assess the association between cognitive capacity and recovery of sensibility after peripheral nerve damage of the forearm. Patients with a traumatic peripheral nerve

Applied anatomy of the superficial branch of the radial nerve.

The superficial branch of the radial nerve (SBRN) is highly vulnerable to trauma and iatrogenic injury. This study aimed to map the course of the SBRN in the context of surgical approaches and identify a safe area of incision for de Quervain's tenosynovitis. Twenty-five forearms were dissected. The SBRN emerged from under brachioradialis by a mean of 8.31 cm proximal to the radial styloid (RS), and remained radial to the dorsal tubercle of the radius by a mean of 1.49 cm. The nerve divided into a median of four branches. The first branch arose a mean of 4.92 cm proximal to the RS, traveling 0.

Manual stimulation of forearm muscles does not improve recovery of motor function after injury to a mixed peripheral nerve.

Transection and re-anastomosis of the purely motor facial nerve leads to poor functional recovery. However, we have recently shown in rat that manual stimulation (MS) of denervated vibrissal muscles reduces the number of polyinnervated motor endplates and promotes full recovery of whisking. Here, we examined whether MS of denervated rat forearm muscles would also improve recovery following transection and suture of the mixed (sensory and motor) median nerve (median-median anastomosis, MMA). Following MMA of the right median nerve, animals received no postoperative treatment, daily MS of the fo

High-resolution sonography versus EMG in the diagnosis of carpal tunnel syndrome.

BACKGROUND: Carpal tunnel syndrome (CTS) is a common entrapment neuropathy. Electrodiagnostic testing (EMG) is used to confirm the diagnosis. It is not known what the diagnostic accuracy of high-resolution sonography is in comparison to EMG. OBJECTIVE: The aim of this study was to compare the diagnostic accuracy of both tests in CTS patients. METHODS: A prospective cohort of 207 patients with possible CTS underwent high-resolution sonography and EMG. The diagnosis of CTS was based on clinical signs and symptoms. The cross-sectional area of the median nerve at the carpal tunnel inlet and at t

Ultrasound examination of peripheral nerves in the forearm.

BACKGROUND AND OBJECTIVES: We examined in a volunteer population whether nerves in the forearm could be seen consistently using ultrasound imaging and whether this new information could have implications for the way we perform regional anesthesia of the median, radial, and ulnar nerves. METHODS: Eleven volunteers underwent ultrasound examination of both forearms. The median, ulnar, and radial nerves were followed and images were obtained at the elbow, proximal forearm, mid forearm, distal forearm and wrist levels. In addition the radial nerve was followed proximally to a point 5 cm above the

Hand function after nerve repair.

Treatment of injuries to major nerve trunks in the hand and upper extremity remains a major and challenging reconstructive problem. Such injuries may cause long-lasting disabilities in terms of lost fine sensory and motor functions. Nowadays there is no surgical repair technique that can ensure recovery of tactile discrimination in the hand of an adult patient following nerve repair while very young individuals usually regain a complete recovery of functional sensibility. Post-traumatic nerve regeneration is a complex biological process where the outcome depends on multiple biological and envi

Enhanced sensory relearning after nerve repair by using repeated forearm anaesthesia: aspects on time dynamics of treatment.

BACKGROUND: We describe a new principle for enhancing the effects of sensory re-education following nerve injury and repair. The outcome from nerve repair in adult patients is generally poor. One reason is the functional cortical reorganisation which always occurs because of axonal misdirection at the repair site. In healthy individuals selective anaesthesia of the forearm results in improved hand sensation. Here we hypothesised that this principle would be valid also after nerve injury and repair. METHOD: In a prospective, randomised, double blind study we studied the effects of cutaneous fo

Enhanced function in nerve-injured hands after contralateral deafferentation.

To explore whether contralateral deafferentation by tourniquet-induced anaesthesia can improve function in a nerve-injured hand, we investigated 14 patients with median or ulnar nerve injuries at wrist level. Deafferentation of the contralateral forearm and hand resulted in significantly improved tactile discrimination, threshold for perception of touch in the injured nerve area and in the area corresponding to the intact nerve in the injured hand, and significantly increased grip strength. Improvements lasted at least 15 min. The results show that hand function after peripheral nerve injury c

Carpal canal pressure of the distracted wrist.

PURPOSE: This study was conducted to study the effect of distraction across the wrist joint on carpal canal pressure. METHODS: Ten cadaver specimens were mounted vertically in neutral forearm rotation by 2 half pins that transfixed the radius and ulna. The wrist joint was distracted by suspending weights from the middle finger. A balloon-tipped catheter, percutaneously introduced into the carpal canal and connected to a transducer, was used to measure carpal canal pressure. The carpal canal pressure was measured at 0 to 4.54 kg of distraction in 0.45-kg increments and at 6.81 kg and 9.08 kg o
