BACKGROUND: Iatrogenic and traumatic sensory nerve injury at the level of the wrist can lead to debilitating neuroma. Targeted muscle reinnervation (TMR) is an effective treatment for the management of symptomatic neuromas. We investigate the use of the anterior interosseous nerve (AIN) as a recipient nerve for the treatment of iatrogenic neuromas. This case series describes 4 patients treated for neuromas of the lateral antebrachial cutaneous nerve (LABC), palmar cutaneous branch of median nerve (PCB), and radial sensory nerve (RSN).
METHODS: Four cases involved a symptomatic neuroma of the

PURPOSE: Median nerve injuries can result in substantial functional deficits with both motor and sensory components. Sensory loss results in impaired hand function. The most important areas are the ulnar and radial aspects of the thumb and index finger, respectively. This study aimed to evaluate the feasibility of nerve transfer from the lateral antebrachial cutaneous nerve (LACN) to the median nerve for restoring hand sensation.
METHODS: This study used 16 cadaveric upper limbs. After dissecting the LACN and its branches, the length and size were recorded. The median nerve was identified via

PURPOSE: To evaluate the precise origin of sensory nerves through gross anatomical study of the TFCC, synthesized alongside imaging and histological techniques.
METHOD: Six cadaveric forearm specimens were obtained to map the course and branches of the ulnar nerve through macrodissection. Immunohistochemical staining targeting PGP 9.5 and type IV collagen was performed on frozen TFCC sections to visualize nerve fibers microscopically. Computed tomography, magnetic resonance imaging, and arthrography findings were also reviewed.
RESULTS: At the macroscopic level, the articular branches supply

Penetrating lacerations to the hand are a common cause of nerve injury and can lead to debilitating pain and numbness in the distribution of the nerve affected. Owing to an overlap in the cutaneous innervation from different sensory nerves, clinically identifying the injured nerve can be difficult. We present a novel case of isolated injury to the palmar cutaneous nerve from a penetrating knife injury which was detected using 'comparison waveform' nerve conduction studies. Using this technique, we can isolate injuries to the palmar cutaneous branch of the median nerve (PCBmdn) from the median

INTRODUCTION: Radial nerve palsy is a classical complication of a humeral shaft fracture. In clinical practice, motor palsy of the radial nerve is sometimes observed without an abnormality felt in the sensory territory.
HYPOTHESIS: We hypothesised that this dissociation between sensory and motor involvement is related to anatomical variations of the sensory innervation of the dorsal surface of the first digit space, thus, we decided to study the nature and frequency of these variations.
MATERIAL AND METHOD: A cadaveric study was conducted on 24 upper limbs to analyse the truncal origin of th

BACKGROUND: Intercostal-to-musculocutaneous nerve transfer is commonly performed in patients with brachial plexus avulsion injuries. As techniques have improved since its inception in 1963, most patients now experience some level of motor function improvement of their affected arm. While motor outcomes are well described, there is a paucity of literature describing sensory outcomes. It is thus difficult to gauge surgical success with respect to sensory function, and there is a necessity to share clear expectations with patients regarding intended or unintended postoperative sensation.

PURPOSE: To report the results of open reduction and internal fixation (ORIF) of Bennett fractures in young, active patients using a K-wire and wire loop construct to achieve anatomical reduction and to allow return to manual labor.
METHODS: In this prospective series, we treated 10 male manual laborers (mean age, 30 y; range, 20-44 y) with Bennett fractures diagnosed after a minimum of 12 weeks (mean, 16 wk; range, 12-18 wk). ORIF using 2 K-wires with a wire loop and a neutralizing transarticular K-wire was performed with direct articular visualization. Patients were evaluated for range of m

INTRODUCTION: There are various anaesthetic techniques for ambulatory surgery of carpal tunnel release. The ultrasound-guided nerve blocks offer advantages compared to other techniques. The purpose of this study was to evaluate the efficacy using ultrasound-guided block at the antecubital fossa, as well as to evaluate complications, patient satisfaction and surgeon satisfaction with the block.
MATERIALS AND METHODS: Prospective observational study with 32 elective patients for carpal tunnel release in a one-day case unit. An ultrasound-guided block with mepivacaine 1% was performed at the ant

The lateral antebrachial cutaneous nerve (LACN) is the terminal sensory branch of the musculocutaneous nerve supplying the lateral aspect of forearm. Because of its close proximity to the biceps brachii tendon (BBT), the lateral epicondyle (LE), and the cephalic vein (CV), surgery and venipuncture in the cubital fossa can injure the LACN. Measurement data regarding the relative anatomy of LACN are scarce. We, therefore, dissected 96 upper extremities from 26 males and 22 females to expose the LACN in the cubital fossa and forearm. The LACN consistently emerged from the lateral margin of BBT. I

The superficial branch of the radial nerve (SBRN) is highly vulnerable to trauma and iatrogenic injury. This study aimed to map the course of the SBRN in the context of surgical approaches and identify a safe area of incision for de Quervain's tenosynovitis. Twenty-five forearms were dissected. The SBRN emerged from under brachioradialis by a mean of 8.31 cm proximal to the radial styloid (RS), and remained radial to the dorsal tubercle of the radius by a mean of 1.49 cm. The nerve divided into a median of four branches. The first branch arose a mean of 4.92 cm proximal to the RS, traveling 0.

Treatment of injuries to major nerve trunks in the hand and upper extremity remains a major and challenging reconstructive problem. Such injuries may cause long-lasting disabilities in terms of lost fine sensory and motor functions. Nowadays there is no surgical repair technique that can ensure recovery of tactile discrimination in the hand of an adult patient following nerve repair while very young individuals usually regain a complete recovery of functional sensibility. Post-traumatic nerve regeneration is a complex biological process where the outcome depends on multiple biological and envi

BACKGROUND: We describe a new principle for enhancing the effects of sensory re-education following nerve injury and repair. The outcome from nerve repair in adult patients is generally poor. One reason is the functional cortical reorganisation which always occurs because of axonal misdirection at the repair site. In healthy individuals selective anaesthesia of the forearm results in improved hand sensation. Here we hypothesised that this principle would be valid also after nerve injury and repair.
METHOD: In a prospective, randomised, double blind study we studied the effects of cutaneous fo

The posterior interosseous artery is located in the intermuscular septum between the extensor carpi ulnaris and extensor digiti minimi muscles. The posterior interosseous artery is anatomically united through two main anastomoses: one proximal (at the level of the distal border of the supinator muscle) and one distal (at the most distal part of the interosseous space). In the distal part, the posterior interosseous artery joins the anterior interosseous artery to form the distal anastomosis between them. The posterior interosseous flap can be widely used as a reverse flow island flap because i

The median nerve compressed in the carpal tunnel can be released by an incision in the "line of life", a natural fold along the axis of the third metacarpal bone. By sectioning the carpal tunnel in the palm of the hand, on the cubital side of the median nerve, division of the external branch and of the sensory cutaneous rami of the nerve can be avoided. Hypertrophic scars are prevented. The part of the carpal tunnel situated in the forearm can be freed by a subcutaneous incision if needed, but compression at that level is much less frequent than in the palm of the hand.
